La concha. The shell. 贝壳






Los bivalbos son seres acuáticos de carnes blandas que tienden a vivir protegidos de las adversidades dentro de una concha calcárea que mantienen cerradas y a ratos abren para aspirar las partículas que las corrientes les proveen. Creo necesario poner un momento nuestra atención en ellos para mirarnos como en un espejo, pues al contemplarlos vemos millones de años de evolución biológica en un diseño simple y robusto capaz de sobrevivir meteoritos, maremotos y cambios climáticos, algo que quizá nuestra sofisticada especie no consiga. 


De alguna forma en nuestra sociedad de la información nos estamos convirtiendo en bivalvos, seres metidos dentro de una burbuja rígida que pese a poder vivir rodeados de otros muchos pierden la habilidad de compartir el mismo espacio. Quizá aspiremos las mismas aguas y partículas parecidas, aunque cada cual sienta que su menú de contenidos es exclusivo. Lógicamente es una falacia, las compañías que nos proveen de información la trocean según nuestro gustos pero con un poco de distancia queda claro que es la misma sopa para todos. De esta forma cada cual pasa sus días filtrando litros y litros de agua salada para al final conseguir un puñado de migajas que nos permita seguir con vida digital. 


Hoy escuchaba a un adolescente decir que se había quitado de Instagram, harto de consumir la misma dieta insípida. Recordé la que tenía yo con su edad a base de los primeros videojuegos que llegaron a los primeros ordenadores y consolas, un alimento terrible que nos encantaba a todos y que consumimos hasta el hartazgo más absoluto que nos hizo retornar poco a poco al mundo real. Me temo que hoy la situación es más complicada y pocos tienen la autentica capacidad de apagar definitivamente sus aparatos electrónicos. Lo que veo más bien es que las burbujas que nos rodean, que inicialmente eran plásticas, transparentes y flexibles, se van convirtiendo en conchas rígidas que remedan la del mejillón. Y es que lo que escuchamos cada vez es menos diverso y más rígido, áspero y moral. Las redes sociales nos etiquetan y de este modo nos clasifican y dividen. Azules frente a rojos, amarillos frente a morados, listos frente a tontos, morales frente a amorales, y así hasta el infinito. La plaza digital es pues una bronca permanente que genera ruido y peleas infinitas que se terminan pegando al exterior de nuestras conchas añadiendo una capa de un apestoso chapapote que nos anima a mantenernos más cerrados si cabe. De dialogar nos estamos olvidando, de abrirnos a la diversidad también. Esta pérdida de tolerancia a lo distinto es de gran toxicidad, perdemos la capacidad de pensar por nosotros, sin las ideas no propias no podemos seguir construyendo nuevas propuestas. 


Los que tengan la protección de una familia o comunidad humana alrededor y quizá la suficiente cultura y sensibilidad quizá puedan defenderse de la marea negra que aparentemente nos rodea. Nunca habían sido tan necesarias las viejas narrativas de la literatura, la música, las demás bellas artes, la historia y la filosofía. Necesitamos recurrir a todos esos sabios que nos legaron desde antiguo las cartografías de la mente y el corazón humanos. La sabiduría en piedra de aquellos templos, las pinturas al fresco o en vasijas, los objetos, la arquitectura, todos sus intentos de hacer perdurar un mensaje que sigue siendo válido porque sigue hablando de la dificultad humana. Queremos ser dioses y no damos la talla. Queremos desear como dioses, consumir como dioses, ser honrados como dioses. Y al final nos comportamos como un puñado de monos agresivos enseñando los dientes o golpeándonos unos a otros con garrotes. Al final terminamos metiendo nuestro orgullo dentro de una concha, y cerrando la puerta, sin imaginar que al hacerlo nos morimos en vida. 




