Reprogramar los centros de salud. Reprogramming primary health centres.

Nos hemos acostumbrado a que nuestros aparatos electrónicos tengan sistema operativo. Una narrativa digital en forma de código que permite que funcionen. Sin ella ni el móvil, ni el ordenador ni el coche se pondrán en marcha, serían solo chatarra. De alguna forma también lo son cuando dicha programación queda obsoleta y deja de ser capaz de manejar aplicaciones o programas diseñados para versiones más actualizadas. En ese momento no témenos más remedio que cambiar a un nuevo aparato con el último sistema operativo disponible. Obsolescencia programada lo llaman.

 

Obsolescencia sanitaria

 

A muchas de nuestras instituciones, empresas y organizaciones humanas les está pasando esto. Elijan la que quieran, por mi parte me centraré en la que más conozco que es la sanitaria. Una estructura humana, tecnológica y estructural enorme con un gran presupuesto y una ingente capacidad de servicio y sostén social. Sin embargo llega un momento en que el diseño y la programación original quedan obsoletos lo que empieza a sobrecargar y sobrecalentar delicados circuitos, disminuir la capacidad funcional, dar fallos repetidos del sistema, quedarse colgada cada dos por tres y aumentar el malestar en los usuarios. Cuando es un teléfono el queda obsoleto lo cambiamos pero ¿qué hacemos cuando es la sanidad la que se ha vuelto zombi?

 

¿Inyectar recursos o hackear el sistema?

 

Los responsables y directivos se muestran incapaces de hacer frente a esta situación que no es solucionable únicamente inyectando recursos. No es un problema de gestión, es un problema de programación, de sistema operativo. Todo el capital humano, monetario, tecnológico que apliquen a un sistema con su programación obsoleta no funcionará, como no lo hace poner aplicaciones nuevas en un móvil antiguo que no las soporta. El vino nuevo en odres viejos nunca maridó bien como saben los que tengan familia en el pueblo o hayan leído el evangelio. 

 

Las propuestas de nueva programación son infinitas, cada colectivo profesional tiene las suyas, pero me temo que la cosa no va así. El programador suele ser alguien alejado del usuario (interno y externo) aunque tenga obligación de conocerlos. Por eso solo apuntaré algunas pinceladas para hackear el sistema en espera de que dispongamos de una nueva versión, que puede que tarde pero es inevitable que termine llegando. 

 

¿Podrá el sistema cuidar mejor a los sanitarios?

 

A nivel de gestión priorizaría la atención a los circuitos integrados de mayor calidad del sistema: los profesionales. Y lo haría cambiando el foco desde las métricas de cartera de servicios y programación asistencial, que probablemente se puedan automatizar sin mucho esfuerzo, hacia el cuidado, asesoría y sostén de los profesionales desarrollando para ellos nuevos servicios que mejoren su desempeño profesional y los protejan de la sobrecarga. Mejorar la gestión de “recursos humanos” y tratar de minimizar el maltrato profesional es fundamental si no queremos quedarnos con profesionales que en buena lógica cambian a puestos de trabajo mejores. 

 

¿Se apostará algún día por la autogestión?

 

A nivel de los centros de salud permitiría más autogestión de una vez por todas confiando en que muchas decisiones operativas son más eficaces si se toman desde el nivel que tiene que bregar con las mayores dificultades. Potenciaría el trabajo en equipo real que evite que unos profesionales tengan el doble de agenda que otros y que algo tan evidente como que un paciente sin cita con un motivo de consulta no complicado pueda ser atendido tanto por enfermeras como por médicos sea por fin una realidad. Aunque parezca mentira en la mayoría de los centros de salud esto sigue siendo imposible. Añadiría para compensar mecanismos de refuerzo de centros sobrecargados fomentando una nueva distribución de recursos proporcional al grado de complejidad que el mismo afronte así como otros de solidaridad que permitan que unos centros más descargados puedan apoyar a los que tengan más necesidad.

 

¿Conseguiremos dar máximo valor a la logitudinalidad?

 

Dentro de la consulta la programación debería priorizar ante todo la relación entre profesionales y pacientes que es el centro de todo el Sistema Sanitario. Para facilitarla se debería potenciar la longitudinalidad que se define como el tiempo que un profesional mantiene un destino y una población a su cargo. Esto permite que profesionales y pacientes se conozcan, mejora la información familiar y comunitaria por parte del profesional y la confianza y seguridad por parte del paciente. Solo será posible si se incentiva con vehemencia, no hacerlo hace que los profesionales busquen el cambio para mejorar turno, cercanía al domicilio o disminuir la complejidad del centro de salud. 

