Salud egocéntrica. Self-centred health. 以自我為中心的健康。

 




Salud no es bienestar. Lamentablemente todas las pantallas parecen decir lo contrario. Los paraísos artificiales que nos venden, las sonrientes modelos y personajes célebres, la vida de postín de aquellos famosos en el Olimpo de la fama, el poder y el placer… todos señalan hacia el bienestar. Y por supuesto a los bienes y servicios que la hacen posible. “El paraíso en la tierra existe” nos dicen para luego invitarnos a pasar por caja. 



¿Pero entonces qué hago con mi vida cuando me sale un grano, me duele el cuello o tengo la regla? Cada vez con más frecuencia acudimos al sistema sanitario buscando una pastilla que nos libere de cualquier contratiempo, ya sea una cuestión de salud menor, un problema con el jefe o la tristeza por haber perdido a un ser querido. Todo tiende a medicalizarse para recuperar lo antes posible el bienestar perdido. Lamentablemente no es posible alejar del todo la sombra de la vejez, la enfermedad, el deterioro y la pérdida progresiva de funciones. El malestar inherente a la vida está tan pegado a nosotros como la sombra a cualquier cuerpo que reciba una fuente de luz. 


Lo que estamos viendo es que esta tendencia de salud egocéntrica, lúdica y placentera es uno de los factores que está reventando los sistemas sanitarios europeos que no pueden dar a basto atendiendo a los enfermos al recibir una presión progresiva de sanos que consultan por diversos malestares. 


No habrá presupuesto que solucione este problema si como sociedad no nos damos cuenta de la falacia que nos quieren vender. Lo malo es que el mercado no nos lo va a poner fácil al apoyarse en una industria de la publicidad y el entretenimiento cuyos argumentos tienen una apariencia sólida. Todos esos rostros de apariencia feliz que aparecen en los medios y en las redes sociales son muy convincentes dado que cuando se ponen enfermos o tienen mala cara no les vemos. Queremos sentirnos bien y ser lo más productivos, fuertes y bellos dentro de nuestras posibilidades. Cuando la salud no nos acompaña al 100% no lo llevamos bien. El sufrimiento no está de moda ni tiene valor alguno. La enfermedad tampoco y en la medida de lo posible tratamos de esconderla. En esta sociedad postmoderna es paradójico que pese al progresivo bienestar que algunas sociedades disfrutan cada vez nos sentimos más cansados y agotados. Quizá haya que reaprender a llevar la propia sombra con dignidad tal y como hacían nuestros mayores y han hecho todos los familiares que nos han precedido. Aceptar que la vida implica una dosis de malestar y que no siempre es posible estar en plena forma es hoy un reto. Aprender que la salud es algo más amplio que estar bien también.







 Self-centred health.


Health is not wellness. Unfortunately, all the screens seem to say the opposite. The artificial paradises they sell us, the smiling models and celebrities, the high life of those celebrities on the Olympus of fame, power and pleasure... all point to wellness. And of course to the goods and services that make it possible. "Paradise on earth exists" we are told and then invited to check out. 


But then what do I do with my life when I get a pimple, a sore neck or my period? More and more often we turn to the health system looking for a pill to get rid of any setback, whether it is a minor health issue, a problem with the boss or the sadness of having lost a loved one. Everything tends to be medicalised in order to regain lost wellbeing as quickly as possible. Unfortunately, the shadow of old age, illness, deterioration and progressive loss of function cannot be entirely removed. The inherent discomfort of life is as close to us as the shadow is to any body that receives a light source. 

What we are seeing is that this egocentric, playful and pleasurable health trend is one of the factors that is bursting European health systems that cannot cope with the increasing pressure of healthy people consulting for various ailments. 

There will be no budget to solve this problem if we as a society do not realise the fallacy they want to sell us. The bad news is that the market is not going to make it easy for us by relying on an advertising and entertainment industry whose arguments look solid. All those happy-looking faces in the media and on social media are very convincing because when they get sick or look bad we don't see them. We want to feel good and be as productive, strong and beautiful as we can be. When health is not 100% with us, we don't cope well. Suffering is not fashionable and has no value. Neither is illness and as far as possible we try to hide it. In this postmodern society it is paradoxical that despite the progressive wellbeing that some societies enjoy, we feel more and more tired and exhausted. Perhaps we need to relearn to carry our own shadow with dignity, as our elders did and as all the relatives before us have done. Accepting that life involves a dose of discomfort and that it is not always possible to be in top form is a challenge today. Learning that health is more than just being well is also a challenge.

