Visibilizar talento y proyectos de valor en Atención Primaria. Make talent and value projects visible in Primary Health Care. 在初級衛生保健中體現人才和價值項目。




No me gusta la queja por lo que tiene de viscoso. Se queda fácilmente pegada en el ánimo y termina envenenando la cotidianidad. Por eso prefiero proponer ideas, aunque sean pequeñas, locas o no consigan mucho recorrido.

Hablando con Miguel Ángel Mañez sobre la posibilidad de cartografiar recursos de valor en Atención Primaria me animó a abrir una wiki, un proyecto colaborativo. Lo monté en unos minutos y lo dejé en abierto para que quien quiera pueda añadirse y proponer.

En los principios de la blogosfera sanitaria, hace diez años, un grupo de valientes creo Wikisanidad, pero ese proyecto terminó cerrando (por cierto habría que ver si se pudiera rescatar). Desde entonces no me consta que se hayan acometido iniciativas parecidas. Hasta ahora.

Sin mapas estamos perdidos en medios tan complejos como los sanitarios. Y mapas no hay dada la crisis institucional que padecemos con sistemas de mucha inercia que tienen muy difícil la adaptación a medios líquidos como los actuales.

Comparto aquí la idea esperando que haya alguien que considere interesante saber qué proyectos, grupos o ideas de valor se están moviendo bajo las aguas de la Atención Primaria.

Make talent and value projects visible in Primary Health Care.



I don't like the complaint becouse it´s viscous.  It easily gets stuck in the mood and ends up poisoning everyday life. That's why I prefer to propose ideas, even if they're small, crazy or don't go far away.

Talking with Miguel Ángel Mañez about the possibility of mapping valuable resources in Primary Health Care, he encouraged me to open a wiki, a collaborative project. I set it up in a few minutes and left it open for anyone to add and propose.

At the beginning of the sanitary blogosphere, ten years ago, a group of brave people created Wikisanidad, but that project ended up closing (by the way we would have to see if it could be rescued). Since then, I don't know that similar initiatives have been undertaken. Until now.

Without maps we are lost in such complex environments as health care. And there are no maps given  by organizations that suffer institutional crisis. Public Health Systems with great inertia are addapting badly to liquid media such as the current ones.

I share here the idea hoping that there will be someone who considers it interesting to know what projects, groups or ideas of value are moving under the waters of Primary Health Care.

在初級衛生保健中體現人才和價值項目。



我不喜歡投訴,因為它很粘稠。它很容易陷入困境,最終導致日常生活中毒。這就是為什麼我更喜歡提出想法,即使它們很小,很瘋狂或者不會很遠。

MiguelÁngelMañez 談論在初級衛生保健中繪製寶貴資源的可能性時,他鼓勵我開設一個維基,一個合作項目。我在幾分鐘內完成了設置並將其打開以供任何人添加和提議。

El arte de escuchar. The art of listening. 聽的藝術。






Dentro de las competencias básicas de la vida para cualquier persona está saber escuchar. Es fundamental tanto para nuestras relaciones personales como profesionales, en la esfera pública, privada o íntima, como en cualquier proceso comunicativo que establezcamos.

Pero no todo es fácil de ser escuchado. Solemos escapar de lo que nos cuestiona, nos confronta con nuestras dificultades o nos acerca a pensamientos o sentimientos difíciles de sostener. Por eso es tan arduo escuchar el dolor y el sufrimiento ajeno, de alguna manera nos recuerda el nuestro y a penas somos capaces de mirarlo de reojo.

Los profesionales de la salud se supone que estamos entrenados para ello, pero esta habilidad no se enseña en los planes de estudio y cada cual la desempeña como puede. Para ello nos apoyamos en habilidades personales de comunicación y en el aprendizaje de patrones de escucha de personas conocidas o del ámbito sanitario que nos sirven de referentes.