 

¿Asitencia sanitaria tipo “fast food” o “low food”?

 

Lo siguiente a priorizar serían los módulos de comunicación clínica y  reflexión, algo que precisa de tiempo y silencio. Una consulta con una agenda desproporcionada o con mucho ruido de fondo (llamadas, distracciones, pacientes sin cita, urgencias, avisos, compañeros que preguntan, puertas que se abren…) lo impiden por definición. Si el módulo de comunicación clínica falla y no hay suficiente escucha será imposible orientar correctamente la anamnesis. Sin una buena anamnesis y el suficiente silencio reflexivo no será posible que el módulo de reflexión oriente bien el caso. La asistencia tipo “fast food” produce gran cantidad de actos al día, pero de poca calidad. Es mucho más rentable priorizar el “low food” sabiendo que será más eficiente, protegerá al profesional y obtendrá mejores resultados en salud para el paciente y la organización.

 

Esto puede ser costoso para profesionales que han sido entrenados para desarrollar técnicas diversas, cirugía menor, infiltraciones, ecografía, crioterapia… que ocupan un tiempo que no siempre está disponible en una agenda que exceda lo recomendable. No es sencillo asumir una reducción en la cartera de servicios ni en el despliegue de habilidades profesionales, pero es necesario aplicar la máxima inteligencia y flexibilidad a la hora de evaluar la practica asistencial de cada cual. Sin un diagnóstico preciso del tipo de práctica, el tipo de centro de salud y el tipo de agenda seguiremos trabajando en modo piloto automático y seguirá creciendo la frustración y el queme profesional según pasa el tiempo. Para esto quizá una auditoría por pares sea una herramienta útil, con apoyo de las unidades de gestión cuando su ayuda sea requerida. 

 

El despotismo ilustrado en la gestión sanitaria (todo para el paciente pero sin el paciente, todo para el profesional pero sin el profesional) tiene los días contados siendo urgente redefinir los roles y las reglas del juego. Si no lo hace la propia organización lo hará el mercado, de hecho ya lo está haciendo compensando la pérdida de calidad del sistema público con un aumento vertiginoso de  servicios y seguros de salud privados. 

 

La reprogramación más difícil

 

Reprogramar el modo en que los ciudadanos interactúan con el sistema probablemente sea la parte más difícil, no porque sea compleja sino porque no es políticamente rentable y ningún partido apostará por medidas que le resten votos salvo que se consigan consensos que en el caso sanitario no son sencillos de obtener. Si no se logra que se haga un uso más prudente y correcto del sistema será imposible que este sobreviva. Los Sistemas Sanitarios tipo “barra libre” como el nuestro no tienen capacidad para asumir todo el malestar de una sociedad, ni tampoco los pequeños problemas de salud inherentes a la vida ordinaria susceptibles de ser manejados con autocuidados. Toda la energía que se decida a esto se retira de los pacientes más enfermos o potencialmente graves, de los más mayores o los más frágiles. Facilitar que la sociedad lo entienda y asuma su responsabilidad a la hora de usar bien la Sanidad es el primer paso para su supervivencia. Si no aprobamos esta asignatura pendiente tendremos que conformarnos con una Sanidad zombi que no será del agrado ni de gestores, ni de profesionales, ni de usuarios.

Reprogramming primary health centres.

 

 

Mainly all our electronic devices have operating systems. A digital narrative in the form of code that allows them to function. Without it, neither the mobile phone, nor the computer, nor the car will run, they would just be junk. In a way, they are also junk when that programming becomes obsolete and is no longer capable of handling applications or programmes designed for more up-to-date versions. At that point we have no choice but to switch to a new device with the latest operating system available. Planned obsolescence, they call it.

 

Health obsolescence

 

This is happening to many of our institutions, businesses and human organisations. Take your pick, for my part I will focus on the one I know best, which is the health sector. It is a huge human, technological and structural structure with a large budget and an enormous capacity for service and social support. However, there comes a time when the original design and programming becomes obsolete, which begins to overload and overheat delicate circuits, reduce functional capacity, cause repeated system failures, hang up every so often and increase user discomfort. When it is a phone that becomes obsolete, we replace it, but what do we do when it is the health system that has become zombie-like?

 

Do we inject resources or hack the system?

 

Managers and directors are incapable of dealing with this situation, which cannot be solved by injecting resources alone. It is not a management problem, it is a problem of programming, of the operating system. All the human, monetary and technological capital applied to a system with obsolete programming will not work, any more than putting new applications on an old mobile phone that does not support them. New wine in old wineskins never paired well as those who have family in the village or have read the gospel know. 

