The health professionals revolution


Training a health professional requires a huge effort, it takes motivation, stamina, skills and attitudes. Added to this are expensive teaching resources and a generous amount of time. The medical case is even worse.

For these reasons, it is common that when we arrive at the labour market, we usually work intensively by adding guards, shifts, doublets, part-time supplementary work or private consultations. If a standard 35-hour workday is usually exhausting, imagine if we lengthen it. Too much time spent with hands in the black waters of human suffering takes a heavy toll.  To have a perfect storm add to this the overload conditions imposed by health systems that try to optimise profit or performance to the detriment of the well-being of their workers.

The swell is so powerful that it's starting to crack the structure. A burned-out healthcare professional cannot provide the highest quality care a person needs in sick time. You will be able to provide service, diagnose, treat, apply techniques and care but not build quality encounters, quality conversations or generate narratives that reach the patient core and help them heal.

This implies that patients do not see their problems attended to and return to the system looking for answers. At the level of a certain degree of complexity, it is usual for the patient to end up visiting numerous specialized services where all their devices and systems are reviewed one by one without anyone stopping to see the situation with perspective, to look at the whole, to take into account the totality and complexity of the person. And whoever is supposed to have this overview, which is the family doctor, is kept in the backstage of the system, overloaded to the limit, treating patients every six minutes (in the spanish National Health Service).

These multiple pilgrimages of a patient through the system are not free of charge. In addition to the money they cost, they undergo various diagnostic tests from which they will derive additional treatments that in many cases are nothing more than over-diagnostics and over-treatments that provide little value and benefit to the recipient. Older people, whose main problem is loneliness and poor social support, are often found bearing burdens of more than ten drugs that scarcely alleviate their suffering.

Health professionals complain a lot at coffee time, but they don't stand out for their mobilizations. They know that strikes in their case have little use given the high mandatory minimum service standards that keep the service overburdening those who support it. It is also true that the bargaining between trade unions is rather limited given the enormous inertia of a gigantic system. That's why we're used to pushing the cart a little bit by raising our heads. Proposing a revolution is not on our agenda.

What may be lacking in this scenario is an awareness. The situation of global crisis that we are living in demands adaptations at all levels and it will be the most solid social systems that suffer the most (as Zygmunt Bauman said). Health systems are breaking down as they are unable to balance an upward expenditure line because of the increased complexity and demand for services with another downward revenue line as funding for them is shrinking.

No one dares to make proposals despite the fact that more and more people are realizing the seriousness of the situation. No one likes the alternatives, neither citizens who will have to contribute more money and, above all, greater self-responsibility, nor health professionals who will have to contribute more work with less pay, nor politicians and managers who will have to make unpopular decisions in search of the common good.

The debate and reflection that rethinking health care needs to include all actors and it would be good if it were encouraged both within and outside the system. The level of collective intelligence required is high, as is the level of consensus. That is why it is essential to start from values that are appropriate for the society and time we live in. You have to choose well what is most important and this requires a narrative help, you have to tell it in a way that is understood.

Victor Montori's latest book provides valuable help in taking a good picture of today's healthcare industries. Getting them to evolve into information and knowledge based care and support organizations is the challenge we should all be facing. Pampering the sacred meeting place for health professionals with the capacity to help and all those who need such support is undoubtedly the ultimate goal along with getting each citizen to improve their capacity for self-care and improve their lifestyles.

Flexibility in structures, schedules, agendas, work modes, circuits, programs, protocols... must undoubtedly be the roadmap. To take advantage of the enormous amount of data generated in each interaction to create new systems and ways of working that do not overload the professionals, do not generate greater demand for care and help to self-manage patients without leaving them abandoned to their fate.

Defining well what a health system can and cannot do will be essential for all of us to learn how to use it and for the current abuses to cease. This will involve determined policies and social education actions that seek to include everyone in raising awareness of the problem and designing solutions.

We will have to keep on talking, thinking and talking, but above all we will have to dare to leave our routines towards an exterior full of uncertainty where the answers await us. It is there, open for all, where we can discover the right course. Achieving it will have a lot of value, it is worth taking on the task with confident perseverance.






La revolución de los profesionales de la salud


Formar un profesional de la salud requiere un enorme esfuerzo, hace falta motivación, resistencia, capacidades y actitudes. A esto hay que añadir costosos recursos docentes y una cantidad generosa de tiempo. El caso de lo médicos es peor aún.

Por estas razones es común que cuando llegan al mercado laboral suelen trabajar de forma intensiva añadiendo guardias, turnos, dobletes, trabajos suplementarios a tiempo parcial o consultas privadas. Si una jornada sanitaria estándar de 40 horas es habitualmente agotadora imaginen si la alargamos. Pasar demasiado tiempo con las manos dentro de las negras aguas del sufrimiento humano pasa una elevada factura. Si a esto añadimos las condiciones de sobrecarga que imponen unos y otros sistemas sanitarios que tratan de optimizar beneficio o rendimiento en detrimento del bienestar de sus trabajadores tenemos una tormenta perfecta.