La formación sobre técnicas de comunicación y la práctica e implementación de las mismas tiene una distribución muy desigual según dónde se forme cada discente y según sea su interés y disposición frente al tema.

Los profesionales sanitarios tienen hoy varios condicionantes importantes que dificultan la escucha que podríamos sistematizar de la siguiente manera:

1. Exceso de ruido de fondo social.
2. Exceso de ruido de fondo organizacional.
2. Exceso de ruido de fondo personal.

A nivel social estamos sometidos a una gran presión de información, noticias, cotilleos, memes, anuncios y demás que recibimos de forma audiovisual las 24 horas. La multitarea, la prisa y los ritmos de vida acelerados sirven de amplificador cotidiano de este ruido del que es muy difícil escapar.

Por otro lado las instituciones también generan ruido con su diseño industrial. El mundo sanitario no es una excepción y aunque no tengamos maquinaria pesada en las consultas si contamos con ordenadores con sistemas de información e historias clínicas digitales cuyo diseño es ruidoso y obligan a perder tiempo y atención en la interacción con ellas. La sobrecarga asistencial y las interrupciones son otras potentes fuentes de ruido. La generación de climas laborales adversos en los que el trabajador sanitario no se siente escuchado sino más bien maltratado por la organización es otro foco de malestar y añade ruido de fondo en forma de queja o rumiación de negatividad ante la tarea.

El frente más arduo y que más ruido de fondo produce es el personal. La mente humana no deja de saltar de rama en rama en un discurso interno que en muchas ocasiones es imparable. Nos asaltan preocupaciones, ideas, cuestiones variadas y problemas que no somos capaces de solucionar o aceptar y que se enquistan en bucles que permanecen en segundo plano produciendo un desagradable ruido de fondo.



The art of listening.


Within the basic competencies of life for any person is to know how to listen.  It is fundamental both for our personal and professional relationships, in the public, private or intimate sphere, as in any communicative process that we establish.


But not everything is easy to be heard. We usually escape from what questions us, confronts us with our difficulties or brings us closer to thoughts or feelings that are difficult to sustain. That's why it's so hard to listen to the pain and suffering of others, it somehow reminds us of our own and we're barely able to look at it sideways.


Health professionals are supposed to be trained for it, but this skill is not taught in the curriculum and everyone does it as best they can. For this we rely on personal communication skills and learning patterns of listening to people known or the health field that serve as references.


Training in communication techniques and the practice and implementation of these have a very unequal distribution according to where each learner is formed and according to their interest and disposition towards the subject.


Healthcare professionals today have several important conditioning factors that make it difficult to listen, which we could systematize in the following way:


1. Excessive social background noise.

2. Excess of organizational background noise.

2. Excessive personal background noise.


At a social level we are subject to a great pressure of information, news, gossip, memes, announcements and others that we receive in an audiovisual form 24 hours a day. The multitasking, the haste and the accelerated rhythms of life serve as a daily amplifier of this noise from which it is very difficult to escape.


On the other hand, institutions also generate noise with their industrial design. The health world is no exception and, although we do not have heavy machinery in the clinics, we have computers with information systems and digital medical records whose design is noisy and force us to lose time and attention in the interaction with them. Care overloads and interruptions are other powerful sources of noise. The generation of adverse work climates in which the health worker does not feel heard but rather mistreated by the organization is another focus of discomfort and adds background noise in the form of complaint or rumination of negativity to the task.


The most arduous front and the one that produces the most background noise is the personnel. The human mind does not stop jumping from branch to branch in an internal discourse that in many occasions is unstoppable. We are assailed by worries, ideas, varied questions and problems that we are not capable of solving or accepting and that become entrenched in loops that remain in the background producing an unpleasant background noise.