 

The proposals for new programming are infinite, each professional collective has its own, but I'm afraid that's not the way it goes. The programmer is usually someone far removed from the user (internal and external) even though he or she is obliged to know them. That's why I'll just point out a few tips for hacking the system while waiting for a new version, which may take a while but will inevitably arrive. 

 

Will the system be able to take better care of healthcare workers?

 

At the management level, I would prioritise attention to the system's highest quality integrated circuits: the professionals. And I would do so by shifting the focus from service portfolio metrics and care programming, which can probably be automated without much effort, to caring for, advising and supporting professionals by developing new services for them that improve their professional performance and protect them from overload. Improving the management of "human resources" and trying to minimise professional mistreatment is essential if we do not want to be left with professionals who logically move on to better jobs. 

 

Will there ever be a move towards self-management?

 

At the level of health centres, it would allow more self-management once and for all, trusting that many operational decisions are more effective if they are taken at the level that has to deal with the greatest difficulties. It would promote real teamwork that avoids some professionals having double the agenda of others and that something as obvious as a patient without an appointment with an uncomplicated reason for consultation can finally be attended to by both nurses and doctors becomes a reality. Believe it or not, this is still impossible in most health centres. In order to compensate for this, I would add mechanisms to reinforce overburdened centres by encouraging a new distribution of resources proportional to the degree of complexity they face, as well as other solidarity mechanisms that would allow some centres that are more overburdened to support those that are more in need.

 

Will we succeed in giving maximum value to logitudinality?

 

Within the consulting room, programming should prioritise above all the relationship between professionals and patients, which is at the heart of the entire healthcare system. To facilitate this, longitudinality should be promoted, which is defined as the time that a professional maintains a destination and a population under his or her care. This allows professionals and patients to get to know each other, improves family and community information on the part of the professional and confidence and security on the part of the patient. It will only be possible if it is vehemently encouraged; if it is not, professionals will seek to change in order to improve their shifts, be closer to home or reduce the complexity of the health centre. 

 


 

Fast food or low food health care?

 

The next thing to prioritise would be the clinical communication and reflection modules, something that requires time and silence. A practice with a disproportionate agenda or with a lot of background noise (calls, distractions, patients without appointments, emergencies, announcements, colleagues asking questions, doors opening...) prevents this by definition. If the clinical communication module fails and there is not enough listening, it will be impossible to orientate the anamnesis correctly. Without a good anamnesis and sufficient reflective silence, it will not be possible for the reflection module to orientate the case correctly. Fast food" care produces a large number of acts per day, but of poor quality. It is much more cost-effective to prioritise "low food" in the knowledge that it will be more efficient, protect the practitioner and produce better health outcomes for the patient and the organisation.

 

This can be costly for professionals who have been trained to develop diverse techniques, minor surgery, infiltrations, ultrasound, cryotherapy... that take up time that is not always available in a schedule that exceeds what is recommended. It is not easy to assume a reduction in the portfolio of services or in the deployment of professional skills, but it is necessary to apply maximum intelligence and flexibility when assessing the care practice of each one. Without an accurate diagnosis of the type of practice, the type of health centre and the type of agenda, we will continue to work in autopilot mode and frustration and professional burnout will continue to grow as time goes by. For this, perhaps a peer audit would be a useful tool, with support from the management units when their help is required. 

 

The days of enlightened despotism in healthcare management (everything for the patient but without the patient, everything for the professional but without the professional) are numbered and it is urgent to redefine the roles and rules of the game. If the organisation itself does not do it, the market will, in fact it is already doing so, compensating for the loss of quality in the public system with a dizzying increase in private health services and insurance. 

 

The most difficult reprogramming

 

Reprogramming the way in which citizens interact with the system is probably the most difficult part, not because it is complex but because it is not politically profitable and no party will go for measures that will take votes away unless consensus is achieved, which in the case of health is not easy to obtain. If a more prudent and correct use of the system is not achieved, it will be impossible for it to survive. Open bar" health systems such as ours are not capable of dealing with all the discomfort of a society, nor with the small health problems inherent in ordinary life that can be managed by self-care. All the energy that goes into this is withdrawn from the sickest or most potentially serious patients, the oldest or the most frail. Making it easier for society to understand this and to assume its responsibility for the proper use of healthcare is the first step towards its survival. If we do not pass this unfinished business, we will have to make do with a zombie healthcare system that will not be to the liking of managers, professionals or users.

