De doctor Google a doctor Chatgpt. From doctor Google to doctor Chatgpt.

¿Sustituirán a enfermeras y médicas?

From doctor Google to doctor Chatgpt.

It is very common that before going to the doctor, patients consult their symptoms on Google or any other search engine. Help is often scarce given the variable quality of the information that the search engines provide, which causes more anxiety to those who involuntarily come across words such as cancer or frightening names when searching.

This situation is set to change with artificial intelligence algorithms that will eventually find their way into our screens, mobiles and watches. Virtual health assistants will be one of the biggest inventions in the coming months and will proliferate like mushrooms at the hands of the big technology companies.

Among the current systems, Chatgpt stands out as it can be used for almost anything and tends to give quite reasonable answers, such as the example I bring you.

Will they replace nurses and doctors? Probably not, but if they are designed and used well, they may help to solve minor problems and prevent a cold from ending up in the hospital emergency room or a pimple from ending up as a walk-in consultation at the health centre.

Public health systems will eventually incorporate it, but it will be long after we all have a similar system in our pockets.

At the same time, we will recommend it to our politicians and health managers, who, if they would listen, would surely improve our ailing health system.

Ruido y pantallas. Noise and screens.

Noise and screens

The time of our parents was the time of light bulbs and neon that transformed the night in cities and homes. One screen per family in the living room and the radio on in the kitchen. Nowadays we live surrounded by screens and the noise is deafening. A few seconds of watching a news bulletin is enough to make you shudder. On the one hand there is maximum distraction with the lights permanently on and on the other the dense darkness of unbridled fear and horror. And sure enough, being whipped into a frenzy of bad news and black uncertainty, we are thrown like mad into a maelstrom that distracts us from what we think and feel. We prefer to swallow preconceived, sweetened and conveniently sanitised ideas that someone fuzzy is spoon-feeding us as we browse our social networks and butterfly from flower to flower on the internet. Or experience the tempered pleasures of our favourite series or the influencer of our choice. All before facing the tangled disquiet of our feelings, the intuitions we don't care to hear, or the terrible feeling of inner emptiness that awaits us at the end of the abyss. Never before have we been so distracted, never before have we been so far from ourselves.

What we don't know is that this makes us ill, or more accurately, it makes us unhealthy. Everyone understands that it is unhealthy to eat only burgers from McDonald's and that if you overdo it, you will end up with indigestion. What we don't want to assume is that the same is true of the cognitive banquet that our screens offer us every day with a brutal excess of information of poor quality for our fragile mental health. The human brain is designed to spend the day wandering around the countryside looking for a living, not locked up in a dungeon staring at screens. Seclusion robs us of muscle mass, fitness and sanity. And it does not allow us to make even the slightest contact with the shadow, since light, even artificial light, has the habit of eliminating darkness.

The shadow is that dark part that is inevitably produced when we illuminate an object. Carl Gustav Jung realised that in our inner world there is also one that comes to the fore when we walk through it with the lit candle of consciousness. It is that dense and impenetrable part that we strive to keep in the depths of inner wardrobes and that sometimes cries out to be released from them. There we deposit desires, yearnings, identities, feelings, ideas, guilt, shame, pain, suffering, rejections... and as it remains stored and folded up we do not allow the internal purification system to release it when we effortlessly flush the toilet. It ends up producing bad smells and sours our mood and hope. In the end we lose energy and the meaning of life because keeping a cupboard closed that is trying to open is very tiring and not recognising who we are makes us lose our footing.

But there we are, with our problems, filling our eyes with memes about the president of the government or Miguel Bosé's latest scandal, while inside we bleed to death and refuse to look at where the wound is. In the end the body or our psychology stops us, the former with discomfort and symptoms, the latter with anxiety and sadness. And we end up at the family doctor's asking for a blood test because we feel exhausted. While he listens to us and realises what is going on, he sighs deeply, asks us for the analysis and doesn't say anything because he would need twenty minutes and he only has three to attend to us. And because our sludge will not put our physical integrity at risk and, in the end, we have to be the ones to purify it. The problem is that nothing and nobody helps. The others are also abducted with their own things and we don't usually pay a psychologist for this. Therefore, we leave the consulting room with our paper , we do the analysis whose results will be perfect and we keep turning the wheel, like the mouse in its cage, to see if we can relax the bad feeling.

