最好的創新


多年來,我反映了公共衛生保健的哪些方面可以改進。這個清單非常龐大。我重新調整了這個問題的重點,重點關注了我關心的問題,並將這個問題變成了為我提供更好服務的家庭醫生。

我首先看到的第一件事就是走出辦公室。由於護理過重,我的組織迫使我花費大量時間,但這最終會損害我最脆弱的患者的家庭護理和普遍被忽視的整個社區健康問題。它還會造成專業隔離並與系統中的初級保健團隊和其他專業人員失去聯繫。為了建立專業聯繫並改善與他們和我工作的社區的溝通,這是多年來一系列的行動。

The best innovation in Health


For years I have reflected on what aspects of public health care could be improved. The list was immense. I refocused the issue and focused on what concerns me and changed the question to what I could do as a family doctor to offer a better service.

The first thing I saw as a priority was to get out of the office. My organisation forces me to spend many hours in it due to the overload of care, but this ends up damaging home care for my most fragile patients and the whole dimension of community health that is usually neglected. It also generates professional isolation and loss of connections with both the Primary Care team and the rest of the professionals in the system. To generate professional connections, to improve communication with them as well as in the community where I develop my work is therefore a line of action to promote for many years now.

The second thing I wanted to improve was health education for both the people who go to the health service and the general population. I have used the traditional media to do this by agreeing to participate in television, radio and print media programmes. Also from the social networks where I share health content from both my blog and my video channel. Writing a book has been another step in this direction that, according to the good reviews of colleagues and patients, has more than justified my effort. And of course to give talks or conferences in schools, conferences, congresses or other forums where I have been required.

The third thing to review concerns my personal and professional maintenance. Given the very high rates of overload and professional burn out in the world of health, it is essential to have a daily tune-up to superimpose on the work of study and continuous training. Many health professionals are burned by a system that overloads and does not allow care under humanized conditions. The industrial structure forces to attend patients in much shorter times than any veterinarian uses. A burned-out doctor will offer poor quality care. That is why it is essential to evaluate on a daily basis how the consultation begins and ends.


I can therefore say that the best innovation is to do things as best as possible with the means at one's disposal. Dare to make small changes, improve communication and connections. Be aware of what others are doing to be inspired or to incorporate issues that add value. And above all, to take care of oneself in order to take care of others.

With regard to the applications, programs, devices and technologies that defeat us as innovative, I manifest a healthy scepticism. I know the area well and I know that there is much ado about nothing. There are plenty of smoke sellers and promises of eternal youth but results and scientific evidence are scarce.

My vision is therefore simple. I'm betting on the transforming power of the small and, who knows, maybe this water will end up moving the mill.





Durante años he reflexionado sobre qué aspectos de la asistencia sanitaria pública serían mejorables. La lista era inmensa. Reenfoqué el tema y me centré en lo que a mi me atañe y cambié la pregunta a qué podría hacer yo como médico de familia para ofrecer un mejor servicio.

Lo primero que vi como prioritario fue salir de la consulta. Mi organización me obliga a pasar muchas horas en ella dada la sobrecarga asistencial pero esto termina dañando la asistencia a domicilio a mis pacientes más frágiles y toda la dimensión de salud comunitaria que habitualmente es desatendida. También genera aislamiento profesional y pérdida de conexiones tanto con el equipo de Atención Primaria como con el resto de profesionales del sistema. Generar conexiones profesionales, mejorar la comunicación con los mismos así como en la comunidad donde desarrollo mi labor es pues una línea de acción a potenciar desde hace ya muchos años.

Lo segundo que quise mejorar fue la educación en salud tanto a las personas que acuden al servicio sanitario como a la población general. Para esto me he valido de los medios de comunicación tradicionales aceptando participar en programas de televisión, radio y prensa escrita. También de las redes sociales en las que comparto contenido en salud tanto de mi blog como de mi canal de vídeos. Escribir un libro ha sido otro paso en este sentido que a tenor de las buenas críticas de compañeros y pacientes me han justificado con creces el esfuerzo. Y por supuesto dar charlas o conferencias en colegios, jornadas, congresos u otros foros donde se me ha requerido.

Lo tercero a reseñar tiene que ver con mi mantenimiento personal y profesional. Dadas las altísimas tasas de sobrecarga y burn out profesional en el mundo de la salud es fundamental un afinamiento diario a superponer a la labor de estudio y formación continuada. Son muchos los profesionales de la salud quemados por un sistema que sobrecarga y no permite una asistencia en condiciones humanizadas. La estructura industrial obliga a atender pacientes en tiempos mucho menores que lo que usa cualquier veterinario. Un médico quemado ofrecerá una pobre calidad de asistencia. Por eso es básico evaluar a a diario cómo empieza uno la consulta y cómo la acaba.


Puedo decir por tanto que la mejor innovación es hacer las cosas lo mejor posible con los medios que cada cual tenga. Atreverse a hacer pequeños cambios, mejorar la comunicación y las conexiones. Estar al tanto de lo que otros hacen para inspirarse o incorporar cuestiones que aporten valor. Y sobre todo cuidarse mucho para poder cuidar a los demás.

Con respecto a las aplicaciones, programas, aparatos y tecnologías que nos vencen como innovadoras manifiesto un saludable escepticismo. Conozco bien el ámbito y sé que hay mucho ruido y pocas nueces. Abundan los cantamañanas y las promesas de eterna juventud pero escasean los resultados y la evidencia científica.

Mi visión es pues sencilla. Apuesto por la fuerza transformadora de lo pequeño y, quién sabe, tal vez este agua sí termine moviendo el molino.