Hermana Navidad

Cada cual celebra la Navidad como puede según sus creencias y circunstancias. Como suele pasar hay mucho ruido de fondo y demasiadas luces que no dejan ver el cielo nocturno. Este año me pregunto dónde está mi estrella, qué ilumina mis pasos, qué camino sigo para encontrar sentido. Porque la sensación es de caminar a oscuras. Llevamos ya seis olas de pandemia y parece que hemos aprendido poco. Seguimos con colapsos sanitarios y colas delante de farmacias, hospitales y centros de salud, lo que de forma inesperada nos hermana a la gente que en Venezuela, Bangladesh o Nigeria atraviesa los mismos problemas. 


En los últimos meses me doy un pequeño paseo antes de entrar en consulta. Esta semanas a oscuras por ser los días tan cortos. Camino por una playa rozando el agua, unos días con estrellas, otros sin ellas. Lo cierto es que ese silencio me ayuda al acallar un poco el griterío interno que remeda al externo, o tal vez lo correcto sea lo opuesto. De cualquier manera salgo renovado de mi caminata sobre las aguas, un pequeño milagro que me anima cuando luego acompañe a tantos pacientes que naufragan, se hunden y tragan agua sin parar.


En el silencio rescato que hay sentido, la naturaleza lo tiene por el mero hecho de existir. La belleza del amanecer es una declaración de intenciones, por muy negra que sea la noche, la luz sigue naciendo. Ser transmisor de este sencillo mensaje convierte a cada cuidadora y a cada cuidador en una antorcha humana que lleva luz y calor a quien lo necesita. No podemos perder algo tan importante ni tampoco recluirlo en un puñado de sanitarios agobiados y sobrecargados. Cada cual es responsable de cuidarse lo mejor que sepa y de tratar de cuidar a los cercanos. Ser capaces de dar esa respuesta nos humaniza, no hacerlo nos revela una pobreza que el dinero no puede vencer. 


Por todo esto viene bien recordar que es fácil quedarse en el titular, en la bronca o en la queja, pero que debajo de todo eso hay un silencio oscuro que si nos atrevemos a pisar quizá nos termine revelando estrellas en el cielo. Y si las contemplamos lo suficiente nos dirán dónde está el norte, algo que nunca verán en el telediario.

La sexta ola significa un sexto colapso sanitario. La sanidad tiene tan poco fuelle que bastan unos pocos catarros de más para que hagamos aguas, sobretodo si acuden a la vez. Es verdad que en este caso son cientos de miles, y que todo el mundo quiere su test para saber si puede ir tranquilo a las comidas navideñas (en cualquier caso la gran mayoría irá, no lo duden).


El espectáculo de los presidentes y presidentas sigue siendo lamentable. El grito de ahogo de los sanitarios también. Y el personal termina ignorando a unos y a otros, hacen la cola con resignación y a seguir.


Los expertos, epidemiólogos y demás tampoco dan la nota. Cada cual dice una cosa y todo sigue igual. Los sanitarios corremos como pollo sin cabeza, sudando la camiseta por la imposibilidad de llegar a todo. Y así estaremos las siguientes semanas hasta que llegue el próximo follón (la gripe) que ya se avecina.


Por eso mi sencilla conclusión para la ciudadanía es: hagan ustedes un prudente uso de su libertad, pero luego no vengan al centro de salud a contarnos su catarro (cuídeselo usted como otras veces).

La desaparición de los generalistas. The disappearance of the generalists. 普通人的消失。






Nuestro tiempo líquido nos ofrece múltiples ventajas. Es posible acceder a infinitas bibliotecas digitales, escuchar cualquier canción que deseemos, ver cualquier película, contactar al instante con quien queramos. Nos pone en el supermercado infinitos productos y con frecuencia a precios sorprendentes. Pero luego nos lo cobra caro con contaminación, ruido de fondo e incertidumbre. Es mucho más sencillo pensar en distopías que en utopías, más fácil quejarse de la propia situación que bendecirla, más frecuente sentirse estresado que en paz. 


Por estas razones y por otras nadie quiere aceptar más incertidumbre de la necesaria y los jóvenes prefieren ser especialistas que generalistas dado que construir una carrera sobre un estrecho margen de conocimiento y habilidad limita las posibilidades y el caos. Mantener el horizonte abierto es mucho más complejo y agobiante dado que nunca será controlable. 