The shell

Today I was listening to a teenager say that he had quit Instagram, tired of consuming the same bland diet. I remembered what I had at his age based on the first video games that came to the first computers and consoles, a terrible food that we all loved and that we consumed to the point of absolute satiety that made us return little by little to the real world. I fear that today the situation is more complicated and few have the true ability to permanently turn off their electronic devices. What I see rather is that the bubbles that surround us, which were initially plastic, transparent and flexible, are becoming rigid shells that mimic that of the mussel. And what we hear is less and less diverse and more rigid, harsh and moral. Social networks label us and in this way they classify and divide us. Blues versus reds, yellows versus purples, smart versus stupid, moral versus amoral, and so on ad infinitum. The digital plaza is therefore a permanent fight that generates noise and infinite fights that end up sticking to the outside of our shells, adding a layer of stinking chapapote that encourages us to remain more closed if possible. We are forgetting to dialogue, to open ourselves to diversity as well. This loss of tolerance to what is different is highly toxic, we lose the ability to think for ourselves, without ideas that are not our own we cannot continue building new proposals.


Those who have the protection of a family or human community around them and perhaps sufficient culture and sensitivity may be able to defend themselves from the oil spill that apparently surrounds us. Never have the old narratives of literature, music, the other fine arts, history and philosophy been so necessary. We need to turn to all those wise men who bequeathed us from ancient times the cartographies of the human mind and heart. The wisdom in stone of those temples, the fresco paintings or on vessels, the objects, the architecture, all their attempts to make a message endure that remains valid because it continues to speak of human difficulty. We want to be gods and we don't measure up. We want to desire like gods, consume like gods, be honored like gods. El arte de contar. The art of storytelling. 计数的艺术





El lunes pasado atendí dos presentaciones. La primera fue una reunión por Zoom que impartía la gerencia de Atención Primaria en nuestro centro cabecera. Alguien de algún despacho recorría los centros de salud del área hablando de cuestiones corporativas con un power point. Aguanté media hora. Las diapositivas densas, la exposición también. 

Por la tarde me animé a ver la keynote de Apple, la primera que he visto. Dos horas y pico en formato cinematográfico viendo a ingenieros de la compañía vender todo tipo de productos. No sé lo que les habrá costado en dinero y en horas pero he de reconocer que fue una obra maestra del marketing. El actual presidente de la compañía no tiene el carisma ni el ego de Steve Jobs, gran amante del directo, y optó por una gran producción audiovisual de primera calidad. Consiguieron que no fuera aburrido pese a la gran cantidad de aspectos promocionados y probablemente elevar el precio de las acciones y las perspectivas de ventas.  

Como médico de Atención Primaria me fije en las técnicas de comunicación, tanto verbal como no verbal y en la ambientación, juego de imágenes y demás efectos facilitadores de la venta de productos e ideas. Siempre he defendido que el marketing debería ser asignatura obligatoria en toda carrera de ciencias de la salud donde una de las habilidades del clínico ha de ser “vender” y explicar ideas a compañeros y pacientes. 

Mi reflexión tras sopesar las dos presentaciones es constatar el bajo nivel de comunicación corporativa de las organizaciones sanitarias grandes, donde se presta muy poca atención y cuidado a la gestión de información y la mejora y potenciación de la comunicación interprofesional, y el altísimo valor de los mensajes de las grandes empresas privadas cuyo poder no deja de crecer gracias a ello. 

Al intercambiar puntos de vista con la médico residente que rota este mes en nuestro centro parece que el estilo comunicativo de la docencia pre y postgrado sigue siendo del nivel de la generación anterior: larguísimas presentaciones PowerPoint infumables impartidas por clínicos sin mucha preparación docente.  Y así seguimos porque los cursos que reciben siguen esa línea y las presentaciones de sesiones clínicas o casos que ella y sus compañeros hacen no distan mucho de las de sus maestros. 

La sugerí que echara un vistazo a los de la manzana. Me parece que no lo hará. 





 The art of storytelling


Last Monday I attended two presentations. The first was a Zoom meeting given by the Primary Care management in our primary care centre. Someone from some office was going around the health centres in the area talking about corporate issues with a power point. I endured half an hour. The slides were dense, and so was the presentation. 