Until we don't have a real life crisis, we won't do anything. It doesn't matter if petrol rises, the heat wave persists, a war breaks out in Crimea or Elvis Presley resigns. We will only react when we feel suffocated, and if necessary we will go back to the family doctor to ask for a pill to take away the burden, so that we can keep on going for another little while. And if the doctor is fuming that day, he or she will quickly extend it to us so that the next one, which will probably be worse, can pass.

Epidemia de miedo. An epidemic of fear.

An epidemic of fear

Fear is a contagious emotion. It spreads through families, neighbourhoods and towns, and in our globalised era we see it at its most extreme. And after experiencing its various manifestations in the form of anxiety, oppression, discomfort, a lump in the throat, a tight stomach, cervical contracture and a long list of possibilities, its effects will follow. Social withdrawal, increased consumption of intoxicants, increased consumption in general, disaffection with political or social issues, a certain clouding of meaning and a desire to distract oneself and escape.

It is an ancient form of social control that is manipulated at will by those in power. We have had two years of pandemic and now we are starting with a war in Europe, against the backdrop of the climate and economic threat. Will there be any antidote (not to say vaccine, which is a rather questionable word)?

I would point out that anything that helps us to strengthen our hope, confidence, connection with others and with ourselves will be of great help. For this, a certain degree of screen fasting is essential. Without limiting negative news, it is impossible to maintain sufficient serenity. This also means a short amount of social networking and instant messaging services. To connect with others I would advise going back to the phone call, the sms text message or, better still, the postcard. Walks are another great way to resist adversity, if it is in a park or natural environment, all the better. And the slow conversation with someone close to us, words are still the supreme ingredient to try to get closer to what we call reality.

Everyone has their own resources in this respect, so I won't go any further. I would only encourage us to look at ourselves a little every day and recognise the emotions that may be present. If we detect an excess of fear, we will have to adjust our level of emotional hygiene to recognise it, accept it and, when the time comes, flush it away.

Take heart and take care of yourselves.

Salud egocéntrica. Self-centred health.

 Self-centred health.

Health is not wellness. Unfortunately, all the screens seem to say the opposite. The artificial paradises they sell us, the smiling models and celebrities, the high life of those celebrities on the Olympus of fame, power and pleasure... all point to wellness. And of course to the goods and services that make it possible. "Paradise on earth exists" we are told and then invited to check out.

But then what do I do with my life when I get a pimple, a sore neck or my period? More and more often we turn to the health system looking for a pill to get rid of any setback, whether it is a minor health issue, a problem with the boss or the sadness of having lost a loved one. Everything tends to be medicalised in order to regain lost wellbeing as quickly as possible. Unfortunately, the shadow of old age, illness, deterioration and progressive loss of function cannot be entirely removed. The inherent discomfort of life is as close to us as the shadow is to any body that receives a light source.

What we are seeing is that this egocentric, playful and pleasurable health trend is one of the factors that is bursting European health systems that cannot cope with the increasing pressure of healthy people consulting for various ailments.

There will be no budget to solve this problem if we as a society do not realise the fallacy they want to sell us. The bad news is that the market is not going to make it easy for us by relying on an advertising and entertainment industry whose arguments look solid. All those happy-looking faces in the media and on social media are very convincing because when they get sick or look bad we don't see them. We want to feel good and be as productive, strong and beautiful as we can be. When health is not 100% with us, we don't cope well. Suffering is not fashionable and has no value. Neither is illness and as far as possible we try to hide it. In this postmodern society it is paradoxical that despite the progressive wellbeing that some societies enjoy, we feel more and more tired and exhausted. Perhaps we need to relearn to carry our own shadow with dignity, as our elders did and as all the relatives before us have done. Accepting that life involves a dose of discomfort and that it is not always possible to be in top form is a challenge today. Learning that health is more than just being well is also a challenge.

Consumir salud. Consuming health.

Dibujo De Fran Ferriz

Consuming health.

Turning health into a consumer product has ended up transforming healthcare into a huge business. Public health systems have not been able to keep out of this current and we are increasingly close to being swallowed up in a whirlpool of rapid spinning.

The market is speeding up all processes and decreasing tolerance for the inherent discomfort of life. This means more consultations and more consumption of health products. The fuse of the system is the family doctor who is supposed to solve 90% of health problems and refers only the complex to the hospital level. But what happens if we overload and overrule family doctors? Well, the system is accelerated by increasing the hospital workload: more processes, more diagnostic interventions, more treatments. And at every turn of the screw the population becomes more dependent on the system and less self-sufficient.