Resulta que yo soy un médico de familia generalista. Atiendo niños, jóvenes, adultos y ancianos. Me enfrento a todo tipo de situaciones, crisis vitales y enfermedades. A nacimientos y muertes. A la alegría y al horror. Y al mirar a mi alrededor, tanto cerca como en otros países, veo como muchos colegas prefieren colgar el generalismo en una percha y dedicarse a otras cuestiones más precisas, algo que entiendo perfectamente en los tiempos que corren. No les culpo dado que no puedo decir que no termine claudicando al ser la corriente tan fuerte, pues el problema de fondo es social, la gente está cada vez más nerviosa y perdida, y acaba en una consulta de un generalista pidiendo ayuda. Lo que antes atendían los sacerdotes acaba en consulta, los secretos que antes escuchaban los amigos o familiares también, los agobios que antes se dirimían en grupo o en la comunidad también. Y eso no hay quien lo pueda sostener.  Por eso preveo un futuro cercano con menos generalistas, con mucha población sin acceso a ellos, más perdidos quizá. 


La gente común tampoco lleva nada bien la incertidumbre. Un catarro que empieza los impulsa a acudir ayuda médica en horas, un grano, una marchita, una picadura de avispa. Recuerdo de niño las veces que a mi o a mis hermanos nos picaron. Duele de verdad, pero jamás nos llevaron al médico por eso. Cada año atiendo cientos de personas que no sabían qué hacer con su picadura. Imagino que mis colegas de urgencias saben de lo que hablo. Esto no hay sistema sanitario que lo contenga. Por eso colapsarán y no tendrán más remedio que crear muros que en lugar de usar la educación usarán copagos. 


Las sociedades y las organizaciones precisan de generalistas, la sanidad también. Y es una pena que se siga sin valorar y proteger esa visión, la única capaz de poner cada cosa en su sitio, planificar una estrategia sensata, llevar una armada a buen puerto. No porque sepa más sino por que se atreve a mirar los problemas de una forma abierta asumiendo la incomodidad de una mayor incertidumbre. Algo que generalmente, pocos quieren hacer.

Our liquid time offers us multiple advantages. We can access infinite digital libraries, listen to any song we want, watch any film, instantly contact anyone we want. It puts infinite products in the supermarket and often at surprising prices. But then it charges us dearly with pollution, background noise and uncertainty. For these reasons and others, no one wants to accept more uncertainty than necessary, and young people prefer to be specialists rather than generalists, since building a career on a narrow margin of knowledge and skill limits possibilities and chaos. Keeping the horizon open is much more complex and burdensome as it will never be controllable. 

I happen to be a general practitioner. I see children, young people, adults and the elderly. I deal with all kinds of situations, life crises and illnesses. Births and deaths. Joy and horror. And looking around me, both at home and abroad, I see how many colleagues prefer to hang generalism on a hanger and dedicate themselves to other, more precise issues, something that I fully understand in this day and age. I do not blame them, since I cannot say that they do not end up giving up because the current is so strong, because the underlying problem is social, people are increasingly nervous and lost, and end up in a generalist's office asking for help. What used to be attended to by priests ends up in the consulting room, the secrets that used to be heard by friends or family members, and the worries that used to be solved in a group or in the community. And there is no one who can sustain that.  That is why I foresee a near future with fewer generalists, with many people without access to them, perhaps more lost. 

Ordinary people don't cope well with uncertainty either. A cold that starts to drive them to seek medical help within hours, a pimple, a pimple, a wasp sting. I remember as a child how many times I or my siblings were stung. It really hurt, but we were never taken to the doctor for it. Every year I see hundreds of people who didn't know what to do about their sting. I imagine my colleagues in the emergency department know what I'm talking about. There is no health system that can contain this. So they will collapse and they will have no choice but to create walls that instead of using education will use co-payments. 

Societies and organisations need generalists, and so does healthcare. And it is a shame that this vision, the only one capable of putting everything in its place, of planning a sensible strategy, of bringing an army to a safe port, is still not valued and protected. Not because it knows more, but because it dares to look at problems in an open way, assuming the discomfort of greater uncertainty. Something that, generally speaking, few want to do.