La marejada es tan potente que está empezando a resquebrajar la estructura. Un profesional sanitario quemado no puede prestar la atención de máxima calidad que una persona en tiempo de enfermar necesita. Podrá proveer servicio, diagnosticar, tratar, aplicar técnicas y cuidados pero no construir encuentros de calidad, construir conversaciones o generar narrativas que lleguen al paciente y le ayuden a sanar.

Esto implica que los pacientes no ven sus problemas atendidos y vuelven al sistema buscando respuesta. A nivel de cierto grado de complejidad es habitual que el paciente acabe visitando numerosos servicios especializados donde revisan uno a uno todos sus aparatos y sistemas sin que nadie se detenga a ver la situación con perspectiva, a mirar el conjunto, a tener en cuenta la totalidad y complejidad de la persona. Y quien se supone debería tener esta visión general, que es el médico de familia, es mantenido en la portería del sistema, sobrecargado al límite, atendiendo pacientes cada cinco minutos.

Las peregrinaciones múltiples de un paciente por el sistema no son gratuitas. Además del capital que cuestan lo van sometiendo a diversas pruebas diagnósticas de las que emanarán tratamietnos añadidos que en muchos casos no son más que sobrediagnósticos y sobretratamientos que aportan poco valor y beneficio a quien los recibe. Es frecuente encontrar personas mayores cuyo principal problema es la soledad y el poco apoyo social soportando cargas de más de diez medicamentos que alivian escasamente su sufrimiento.

Los profesionales de la salud se quejan mucho mientras toman café pero no se destacan precisamente por sus movilizaciones. Saben que las huelgas en su caso tienen poca utilidad dado los elevados servicios mínimos obligatorios que mantienen el servicio sobrecargando a los que los sostienen. También que la negociación que los sindicatos mantienen es bastante limitada ante la enorme inercia de un sistema gigantesco. Por eso están acostumbrados a empujar el carro levantando poco la cabeza. Proponer una revolución no está en su orden del día.

Lo que tal vez falte en este panorama es una toma de consciencia. La situación de crisis global que vivimos exige adaptaciones a todos los niveles y serán los sistemas sociales más sólidos los que más sufran. Los sistemas de salud se están rompiendo al no ser capaces de equilibrar una línea de gasto ascendente por el aumento de complejidad y demanda de servicios con otra línea de ingresos descendente al mermar la financiación de los mismos. Matemáticamente no hay solución con las ecuaciones actuales, a nivel de alta gestión se ponen parches y se tratan de pintar los desconchones pero el barco cada vez está más hundido.

Nadie se atreve a hacer propuestas pese a que cada vez más gente se da cuenta de la gravedad de la situación. Las alternativas no gustan a nadie, ni a ciudadanos que van a tener que aportar más dinero y sobre todo mayor autoresponsabilidad, ni a profesionales de la salud que tendrán que aportar más trabajo con menos sueldo, ni a políticos y gestores que buscando el bien común van a tener que tomar decisiones impopulares.

El debate y la reflexión que repensar la sanidad precisa ha de incluir a todos los actores y sería bueno que se alentara tanto desde dentro del sistema como fuera del mismo. El nivel de inteligencia colectiva que se precisa es elevado, el nivel de consenso también. Por eso es fundamental partir de unos valores apropiados para la sociedad y el tiempo que vivimos. Hay que elegir bien lo que es más importante y esto precisa una ayuda narrativa, hay que contarlo de una manera que se entienda.

El último libro de Victor Montori aporta una ayuda valiosa al hacer una buena fotografía de las industrias sanitarias actuales. Conseguir su evolución a organizaciones de atención y cuidado basadas en información y conocimiento es el reto que todos deberíamos ponernos. Mimar al máximo la zona sagrada de encuentro entre los profesionales de la salud con capacidad de ayudar y todos los que precisan dicho apoyo es sin duda el máximo objetivo junto con conseguir que cada ciudadano mejore su capacidad de autocuidado y sus estilos de vida.

Flexibilizar estructuras, horarios, agendas, modos de trabajo, circuitos, programas, protocolos... ha de ser sin duda la hoja de ruta. Aprovechar el enorme caudal de datos que se generan en cada interacción para conseguir crear nuevos sistemas y formas de trabajo que no sobrecarguen a los profesionales, no autogeneren mayor demanda de cuidados y ayuden a autogestionarse a los pacientes sin dejarles abandonados a su suerte.

Definir bien lo que un sistema sanitario puede y no puede hacer será básico para que todos aprendamos a usarlo y dejen de producirse los abusos actuales. Esto implicará políticas determinadas y actuaciones de educación social que traten de incluir a todos en la toma de consciencia del problema y en el diseño de soluciones.

Habrá que seguir hablando, pensando y dialogando, pero sobre todo habrá que atreverse a salir de nuestras rutinas hacia un exterior lleno de incertidumbre donde nos esperan las respuestas. Es ahí, a la vista de todos, donde podremos descubrir el rumbo adecuado. Conseguirlo tendrá mucho valor, merece la pena tomarnos la tarea con confiada perseverancia.