In the afternoon I was encouraged to watch the Apple keynote, the first I had ever seen. Two hours and a bit in cinematic format watching the company's engineers selling all kinds of products. I don't know what it must have cost them in money and hours, but I have to admit that it was a masterpiece of marketing. The current CEO of the company doesn't have the charisma or ego of Steve Jobs, a great lover of live shows, and opted for a great audiovisual production of the highest quality. They managed to keep it from being boring despite the many aspects promoted and probably boost the share price and sales prospects.  

As a primary care physician, I paid special attention to communication techniques, both verbal and non-verbal, and to the ambience, the play of images and other effects that facilitate the sale of products and ideas. I have always advocated that marketing should be a compulsory subject in every health sciences degree where one of the skills of the clinician has to be "selling" and explaining ideas to colleagues and patients. 

My reflection after weighing up the two presentations is to note the low level of corporate communication in large healthcare organisations, where very little attention and care is paid to information management and the improvement and enhancement of interprofessional communication, and the very high value of the messages of large private companies whose power is constantly growing thanks to this. 

When exchanging points of view with the resident doctor who is rotating at our centre this month, it seems that the communicative style of undergraduate and postgraduate teaching is still at the level of the previous generation: very long, unbearable PowerPoint presentations given by clinicians without much teaching preparation.  And that's still the case because the courses they receive follow that line and the presentations of clinical sessions or cases that she and her classmates give are not very different from those of their teachers. 

I suggested that she take a look at the ones on the block. I don't think she will.

¿Comunicar o no comunicar? To communicate or not to communicate? 沟通还是不沟通?沟通还是不沟通?




Tras asistir al congreso de comunicación de la Semfyc tras dos años sin actividad comparto mi alegría y alguna reflexión.


El congreso tuvo lugar en el Escorial y convocó a 350 asistentes con un formato de talleres y mesas participativas. Asistí a cuatro talleres (de 2,3 y 4 horas, sobre actividad grupal para pacientes, medicina narrativa, metodología con infografías para dolor crónico y desarrollo artístico) y defendí una comunicación oral sobre el ruido y una mesa de 3 horas sobre el silencio (por cierto tuve que hacer ambas cosas al mismo tiempo, pero eso ya lo contaremo otro día). El nivel, como suele ser habitual, muy alto, tanto en ponentes como en participantes, y esto lo digo por el alto valor de los encuentros de pasillo, comidas y demás que añaden un gran plus a este tipo de encuentros. 


Hoy con un poco de perspectiva, tras alejarme varios cientos de kilómetros por causa mayor (me perderé la conferencia de clausura y las valiosas reseñas bibliográficas de Francesc Borrell) puedo preguntarme si a nuestro sistema sanitario y a nuestra sociedad les hace falta más comunicación o menos, en tiempos de tanto ruido. 


Y empezaré mirando la viga en mi propio ojo, ¿Qué es lo que los profesionales sanitarios no queremos escuchar y lo que no queremos decir?


Probablemente lo que más nos cueste escuchar es lo evidente, los tiempos han cambiado y el sistema sanitario no da las suficientes respuestas ni a unos ni a otros. La sociedad ha cambiado y demanda otras cosas a sus profesionales, quieren acceso rápido, respuestas rápidas, soluciones rápidas. La política ha cambiado y al ser cada vez más cortoplacista solo acomete acciones con rédito electoral, delegando todo lo demás en el siguiente… Probablemente a muchos se les habrá caído la venda durante la pandemia y habrán comprendido por fin que nuestra sanidad no tiene arreglo, como tampoco lo tiene el corazón dilatado e insuficiente del nonagenario al final de su vida. Pero no olvidemos que incluso ese corazón puede seguir amando y ser amado. 


También hay que fijarse en lo que no queremos decir, que suele ser amargo como toda verdad que señala una herida. No queremos decir que el sistema no funciona y no tenemos fuerza ni valor para cambiarlo. Que el pronóstico de la enfermedad social sigue empeorando, no se tolera la frustración, el sistema económico social cada vez maltrata más a sus ciudadanos y hay una infantilización y aumento de dependencia en estos a la hora de tomar conciencia de sus verdaderos problemas y asumirlos, al igual que un horror al enfrentarse al sufrimiento que la propia vida conlleva por ser vida y que se trata de medicalizar y convertir en un negocio más. 