El sueño del señor Xi. Mr. Xi's dream. 习先生的梦想





Todavía era de noche. Caminaba al lado del lago en esa hora mágica que precede el amanecer. Me acompañaban el silencio, el frío y su hermana la calma que me facilitaban contemplar la delicadeza de todo lo que me rodeaba. La arena que mis pasos hollaban, la suavidad de la superficie de las aguas, las estrellas lejanas y el reborde pétreo de los montes guardianes. En un determinado momento, sin saber cómo, toqué por un momento el sueño del señor Xi. En ese instante una carpa saltó abandonando la seguridad acuosa para atravesar el horizonte de sucesos que limita su vida y conectar con mi visión permitiéndome a mí conectar con la suya. De ese modo inaudito pude tocar el sueño del hombre más poderoso del mundo, atisbar por un momento sus instancias, los paneles de madera roja de las paredes, la oscuridad de los pasillos, el silencio agobiante de las zonas oscuras, la placidez de las constantes vitales que eran a su vez vigiladas por otros. Al sumergirse de nuevo el pez todo cesó, quedando tan solo un reguero de ondas en la superficie que nos contaba a la luna y a mi que había sido real. Fue entonces cuando sentí que los bancos de peces del fondo eran simétricos con las bandadas de gaviotas que flotaban lejos, las piedras del suelo con las titilantes estrellas y la luz del sol que empezaba a nacer con la tiniebla que empezaba a alejarse. Y que yo era tan solo un testigo que pasa y alumbra sin saberlo un universo que ansía ser contemplado para cobrar vida. 








Mr. Xi's dream



It was still dark. I was walking beside the lake in that magical hour before dawn. I was accompanied by the silence, the cold and its sister the calm that made it easier for me to contemplate the delicacy of everything around me. The sand that my footsteps trod, the softness of the surface of the waters, the distant stars and the stony edge of the guardian mountains. At a certain moment, without knowing how, I touched for a moment Mr. Xi's dream. In that instant a carp jumped out of the watery safety to cross the horizon of events that limits his life and connect with my vision allowing me to connect with his. In that unprecedented way I could touch the dream of the most powerful man in the world, glimpse for a moment his instances, the red wood panelling of the walls, the darkness of the corridors, the oppressive silence of the dark areas, the placidity of the vital signs that were in turn watched over by others. As the fish submerged again, everything ceased, leaving only a trail of ripples on the surface that told the moon and me that it had been real. It was then that I felt that the shoals of fish on the bottom were symmetrical with the flocks of seagulls floating away, the stones on the ground with the twinkling stars and the sunlight that was beginning to rise with the darkness that was beginning to recede. And that I was just a witness that passes by and unknowingly illuminates a universe that yearns to be contemplated in order to come to life.

Insuficiencia técnica. Technical insufficiency. 技术上的不足。





Cuando no se alcanza la suficiencia en una situación podemos definirla como insuficiente. Esa es la sensación que tengo al ver los datos de exceso de mortalidad por Comunidades Autónomas del año 2020. Se podrán aducir múltiples causas a nivel político, de gestión sanitaria, epidemiológicas, poblacionales, de condicionantes sociales de salud y de asistencia sanitaria entre otras. Pero seguiremos teniendo un buen montón de muertos encima de la mesa. Cuando alguno de ellos es alguien cercano, esto duele más. 

Como sanitario de Madrid siento vergüenza por el suspenso, me duele no haber podido hacerlo mejor, pero ¿era posible hacerlo mejor, como me preguntaba ayer Carles Francino desde su programa de radio?

Como médico he tratado de cuidar a mis pacientes lo mejor que he podido, usando el teléfono hasta la saciedad y planteando las visitas presenciales que he considerado necesarias. He salido a la comunidad a coordinarme con el Ayuntamiento, los Servicios Sociales, las farmacias comunitarias. He mantenido línea directa semanal con la pequeña residencia de ancianos que está adscrita a mi cupo de pacientes. He visibilizado la situación en redes sociales, prensa, radio y televisión. He mantenido contacto con grupos profesionales para mejorar el conocimiento de la epidemia según esta se ha ido desarrollando. En fin, he hecho todo lo que humanamente he podido.


Pero ¿se podría haber hecho más? ¿se podría haber hecho mejor?


Los mimbres de la estructura sanitaria donde trabajo están apolillados. Faltan médicos de forma continua, de forma estructural. Eso causa un deterioro lento y progresivo de la asistencia con la incómoda sensación añadida de que no vamos a mejor sino a peor. Cada año menos presupuesto, cada año menos médicos, cada año menos suplentes. 

Y lo peor no es lo mal que lo hayamos podido pasar los sanitarios ni lo mal que lo hayamos podido hacer. Sino que como sociedad no hemos aprendido nada, quedándonos en la discusión sobre qué político tiene la culpa en lugar de tratar de comprender qué ha sido lo más vulnerable o quién ha sufrido más. Y lo tenemos enfrente de los ojos aunque no lo queramos ver, los más vulnerables son los de siempre, los ancianos institucionalizados, los enfermos crónicos más graves, los enfermos mentales, los pueblos y barrios más deprimidos. ¿A alguien se le ha planteado reformular y reforzar el sistema de residencias de ancianos, los centros de salud más desprotegidos, los barrios más necesitados de ayuda?