Gestión de la serenidad. Serenity management. 寧靜管理

 




Como saben los lectores de este blog colaboro con el equipo de Gestión Clínica Varela compartiendo visiones y reflexiones con la intención aportar de elementos que favorezcan la mejora de nuestro sistema sanitario. Dado el desquicie que parece acompañarnos a los sanitarios durante ya demasiado tiempo me parecía oportuno el tema de la serenidad. Dejo este pequeño texto como semilla con la esperanza de que puedan regar la serenidad en sus propios jardines. 





GESTIÓN DE LA SERENIDAD

 

La pandemia mundial que vivimos está siendo un tiempo de tensión, sobresalto, preocupación, incertidumbre, tempestad, crispación, prisa y ruido. Su gestión a nivel político, social y sanitario parece que podría haber sido muy mejorable por el gran nivel de complejidad que ha supuesto para todos. Se han hecho muchas cosas, dicho muchas cosas y equivocado otras muchas. Lo que quizá hemos echado de menos en más serenidad. Por esto considero pertinente reflexionar sobre cómo podríamos mejorar la gestión de la serenidad en el mundo sanitario, dado que este remedio es un buen contraveneno de los sustantivos que iniciaban esta disertación.

Estructuraré el artículo en cuatro pequeños apartados que tendrán en cuenta a pacientes, profesionales sanitarios, gestores intermedios y alta dirección.

 

Serenidad para pacientes.

Muchas personas han sentido agobio, soledad y abandono en estos meses, pero quizá los ancianos y enfermos más si cabe. La situación actual es de crisis sistémica social, y están afectados todos los niveles: económicos, políticos, éticos, estéticos… Entran en crisis los valores, aquello a lo que damos importancia, y también los referentes, aquellas ideas, personas e instituciones que nos guían o nos orientan. ¿Qué podría aportar serenidad a los pacientes? Probablemente saber que van a seguir teniendo una atención sanitaria de calidad, saber que tendrán profesionales que les van a atender y acompañar. Saber que la salud es un bien muy preciado que hay que cuidar y proteger y que tendrán ayuda y apoyo para ello.

El hecho de que todo haya seguido su curso pese a que el sistema sanitario haya estado bloqueado y sobrecargado tantos meses pone a las claras que la gran mayoría puede vivir sin sanidad. Entiéndanme, lo que quiero decir es que es posible gestionar la mayoría de los problemas menores de salud con remedios caseros y autocuidados. Esto implica que es fundamental optimizar el uso de la sanidad pública para acudir a ello solo cuando esté justificado, favoreciendo la responsabilidad de cada cual con su salud para evitar acudir por cuestiones menores que terminan sobrecargando un sistema sanitario frágil.

Devolver la gestión de los problemas menores de salud a la familia y a la comunidad parece que debería ser una gran prioridad a la que sumar el apoyo a este nivel que merecen aquellas personas aquejadas de enfermedades crónicas, graves o poco frecuentes.

Pero los pacientes seguirán necesitando a sus profesionales sanitarios y precisaran que estén serenos. Un profesional crispado ayuda poco.

 

Serenidad para profesionales sanitarios.

La crisis de modelo sanitario que arrastrábamos saltó por los aires al sumarse la crisis pandémica y el sistema se rompió y con él muchos profesionales.

Muchos compañeros del mundo sanitario han tocado fondo en estos meses de pandemia. Su salud física o mental se ha resentido, su labor asistencial se ha desbaratado o sobrecargado a límites nunca vistos, su vida ha sido muy ardua y amarga durante mucho tiempo.

Nos hemos sentido abandonados por los gestores y políticos, hemos sentido miedo por nuestras familias, pacientes y por nosotros mismos, hemos sentido enfado por las medidas de mala gestión y por algunas conductas de ciudadanos poco conscientes con lo que estaba pasando. Hemos atravesado una noche oscura de impotencia por no ser capaces de hacer más y por haber llenado las manos del dolor y sufrimiento de muchos enfermos y familias.

Hemos perdido la serenidad y entrado en un modo de supervivencia profesional del que será difícil salir. Esta situación no beneficia a nadie.

Será fundamental mejorar el  “mantenimiento” tanto personal como a nivel de equipos de trabajo. Curar las quemaduras, aliviar las heridas, aplicar calmantes y emolientes. Será necesario tener paciencia en una rehabilitación que se adivina prolongada y dolorosa. Y tratar de buscar esa fuente de calma, alegría y serenidad que todo ser humano lleva dentro y suele resonar con la naturaleza, el arte y la belleza.