If the people who work in the health sector do not have sufficient resources and time to carry out their work, their responses will not be sufficient and the public system ends up collapsing, forcing many people to seek their living through private health insurance. In this way, the market wins and society and the individual loses.

This situation is not easy to reverse. We need resources, but above all awareness. We need to realise that health is something profoundly important that must be cared for and valued but not commodified. This means taking individual responsibility for looking after ourselves and our families while learning to make prudent use of health resources. It also means taking care of social health through public health measures, caring for the environment and enhancing community relations.

Technology may provide specific and timely improvements in the health field, and we must take advantage of them, but without harbouring the fantasy that they will fix our lives and solve our illnesses. It seems coherent to invite us as a society to reflect on what health means to us and what we should do to maintain it. If we are not clear about this, it will be the market that will respond.

Resbalar. Slide. 滑動

A few months ago I slipped on my street and came out projected to the left. For a moment I lost my footing and experienced a pleasant sensation of flight and weightlessness that the impact on the ground immediately erased. As I haven't forgotten the pain, this week of snow and ice I take all the precautions and go out into the street with mountain shoes and sticks. Despite its bad press, pain and fear are valuable instruments of survival, something that our society sweetens and in doing so makes us helpless without many knowing it.

We are not used to the unexpected. Despite having been on the verge of a nervous breakdown due to a succession of adverse events for almost a year, we still do not realise how important it is to be alert and to react. The basic thing is to read the events and prepare ourselves before they reach us. Winter remains a powerful adversary just like viruses. And these weeks will remind us of that fact. Many people suffer from energy poverty and many others from advanced age for whom the harsh winter is a test of survival that some will not be able to overcome.

Delegating everything to politicians and decision-makers is not enough. I refer to the evidence. A society's ability to adapt depends crucially on its citizens being awake. If the level of numbness is high, we are lost. That is why it is vital to counteract the images of mindless people that reach us on our screens through personal acts that add value to others or at least care for or protect us.

Wearing double socks and clothes suitable for the temperature, maintaining an acceptable degree of mobility, being careful if there is ice on the pavement, taking an interest in the elderly in the family or neighbourhood who may need help with errands, coordinating with other neighbours to remove snow from the street... these are acts that may have little visibility but which build humanity and civilisation. I think it is worthwhile to delicately impregnate our agenda and take care of the small details, those that in the end are the ones that end up building what we call reality.

Preparar la Navidad. Preparing Christmas.

Preparing Christmas

This year we will have a different Christmas. That's why it's worth taking a moment to plan and prepare for it in the best possible way. I'd like to share a few ideas to discuss as a family and make the best of it.

1. Get to know the recommendations of the authorities, but don't spend too much time listening to the opinions of experts, talkers and brothers-in-law.

2. Focus on the main thing: protecting the most vulnerable, your family and yourself.

3. Risk cannot be avoided but can be managed. When planning meals or family gatherings it is essential. But don't forget every social interaction you have these days. It is prioritary that adults, adolescents and children plan their social interactions. If we are going to meet with grandparents or someone sick, it is essential that we all make the most of our isolation the week before.

4. Let's take advantage of technology to communicate with written or audio messages, send photos, videos, songs made by us. Let's make as many video calls as we can. Let's send letters, cards and gifts by mail...

5. The best meetings are short, ventilated or outdoors and with few close friends.

6. Walks in the open air, yes, through the countryside or nature better. As much as possible.

7. High doses of beauty (music, art, a flowering plant...) are preferable to overloads of tension. Turn off your screens as much as you can and kiss or hug generously to your fellow human beings.

And if you want some more structured advice, follow the following guide, made by professionals I trust.

https://www.mscbs.gob.es/en/profesionales/saludPublica/ccayes/alertasActual/nCov/documentos/Recomendaciones_mascarillas_ambito_comunitario.pdf

"They are “moderately” certain that social distancing and the wearing of masks are likely to produce a large decrease in COVID-19 transmission".

https://sciencebasedmedicine.org/do-face-masks-decrease-the-risk-of-covid-19-transmission/

"The current best evidence includes the possibility of important relative and absolute benefits of wearing a facemask".

https://www.thelancet.com/journals/lancet/article/PIIS2213-2600(20)30352-0/fulltext

The masking should be recommended as the best medical preventive measure, and the community must be encouraged to use it.

https://www.bmj.com/content/370/bmj.m3021/rr-5

https://www.acpjournals.org/doi/10.7326/M20-6817