Les digo que tristemente nada cambiara. Los políticos de turno seguirán desfinanciando poco a poco la Atención Primaria dejando que se hunda lentamente mientras se jubilan sus médicos dejando cada vez más plazas sin cubrir. Las residencias de ancianos seguirán como están y los enfermos crónicos o mentales cada vez tendrán más pastillas y menos cuidados. 

Muchos centros de salud están en situación de insuficiencia técnica tras arrastrar meses y años de faltas de médicos no suplidas y sobrecarga crónica progresiva. Podrán maquillarlo con planes de mejora, discursos o libros blancos, pero la realidad es la que es. Hoy lo que quizá sea el último milagro exigible a los sanitarios es que sigamos teniendo fe en lo que hacemos, porque la que teníamos en el sistema y sus gestores hace tiempo que se evaporó.      






 Technical insufficiency.


When sufficiency is not achieved in a situation, we can define it as insufficient. This is the feeling I have on seeing the data on excess mortality by spanich Autonomous Communitys for the year 2020. There may be many political, health management, epidemiological, population, social, health and healthcare conditioning factors, among others. But we will still have a lot of dead people on the table. When one of them is someone close to us, it hurts even more. 

As a health worker in Madrid I feel ashamed of the failure, it hurts me not to have been able to do better, but was it possible to do better, as Carles Francino asked me yesterday on his radio programme?

As a doctor, I have tried to look after my patients as best I could, using the telephone to my heart's content and making the face-to-face visits I considered necessary. I have gone out into the community to coordinate with the City Council, Social Services, community pharmacies. I have maintained a weekly direct line with the small old people's home that is assigned to my quota of patients. I have made the situation visible in social networks, press, radio and television. I have maintained contact with professional groups to improve knowledge of the epidemic as it has developed. In short, I have done everything I humanly could.


But could I have done more? Could I have done better?


The structure of the primary health care structure where I work is in tatters. There is a continuous, structural shortage of doctors. This causes a slow and progressive deterioration of care with the added uncomfortable feeling that we are not getting better but worse. Every year there is less budget, every year fewer doctors, every year fewer substitutes. 

And the worst thing is not how bad it has been for us health professionals, nor how badly we have done it. It is that as a society we have learnt nothing, remaining in the discussion about which politician is to blame instead of trying to understand what has been most vulnerable or who has suffered the most. And it is right in front of our eyes even if we don't want to see it, the most vulnerable are the same as always, the institutionalised elderly, the most seriously chronically ill, the mentally ill, the most depressed villages and neighbourhoods. Has anyone thought of reformulating and strengthening the system of old people's homes, the most unprotected health centres, the neighbourhoods most in need of help?

I tell you that sadly nothing will change. The politicians of the day will continue to defund Primary Care little by little, letting it slowly sink while their doctors retire, leaving more and more vacancies unfilled. Old people's homes will continue as they are and the chronically or mentally ill will have more and more pills and less and less care. 

Many health centres are in a situation of technical insufficiency after months and years of unfilled doctor shortages and progressive chronic overload. They can dress it up with improvement plans, speeches or white papers, but the reality is what it is. Today, what is perhaps the last miracle that can be demanded of healthcare professionals is that we continue to have faith in what we do, because the faith we had in the system and its managers has long since evaporated.

La poesía es un recurso narrativo de primer orden para recordar que no todo es prosa. Más allá de nuestra prosaica realidad hay posibilidades de visión poética. Rescatarlas es sinónimo de esperanza y belleza, ingredientes fundamentales para encontrar sentido en nuestro caminar. Muchas personas penan por no encontrarlo pese a nadar en la abundancia de objetos, informaciones y ruidos. 


Mi amigo Francesc Borrell me invita a participar en la revista que coordina con una aproximación al haiku como posible signo de revolución.

Haikus as signs of revolution.

Poetry is a pricipal narrative resource  to remind us that not everything is prose. Beyond our prosaic reality there are possibilities of poetic vision. Rescuing them bring us hope and beauty, fundamental ingredients for finding meaning in our journey. Many people feel existencial discomfort becouse they don't find it despite swimming in an abundance of objects, information and noises. 

My friend Francesc Borrell invites me to participate in the magazine he coordinates with an approach to haiku as a possible sign of revolution.

Indigestiones y gestiones pesadas en Atención Primaria. Imposible management in Primary Care. 初级保健中的消化不良和重度管理。

Gestionar la actual Atencion Primaria es imposible. Se puede decir con tecnicismos o florituras, ocupando libros blancos o dictámenes de sesudas comisiones. A mi tan solo tendrán que permitirme una frase y una breve argumentación de siete puntos.