Me atrevo a aventurar que será preciso enriquecer la visión asistencial con otra bioética, comunitaria y bio-psico-social-espiritual que nos permitan conectar con todo aquello que pueda ayudar a nuestros pacientes y a nosotros mismos.

 

Serenidad para los gestores sanitarios intermedios.

La pandemia ha puesto en evidencia a todo el sistema sanitario pero quizá han sido los gestores intermedios los que más impotencia han sentido al tener pocas opciones reales de gestión entre unos altos directivos perdidos y confusos y unos profesionales asistenciales desbordados y sobrepasados.

Sus labores de gestión del servicio y de control se han visto suspendidas estos meses lo que les ha relegado a sus despachos con poca cosa que hacer.  En estos tiempos en los que los intermediarios desaparecen alguno se habrá dado cuenta de su futuro incierto viendo quizá lo que está pasando últimamente con los empleados de banca. Quizá sea el momento de cambiar el modelo de gestión de datos (que cada vez harán mejor los algoritmos) por otro de gestión de personal en el que se invierta tiempo y energía en cuidar, apoyar, asistir, aconsejar, escuchar y favorecer a la comunidad de asistenciales.

 

Serenidad para la alta gestión y los responsables políticos.

Cuando el nivel de ruido social es muy intenso los máximos responsables tienen la obligación de aportar claridad, rigor y serenidad. No ha sido el caso. Los asesores no han dado abasto y se han producido protocolos, estadísticas y normas a gran velocidad, algo que ha vuelto locos a gestores intermedios y sobre todo a profesionales. Hemos echado en falta más inteligencia de sistemas, más rigor científico, mejor comunicación a la sociedad y a la organización sanitaria.

El valor principal debería haber sido protección del profesional y de la ciudadanía pero no se ha conseguido. La crispación y confrontación política de los responsables se ha trasladado a la población que ha aumentado su nivel de miedo y enfado ante el espectáculo de sus representantes.

Es cierto que la exposición mediática es muy tóxica y aquellos que por su puesto la han de soportar tienen una gran carga sobre sus hombros. Pero debería ser principal obligación de algún asesor tratar de mejorar la serenidad, silencio interior y paz mental de sus jefes.

 

 

En definitiva podemos convenir en que por múltiples razones, a las que se suma la pandemia, habitamos una sociedad crispada y eso es causa y motivo de malestar y enfermedad. La sanidad no tiene poder por si sola de revertir esta toxicidad pero tal vez debería tenerla en cuenta para trabajar por mantener y potenciar la serenidad de sus profesionales y como fin último la de todos aquellos que a ella recurran. Como pasa en el mar, las olas de la vida se suceden unas a otras. Hemos sido testigo de algunas estos meses, vendrán más. Para poder navegarlas como sociedad civil y como sistema sanitario tendremos que dar lo mejor de nosotros mismos pero esto solo es posible con una base de calma. Darnos cuenta del valor de esa serenidad es clave para elegir el mejor rumbo y desempeñar la propia función lo mejor que se pueda.



 


 

SERENITY MANAGEMENT

 

The global pandemic we are living through has been a time of tension, shock, worry, uncertainty, storm, haste and noise. Its management at the political, social and health levels seems to have been very improvable due to the great level of complexity it has entailed for everyone. Many things have been done, many things have been said and many things have been wrong. What we have perhaps missed is more serenity. For this reason, it would be pertinent to reflect on how we could improve the management of serenity in the healthcare world, given that this remedy is a good countervenom to the nouns that began this dissertation.

I will structure the article in four small sections that will consider patients, healthcare professionals, middle management and high directors.

 

Serenity for patients.

Many people have felt overwhelmed, lonely and abandoned in recent months, but perhaps the elderly and the sick even more so. The current situation is one of systemic social crisis, and all levels are affected: economic, political, ethical, esthetic... Values are in crisis, that to which we give importance, and also the referents, those ideas, people and institutions that guide us or orient us. What could bring serenity to patients? Probably knowing that they will continue to receive quality health care, knowing that they will have professionals who will attend to them and accompany them. Knowing that health is a very precious asset that must be cared for and protected and that they will have help and support to do so.

