Aprender del fracaso de la Atención Primaria. Learning from the drowning of Primary Care. 從基層醫療失敗中學習





Estos días unos compañeros hicieron una manifestación en Twitter anunciando (de nuevo) el fallecimiento de la Atención Primaria en una muestra de dolor por la situación de penosidad que sus profesionales llevan sufriendo demasiado tiempo. 


La pandemia parece haber dado la puntilla a un sistema que ya estaba en las últimas y que incluso con la que está cayendo nadie parece querer cambiar, reforzar,  redimensionar o mejorar. 


Quizá podamos plantear la posibilidad de contemplar lo que tenemos delante y aprender de ello. Es la historia de un fracaso, es cierto, pero también lo es que hay dignidad en ese hundimiento como la ha habido en otros muchos antes. 


Probablemente sea correcto decir que en este asunto hemos fallado todas las partes implicadas. Los profesionales por no haber sabido hacernos valer, los gestores por haber maltratado a los anteriores, los políticos por no haber querido invertir ni priorizar el servicio y los ciudadanos por haber sobreusado una sanidad que bien es cierto se ha dejado sobreusar. 


Y lo malo de esta historia es que si una institución pública se malogra ya saben ustedes lo que pasa, el mercado termina abduciéndola e imponiendo sus reglas. En el caso sanitario va a haber muchos perdedores, en concreto la totalidad de los actores excepto los accionistas de los servicios privados que terminen haciéndose con el mango de la sartén. 


Tengo cerca grandes profesionales que están sufriendo como nunca antes lo habían hecho con un día a día insufrible por la sobrecarga, la pérdida de horizonte y la desesperanza. Sin liderazgo, sin gestores comprometidos, sin salida a la vista. Tengo cerca una sociedad que está recibiendo de lo lindo. Ni unos ni otros son conscientes de que pese a la samanta de palos que reciben lo peor es un tren que se acerca a gran velocidad, como bien decía el filósofo Žižek hace unos días en relación al cambio climático. Pero no nos pongamos trágicos, estábamos hablando de aprender y para esto vamos a tener que mirar donde más duele, a los mimbres de una sociedad cada vez más depauperada, individualizada y dependiente. Y viendo la herida hacernos preguntas, ¿cómo curar tanta complejidad? ¿Quién debe meter el bisturí para desbridar las partes muertas? ¿Quién proveerá cuidados? ¿Por dónde empezar?...


Tras una mañana atendiendo a una multitud de pacientes con covid y sus correspondientes contactos no tengo la seguridad de haber actuado bien con algún caso. A nadie importarán mis desvelos, ni agobios. Y eso quizá sea lo más dramático, por que quizá llegue un día en que a ningún sanitario le merezca la pena desvelarse. 











Learning from the drowning of Primary Care



A few days ago, some colleagues made a demonstration on Twitter announcing (once again) the demise of Primary Care in a show of grief for the situation of hardship that its professionals have been suffering for too long. 


The pandemic seems to have put the final nail in the coffin of a system that was already on its last legs and which, even with the current crisis, nobody seems to want to change, reinforce, resize or improve. 


Perhaps we can consider the possibility of looking at what is in front of us and learning from it. It is the story of a failure, it is true, but it is also true that there is dignity in this collapse as there has been in many others before. 


It is probably correct to say that all parties involved have failed in this affair. The professionals for not having known how to assert ourselves, the managers for having mistreated the previous ones, the politicians for not having wanted to invest or prioritise the service and the citizens for having overused a healthcare system which, it is true, has allowed itself to be overused. 


And the bad thing about this story is that if a public institution fails, you know what happens: the market ends up abducting it and imposing its rules. In the case of healthcare, there are going to be many losers, in particular all the actors except the shareholders of the private services that end up getting the handle of the frying pan. 


I am close to great professionals who are suffering as never before with an insufferable day to day life of overload, loss of horizon and hopelessness. Without leadership, without committed managers, with no way out in sight. I am close to a society that is on the receiving end. Neither one nor the other is aware that despite the samanta of blows they are receiving, the worst is a train that is approaching at great speed, as the philosopher Žižek rightly said a few days ago in relation to climate change. But let's not get tragic, we were talking about learning and for this we are going to have to look where it hurts the most, at the fabric of an increasingly impoverished, individualised and dependent society. And looking at the wound we ask ourselves questions: how to heal so much complexity? Who should put the scalpel in to debride the dead parts? Who will provide care? Where to start?....


After a morning of caring for a multitude of covid patients and their corresponding contacts, I am not sure if I have done well with two of them. No one will care about my sleeplessness, no one will care about my stresses and strains. And that is perhaps the most dramatic thing, because perhaps there will come a day when it will not be worthwhile for any doctor worring about.

Querido compañero, seamos optimistas: de esta crisis profunda como de otras que ha habido en la historia de la humanidad, si Dios quiere podemos salir y tambien el sistema sanitario puede salir una profunda renovacion, y mas en concreto en la medicina de familia.
Por desgracia nuestra labor siempre sera necesaria mientras seamos seres humanos y por tanto limitados, vulnerables e imperfectos.
Estas palabras de Don Gregorio (Marañon), que como todos los de su generacion sufrieron la devastacion social de la guerra civil,son clarificadoras