1. Sin independencia de gestión real en los centros de salud no es posible una gestión óptima de los mismos.

2. El actual sistema impide y castiga el trabajo en equipo favoreciendo el individualismo y el corporativismo estamentario tóxico. Mientras se permita el “esto no es mi competencia” todo termina saturando al perfil profesional que las responde todas. 

3. La gestión de “recursos humanos” es nefasta. La huida de profesionales y el progresivo aumento de puestos de “difícil cobertura” lo demuestran. Cualquier encuesta de satisfacción laboral entre sanitarios echa humo.

4. Si la insatisfacción de los profesionales roza máximos históricos poca buena voluntad se les podrá exigir.

5. Sin compromiso político del Estado y las Comunidades es imposible hacer reformas de calado. Los parches a cuatro años vista no solucionarán un derrumbe que lleva muchos años siendo avisado por los profesionales. La división administrativa en 17 unidades de gestión (sin coordinación ni dirección real) tampoco ayuda mucho.

6. En cinco años se jubilan un 25% de los facultativos, no se están tomando medidas serias para afrontarlo mientras se permite la huida de personal. Esto tendrá un enorme costo social que los actuales responsables no pagarán.

7. Si una pandemia como la actual, con la cantidad de muertos y de dolor producido, no ha servido para tomar conciencia de la necesidad de una reforma de la Atención Primaria nada lo hará.



Por estas razones no es fácil asumir lo que hay. Un panorama heterogéneo de centros de salud que funcionan, otros están al límite y, cada vez más, otros en llamas. Sería deseable una cartografía fiel en tiempo real, que nos defendiera con transparencia e información fidedigna de la espesa capa de humo que parece sumir la Atención Primaria en un extraño bosque neblinoso donde todos dan vueltas sin parar y nadie parece avanzar a ningún sitio. 


Hay centros de salud que funcionan bien, tienen docencia, sus profesionales van a congresos y se siguen formando. Brindo por ellos. Pero hay otros en lo que la situación es muy dura con ausencias de larga duración no suplidas y acumulaciones de desgaste que rayan lo insano. Pensando en estos centros dejen de hacer planes, comisiones, libros blancos. Dejen de montar grupos de trabajo y reuniones por videoconferencia. Quien tenga responsabilidades de gestión que haga algo útil que ayude de verdad, si no es posible sigan en sus despachos, y que Dios les perdone.

Imposible management in Primary Care.



It is impossible to manage nos the Spanish primary care system. It can be said with technicalities or flourishes, occupying white papers or the opinions of brainy commissions. You will have to allow me just one sentence and a brief argumentation of seven points.


1. Without real management independence in the local health centres, optimal management of the health centres is not possible.

2. The current system prevents and punishes teamwork by favouring individualism and toxic corporatism. As long as "this is not my responsibility" is allowed, everything ends up saturating the professional profile that answers them all. 

3. The management of "human resources" is disastrous. The flight of professionals and the progressive increase in "difficult to fill" posts are proof of this. Any survey of job satisfaction among healthcare professionals is a smoke and mirrors survey.

5. Without political commitment from the State and the Communities, it is impossible to carry out far-reaching reforms. Four-year patches will not solve a collapse that professionals have been warning of for many years. The administrative division into 17 management units (without coordination or real direction) does not help much either.

6. In five years 25% of the doctors are retiring, no serious measures are being taken to deal with this while staff are being allowed to flee. This will have a huge social cost that the current managers will not pay.

7. If a pandemic like the current one, with the number of deaths and the pain it has caused, has not served to raise awareness of the need for a reform of primary care, nothing will.



For these reasons it is not easy to come to terms with what is there. A heterogeneous panorama of primary health centres that work, others that are on the brink and, increasingly, others that are on fire. It would be desirable to have a faithful mapping in real time, which would defend us with transparency and reliable information from the thick layer of smoke that seems to plunge Primary Care into a strange misty forest where everyone is going round and round and no one seems to be going anywhere. 


There are health centres that work well, have teaching, their professionals go to congresses and continue to train. Here's to them. But there are others in which the situation is very tough, with long-term absences that are not made up for and accumulations of wear and tear that border on the unhealthy. Thinking of these centres, stop making plans, commissions, white papers. Stop setting up working groups and videoconference meetings. Whoever has management responsibilities should do something that really helps, if that is not possible, stay in their offices, and may God forgive them.