The fact that everything has continued despite the fact that the healthcare system has been blocked and overloaded for so many months makes it clear that the vast majority can live without healthcare. Understand me, what I mean is that it is possible to manage most minor health problems with home remedies and self-care. This implies that it is essential to optimize the use of public health care in order to resort to it only when justified, favoring the responsibility of each person with their health to avoid going for minor issues that end up overloading a fragile health system.

Returning the management of minor health problems to the family and the community seems to be a major priority, in addition to the support at this level that people suffering from chronic, serious or rare diseases deserve.

But patients will continue to need their healthcare professionals and they will need them to be calm. A stressed professional is of little help.

 

Serenity for healthcare professionals.

The crisis of the healthcare model that we were dragging along exploded when the pandemic crisis was added and the system broke down and with it many professionals.

Many colleagues in the healthcare world have hit rock bottom in these months of pandemic. Their physical or mental health has suffered, their care work has been disrupted or overloaded to unprecedented limits, their personal life has been very arduous and bitter for a long time.

We have felt abandoned by managers and politicians, we have felt fear for our families, patients and ourselves, we have felt anger for the mismanagement measures and for some behaviors of citizens unaware of what was happening. We have gone through a dark night of impotence for not being able to do more and for having filled our hands with the pain and suffering of many patients and families.

We have lost our serenity and entered a professional survival mode from which it will be difficult to get out. This situation benefits no one.

It will be essential to improve "maintenance" both personally and at the level of work teams. Healing burns, soothing wounds, applying painkillers and emollients. It will be necessary to have patience in a rehabilitation that is expected to be prolonged and painful. And try to find that source of calm, joy and serenity that every human being carries inside and usually resonates with nature, art and beauty.

I dare to venture that it will be necessary to enrich the healthcare vision with another bioethical, communitarian and bio-psycho-social-spiritual vision that will allow us to connect with everything that can help our patients and ourselves.

 

Serenity for intermediate healthcare managers.

The pandemic has exposed the entire healthcare system, but perhaps it has been the middle managers who have felt the most helpless, having few real management options between lost and confused senior managers and overwhelmed and overstretched healthcare professionals.

Their service management and control tasks have been suspended in recent months, which has relegated them to their offices with little to do.  In these times in which intermediaries are disappearing, some may have realized their uncertain future, perhaps seeing what is happening lately with bank employees. Perhaps it is time to change the model of data management (which algorithms will do better and better) for one of personnel management in which time and energy is invested in caring for, supporting, assisting, advising, listening to and favoring the care community.

 

Serenity for high direction and policy makers.

When the level of social noise is very intense, top management has the obligation to provide clarity, rigor and serenity. This has not been the case. The advisors have not been able to cope and protocols, statistics and standards have been produced at great speed, something that has driven middle management and, above all, professionals crazy. We have lacked more systems intelligence, more scientific rigor, better communication to society and to the healthcare organization.

The main value should have been the protection of professionals and citizens, but this has not been achieved. The tension and political confrontation of those in charge has been transferred to the population, which has increased its level of fear and anger at the spectacle of its representatives.

It is true that media exposure is very toxic and those who, of course, have to bear it have a great burden on their shoulders. But it should be the main obligation of any advisor to try to improve the serenity, inner silence and peace of mind of their bosses.

 

 

In short, we can agree that for many reasons, in addition to the pandemic, we live in a tense society and this is the cause and motive of malaise and disease. Health care alone has no power to reverse this toxicity, but perhaps it should take it into account in order to work to maintain and enhance the serenity of its professionals and, as an ultimate goal, that of all those who resort to it. As happens in the sea, the waves of life follow one after the other. We have witnessed some of them in recent months, and more will come. To be able to navigate them as a civil society and as a healthcare system, we will have to give the best of ourselves, but this is only possible with a base of calm. Realizing the value of that serenity is key to choosing the best course and performing one's role to the best of one's ability.

El centro de gravedad del Sistema Sanitario. The centre of gravity of the Health System. 健康系統的重心

 





A casi nadie le gustan los días “oficiales” que cada jornada ponen el foco en instituciones, enfermedades o motivos varios, pero no quisiera terminar el que visibiliza la Atención Primaria haciendo una reflexión. Para ello me ayudo de Franco Battiato que ha fallecido esta semana dejándonos una estela de música e inspiración. Los más jóvenes seguramente no lo hayan disfrutado pero para los de mi generación sí que marcó tendencia. Una de sus canciones más célebres habla de un centro de gravedad permanente y me hace preguntarme cuál sería hoy el de nuestro sistema sanitario. 