Lo que los pacientes no quieren escuchar es que tienen la capacidad y la obligación de responsabilizarse de manejar las catástrofes vitales que les toquen, ya sean problemas laborales, de convivencia o del estado de ánimo, en lugar de buscar soluciones mágicas en tratamientos medicalizados que nunca podrán restituir al familiar fallecido, reconvertir la relación con el jefe o traer de vuelta a una pareja que nos ha abandonado. Y probablemente los pacientes tampoco quieran decir y asumir que el modo de vida contemporáneo es de locos, que no se puede ir por la vida tan acelerado y despistado. Que somos cutres a la hora de cuidarnos y cuidar a los nuestros, de regalarnos paseos por el campo, de mirar a la luna.


Por eso me parece pertinente que un grupo de profesionales de la salud organicen un foro anual y se encuentren para defender una tecnología que no está de moda pero es potentísima como es la comunicación. Hoy nos tratan de vender complejos aparatos, pantallas, ordenadores mientras nos privan del tiempo necesario para poder comunicar, algo que requiere escucha y reflexión. Algo que requiere una suficiente paz de ánimo que una consulta de 50 pacientes no suele otorgar. 


Si los profesionales de la salud dejamos de escuchar en condiciones la sociedad tendrá un nuevo problema, y a nivel global es lo que está ocurriendo. Profesionales cada vez más agobiados, corren y se agitan en sus centros yendo de un paciente a otro, y cada vez hay más y es más larga la fila. Solo una gran toma de conciencia que permita cambios sociales de fondo permitirá hacer los correspondientes ajustes de fondo en la sanidad. Mientras tanto el reto es mantener la cordura. Este congreso ayuda, pero habrá que ser muy creativo para encontrar refuerzos.  







To communicate or not to communicate?



After attending the Semfyc communication congress after two years without activity, I would like to share my joy and some reflections.


The congress took place in El Escorial and was attended by 350 participants in a format of workshops and round tables. I attended four workshops (of 2, 3 and 4 hours, on group activity for patients, narrative medicine, methodology with infographics for chronic pain and artistic development) and defended an oral communication on noise and a 3-hour table on silence (by the way I had to do both at the same time, but I will tell you about that another day). The level, as usual, was very high, both in terms of speakers and participants, and I say this because of the high value of the corridor meetings, meals and so on, which add a great plus to this kind of meetings. 


Today, with a bit of perspective, after having moved several hundred kilometres away due to a major reason (I will miss the closing conference and the valuable bibliographic reviews by Francesc Borrell), I can ask myself whether our health system and our society need more communication or less, in times of so much noise. 


And I will start by looking at the beam in my own eye: what is it that we health professionals don't want to hear and what is it that we don't want to say?


Probably what we find most difficult to hear is the obvious, times have changed and the health system does not give sufficient answers to either one or the other. Society has changed and demands other things from its professionals, they want quick access, quick answers, quick solutions. Politics has changed and, being increasingly short-sighted, it only undertakes actions for electoral gain, delegating everything else to the next person... Many will probably have dropped the blindfold during the pandemic and will have finally understood that our healthcare system is beyond repair, just as the enlarged and insufficient heart of the nonagenarian at the end of his or her life is beyond repair. But let us not forget that even that heart can still love and be loved. 


We must also pay attention to what we do not want to say, which is often bitter like any truth that points to a wound. We do not want to say that the system does not work and that we have neither the strength nor the courage to change it. That the prognosis of the social disease continues to worsen, frustration is not tolerated, the social economic system is increasingly mistreating its citizens and there is an infantilisation and increased dependence on them when it comes to becoming aware of their real problems and assuming them, as well as a horror when faced with the suffering that life itself entails because it is life and that it is being medicalised and turned into just another business. 