在任何人的基本生活能力範圍內,都要懂得傾聽。它對於我們在公共,私人或私密領域的個人和職業關係都是至關重要的,就像我們建立的任何交流過程一樣。


但並非一切都很容易被聽到。我們通常擺脫了我們的問題,使我們面對困難或使我們更接近難以維持的想法或感受。這就是為什麼聽到別人的痛苦和痛苦是如此困難,它以某種方式提醒我們自己,我們幾乎無法側身看待它。

Respeto. Respect. 尊重







Hoy día de la mujer está convocada una huelga para visibilizar las problemáticas que sufren las mujeres por el hecho de serlo. "Paramos las mujeres pero no paran los cuidados" es uno de los eslóganes. Por mi parte pasaré la jornada ofreciendo servicios mínimos, es decir cuidando, pero también me parecía interesante reflexionar. Como médico veo en consulta mucho sufrimiento en las mujeres, también en los hombres. Veo que existen injusticias, violencias y brechas que separan géneros, edades, condiciones sociales y demás. Veo que la gente sufre porque no se siente suficientemente respetada.

A mi juicio el problema de fondo estriba en una falta de respeto personal. Si una persona no se respeta a sí misma tampoco lo hará al entorno ni a los demás. Y en nuestra cultura no nos enseñan a respetarnos bien. Crecemos en ambientes que usan el castigo, la violencia y la coacción para conseguir objetivos "educativos" desde que somos pequeños. Luego lo reproducimos en nuestras relaciones personales, familiares, laborales y sociales.

Respeto.

Y dentro de él tratar de hacer florecer la consciencia, la compasión y la delicadeza. El día que lo consigamos hacer con nuestras niñas y niños, con los adolescentes, con nuestras amistades y familiares y con nuestras parejas cambiaremos el mundo. Hasta entonces cada pequeño paso cuenta. En paralelo a todo el que teoriza y propone, a las que visibilizan, a las que luchan por medidas políticas, es fundamental cuidar el frente de las personas más cercanas. Cada vez que un hombre ejerce su dimensión de cuidado cuenta, cada vez que una mujer lo hace sigue dando ejemplo y testimonio. Cada vez que en una pareja se consigue una situación de igualdad damos un paso.

Por eso hoy pido respeto y trato de ofrecer el mio.

Por eso hoy es un día en el que merece reflexionar y compartir con los que tenemos cerca estas inquietudes.





Respect

Today, women are called to strike to make visible the problems that women suffer because they are women. "We stop the women but the care doesn't stop" is one of the slogans. For my part, I will spend the day offering minimum services, that is to say, taking care. But I also thought it would be interesting to reflect. As a doctor, I see a lot of suffering in women, also in men. I see that there are injustices, violence and gaps that separate gender, age, social conditions and so on. I see that people suffer because they don't feel respected enough.

In my opinion, the fundamental problem lies in a lack of personal respect. If a person does not respect himself or herself, he or she will not respect the environment or others. And in our culture we are not taught to respect ourselves well. We grow up in environments that use punishment, violence and coercion to achieve "educational" goals since we are small. Then we reproduce it in our personal, family, work and social relationships.

Respect.

And within it we try to make conscience, compassion and delicacy flourish. The day we manage to do it with our children, with adolescents, with our friends and family and with our partners, we will change the world. Until then, every small step counts. Parallel to all those who theorize and propose, those who make visible, those who fight for political measures, it is fundamental to take care of the front of the closest people. Every time a man exercises his dimension of care counts, every time a woman does it she continues to give example and testimony. Every time a couple achieves equality we take a step.

That is why today I ask for respect and I try to offer mine.

That is why today is a day in which it is worth reflecting and sharing with those of us who are close to these concerns.







尊重

今天,婦女被要求罷工,以揭示婦女因女性而遭受的問題。 “我們阻止女性,但護理不會停止”是口號之一。就我而言,我將花一天時間提供最低限度的服務,也就是說,照顧。但我也認為反思會很有趣。作為一名醫生,我看到女性和男性都有很多痛苦。我看到有不公正,暴力和差距將性別,年齡,社會條件等分開。我看到人們受苦是因為他們不夠受人尊重。