What patients do not want to hear is that they have the capacity and the obligation to take responsibility for managing their own life catastrophes, be they problems at work, living together or mood problems, instead of looking for magic solutions in medicalised treatments that will never be able to restore the deceased relative, reconvert the relationship with the boss or bring back a partner who has abandoned us. And patients probably don't want to say and accept that the contemporary way of life is crazy, that you can't go through life so fast-paced and absent-minded. That we are lazy when it comes to looking after ourselves and our loved ones, to taking walks in the countryside, to gazing at the moon.


That is why it seems appropriate to me that a group of health professionals should organise an annual forum and meet to defend a technology that is not fashionable but is very powerful, such as communication. Today they try to sell us complex devices, screens, computers, while depriving us of the time we need to communicate, something that requires listening and reflection. Something that requires a sufficient peace of mind that a consultation with 50 patients does not usually provide. 


If we health professionals stop listening properly, society will have a new problem, and on a global level this is what is happening. Increasingly overburdened professionals are running and bustling around their centres going from one patient to another, and there are more and more of them and the queue is getting longer. Only a major awareness-raising that allows for fundamental social change will allow the corresponding fundamental adjustments to be made in the healthcare system. In the meantime, the challenge is to maintain sanity. This congress helps, but we will have to be very creative to find reinforcements.

Silencio. Silence. 安靜

Tras muchos años escribiendo un blog y participando en redes sociales me voy dando cuenta de que cada vez hago menos ruido. Es inevitable mantener un mínimo, pero parece objetivo decir que abro menos la boca. Conozco muchos casos de compañeros que durante este tiempo desaparecieron, cansados, quemados o directamente contrariados. De un día para otro ya no estaban. Siempre entendí que es una posibilidad, una de las pocas que nos demuestra que tenemos algún pequeño grado de libertad en la recámara. En mi caso tengo varias razones, la primera tiene que ver con la pandemia que como a todos me alcanzó como una gran ola marina, me derribó, me dio bastantes vueltas y me hizo tragar una considerable cantidad de agua salada. Tras las marejada oceánica, o más bien de comparsa, vino un viento furioso y cansino de grito y de ruido. Los medios de comunicación, las pantallas y las redes sociales se pusieron como locas en una espiral de opiniones, descalificaciones, tontunas y demás que azuzaron el miedo y el enfado hasta límites nunca vistos. Ante tanto desvarío muchos nos tuvimos que activar en modo supervivencia, y esto implica cultivar y expandir silencio.

La segunda razón tiene que ver con la impotencia al relacionarme con mi trabajo y con la sociedad. Esa sensación de no poder hacer pese a ver el desastre aproximarse. Esa comprensión que te obliga a entender que el barco se hundirá pase lo que pase. Lo he visto de primera mano en mi centro de salud. Ni siquiera una pandemia como la que hemos vivido ha servido para que nos refuercen, potencien o reformen. No hubo rastreadores, fuimos nosotros, no hubo refuerzos de médicos y enfermeras, tuvimos que desdoblarnos y deslomarnos, no hubo mejoras de gestión, ni reducción de la burocracia, ni nuevos planteamientos estratégicos, hemos seguido haciendo lo que buenamente hemos podido. La sanidad se ha manifestado a las claras como una sanidad zombi, ni viva ni muerta. Fiel reflejo de una sociedad que a su vez avanza en un proceso de “zombificación” y en la que las instituciones y la política se van alejando de la vida para convertirse en entes grises cada vez más incapaces de dar el servicio para el que fueron encomendadas.

Y la tercera razón que pude argüir tiene que ver conmigo. Con la profunda necesidad de callar para alejarme del bullicio de la costa y remar mar adentro. Y mar adentro, como en el desierto, uno inevitablemente tiene que vérselas con sigo mismo. En esa bastedad vacía descubrimos que estamos habitados por una presencia que nos trasciende, algo difícil de nombrar pero dotado de una fuerza fundante que mana como fuente y que al beberla nos regala fuerza, vida y sentido. 


En estos tiempos lo fácil es unirse a la turba y gritar sin parar. No me busquen ahí. Nunca fue mi sitio. Si quieren encontrarme estaré por despoblado, caminando quizá mirando nubes. 






Silence

After many years writing a blog and participating in social networks I am realizing that I am making less and less noise. It is inevitable to maintain a minimum, but it seems objective to say that I open my mouth less. I know many cases of colleagues who during this time disappeared, tired, burned out or directly disgruntled. From one day to the next they were gone. I always understood that it is a possibility, one of the few that shows us that we have some small degree of freedom. In my case I have several reasons, the first has to do with the pandemic that, like everyone else, hit me like a big ocean wave, knocked me down, tossed me around a lot and made me swallow a considerable amount of salt water. After the oceanic tidal waves, or rather as a companion, came a furious and tiring wind of shouting and noise. The media, the screens and the social networks went crazy in a spiral of opinions, disqualifications, nonsense and others that whipped up fear and anger to unprecedented limits. Faced with so much raving, many of us had to activate survival mode, and this implies cultivating and expanding silence.

The second reason has to do with the impotence when relating to my work and to society. That feeling of not being able to do anything despite seeing disaster approaching. That realization that forces you to understand that the ship will sink no matter what. I have seen it firsthand in my health center. Not even a pandemic like the one we have lived through has served to strengthen, empower or reform us. There were no trackers, it was us, there were no reinforcements of doctors and nurses, we had to split up and get out of the way, there were no improvements in management, no reduction in bureaucracy, no new strategic approaches, we have continued to do what we have been able to do. The healthcare system has clearly shown itself to be a zombie healthcare system, neither alive nor dead. A true reflection of a society which in turn is advancing in a process of "zombification" and in which institutions and politics are moving away from life to become gray entities increasingly incapable of providing the service for which they were entrusted.

And the third reason I could argue has to do with me. With the deep need to be quiet in order to get away from the hustle and bustle of the coast and row out to sea. And out to sea, as in the desert, one inevitably has to deal with oneself. In that empty wasteland we discover that we are inhabited by a presence that transcends us, something difficult to name but endowed with a founding force that flows like a fountain and that when we drink it gives us strength, life and meaning. 

In these times it is easy to join the mob and shout without stopping. Don't look for me there. It was never my place. If you want to find me, I'll be in the wilderness, walking, maybe looking at clouds.

Salunautas

 




Cuenta la leyenda que hace mucho tiempo un héroe griego llamado Jasón se embarcó con un puñado de valientes en una misión suicida. Fueron enviados a buscar un imposible por una ruta plagada de peligros, su historia es uno de esos viejos pilares que sostiene nuestra civilización europea. 


Igual que aquellos Argonautas se las tuvieron que ver con la dificultad, en nuestra época también navegamos aguas complicadas. Eso nos parece suficiente razón como para fletar un podcast y echarnos a la mar de los contenidos digitales hablados. Salunautas es un viaje que comienzan tres amigos, la dra. Isabel Núñez, el dr. Francesc Borrell y Salvador Casado, que comparten una pasión por la comunicación, la medicina y el arte. Pero lo verdaderamente importante no es de dónde partimos sino a dónde queremos llegar. Y el destino no es otro que la salud, ese estado tan difícil de definir pero que todo el mundo anhela.


El barco que elegimos tiene que ver con la narrativa y nos gustaría que las velas tengan más de agradecimiento que de queja, de propuesta más que de condena. Para ello nos apoyaremos en los muchos compañeros, amigas y amigos que quieran arroparnos a quienes abrimos la puerta con ilusión. Y por supuesto en todas las personas que quieran escuchar nuestras propuestas que tendrán correspondencia escrita en el boletín Iatros, en este blog, en Twitter y mucho más allá. 


Esperamos que puedan acompañarnos en esta travesía pese a que no podemos garantizarles excesiva comodidad o bienestar. Lo que sí intentaremos que tengan es una buena dosis de arte, cultura y un puntito de alegría

Salunautas és un viatge que comencen tres amics, Isabel Núñez, Francesc Borrell i Salvador Casado, que comparteixen una passió per la comunicació, la medicina i l'art. Però el veritablement important no és d'on partim sinó a on volem arribar. I la destinació no és altre que la salut, aquest estat tan difícil de definir però que tothom anhela.


El vaixell que vam triar té a veure amb la narrativa i ens agradaria que les veles tinguin més d'agraïment que de queixa, de proposta més que de condemna. Per a això ens recolzarem en els molts companys, amigues i amics que vulguin abrigar-als que vam obrir la porta amb il·lusió. I per descomptat en totes les persones que vulguin escoltar les nostres propostes que tindran correspondència escrita en el butlletí Iatros, el bloc de doctor Casado, a Twitter i molt més enllà.


Esperem que puguin acompanyar-nos en aquesta travessia tot i que no podem garantir-los excessiva comoditat o benestar. El que sí que intentarem que tinguin és una bona dosi d'art, cultura i un puntet d'alegria.

Salunautas is a journey started by three friends, Isabel Núñez, Francesc Borrell and Salvador Casado, who share a passion for communication, medicine and art. The boat we have chosen has to do with the narrative and we would like the sails to be more of a thanks viven than a complaint, more of a proposal than a condemnation. ¿Es posible unificar el número de teléfono de los centros de salud para que el usuario sepa quién le llama? Unifying the telephone number of the Health System. 統一衛生系統的電話號碼。

 




Hace unos días mi amigo Rafa Bravo compartía en Twitter la iniciativa del Servicio Andaluz de Salud de ofrecer un número de teléfono único para que los ciudadanos identifiquen la llamada y faciliten que contesten cuando se intenta comunicar con ellos desde los centros de salud. 

Es un problema importante. Este año la asistencia telefónica ocupa muchas horas del tiempo de administrativos sanitarios, enfermeras y médicas. Es común que al recibir un número largo y desconocido de una centralita de centro de salud, muchos no contesten creyendo que es publicidad no deseada, lo que obliga a los profesionales a perder tiempo y desesperarse en muchos casos. 

El responsable de la implementación de esta mejora en Andalucía es mi admirado Francisco Sánchez Laguna que me comentó en el acto que la medida es más técnica que funcional y tan solo requería un ajuste del contrato de telefonía corporativo, algo poco complejo y de coste cero. 

Inmediatamente lo compartí en redes sociales y escribí varios correos electrónicos a mis superiores. La dirección general de sistemas de información tuvo a bien responderme, por lo visto el asunto depende de Madrid digital. Como un servidor no tiene mucho tiempo para más trámites lo comparto por si alguien tiene más suerte que yo.

En tiempos complejos es necesario ser audaces, no toda mejora requiere aumentar el gasto. Lo cierto es que la comunicación dentro del sistema sanitario sigue siendo un enorme problema que limita la innovación, mejora continua y el necesario flujo de ideas. 


(Probáblemente este artículo llegue antes a Pekín o a Londres que a la Consejería de Sanidad de Madrid. De todas formas lo seguiremos intentando).



Unifying the telephone number of the Health System.


A few days ago my friend Rafa Bravo shared on Twitter the initiative of the Andalusian Health Service to offer a single telephone number for citizens to identify the call and make it easier for them to answer when trying to contact them from the health centres. 

It is a major problem. This year telephone assistance is taking up many hours of the time of health administrators, nurses and doctors. It is common that when receiving a long and unknown number from a health centre switchboard, many do not answer, believing it to be unwanted advertising, forcing professionals to waste time and despair in many cases. 

The person responsible for the implementation of this measure in Andalusia is my admired Francisco Sánchez Laguna who told me at the event that the measure is more technical than functional and only required an adjustment of the corporate telephony contract, something not very complex and at zero cost. 

I immediately shared it on social networks and wrote several emails to my superiors. The general management of information systems was kind enough to reply, apparently the matter depends on Madrid digital. As I don't have much time for more paperwork, I'm sharing it in case someone else is luckier than me.

In complex times it is necessary to be bold, not every improvement requires an increase in spending. The truth is that communication within the healthcare system continues to be a huge problem that limits innovation, continuous improvement and the necessary flow of ideas.