Si rebusco en su diseño original, tengo que reconocer que los sistemas sanitarios públicos se diseñaron con un cimiento basado en Atención Primaria del que pendían servicios hospitalarios especializados que progresivamente se fueron ampliando y complejizando. El centro de gravedad ha ido pasando pues desde primaria a hospitalaria a medida que el presupuesto y las prioridades políticas pusieron el foco en lo que más votos daba y no en lo que más beneficiaba a la sociedad. Y todo ello a su vez sometido a la poderosa fuerza gravitacional del mercado que tiende a centrar todo sus giros en el beneficio económico. 


Pero independientemente de la lamentable situación presupuestaria, de gestión y de motivación que sufre el primer nivel asistencial tal vez venga bien preguntarse cuál es el centro de gravedad de sus profesionales. Y no es fácil responder cuando estamos zarandeados por una potente fuerza centrípeta que nos mueve cada vez más rápido obligándonos a atender cada vez más pacientes en menos tiempo. La verdad es que no estamos centrados. La irritación, agotamiento, desesperanza y otros sentimientos cromáticos oscuros tiñan la actividad asistencial de muchos impidiendo hacer pie en un centro de gravedad que produzca estabilidad y equilibrio. 


Evidentemente estar descentrados y desequilibrados no es lo mejor ni para nosotros ni para nuestro pacientes. ¿Pero cómo asumir que nuestras Consejerías de Salud no den respuesta a la falta de profesionales? ¿Cómo encarar un futuro en el que se va a jubilar un cuarto de la plantilla en pocos años sin que nuestros políticos hagan absolutamente nada para afrontar semejante desastre?


Me siento a mirar el mar escuchando a Battiato y sonrío al recordar que sigo siendo responsable de mi propio centro de gravedad. Pase lo que pase fuera cada cual es soberano en su equilibrio interno. Eso me da un punto de apoyo para colocar la necesaria palanca de esperanza que pueda levantar el peso que me toca afrontar cada día. Espero que mis compañeras y compañeros puedan también encontrarlo. 







The centre of gravity of the Health System Hardly anyone likes the "official" days that focus on institutions, diseases or various reasons, but I would not like to end the day that makes Primary Care visible by reflecting on it. To do so, I would like to invite Franco Batiato, who passed away this week, leaving us a trail of music and inspiration. Younger people may not have enjoyed him, but for those of my generation he was a trendsetter. One of his most famous songs speaks of a permanent centre of gravity and makes me wonder what the centre of gravity of our healthcare system would be today. If I go back to its original design, I have to recognise that public health systems were designed with a foundation based on primary care, with specialised hospital services that gradually expanded and became more complex. The centre of gravity has therefore shifted from primary to hospital care as the budget and political priorities focused on what brought in the most votes rather than on what most benefited society. And all this in turn has been subject to the powerful gravitational pull of the market, which tends to focus on economic profit. But regardless of the lamentable budgetary, management and motivational situation that the first level of care suffers, perhaps it is worth asking ourselves what is the centre of gravity of its professionals. And it is not easy to answer when we are buffeted by a powerful centripetal force that is moving us faster and faster, forcing us to see more and more patients in less and less time. The truth is that we are not centred. Irritation, exhaustion, hopelessness and other dark chromatic feelings stain the care activity of many, preventing us from standing on a centre of gravity that produces stability and balance. Clearly, being off-centre and unbalanced is not the best thing for us or for our patients, but how can we deal with the fact that our Health Departments do not respond to the lack of professionals? How can we face a future in which a quarter of the workforce is going to retire in a few years and our politicians do absolutely nothing to deal with such a disaster? I sit looking out to sea listening to Batiato and I smile as I remember that I am still responsible for my own centre of gravity. Whatever happens outside, everyone is sovereign in their own internal balance. That gives me a foothold to place the necessary lever of hope that can lift the weight I have to face every day. I hope that my colleagues can also find it.

Consumir salud. Consuming health. 消耗健康。

Convertir la salud en un producto de consumo ha terminado transformando la asistencia sanitaria en un grandísimo negocio. Los sistemas sanitarios públicos no hemos sido capaces de mantenernos fuera de esta corriente por lo que cada vez estamos más cerca de ser engullidos por un remolino de giro veloz.

El mercado imprime velocidad a todos los procesos y disminuye la tolerancia al malestar inherente a la vida. Esto implica más consultas y más consumo de productos sanitarios. El fusible del sistema es el médico de familia que se supone que es el que soluciona el 90% de los problemas de salud y deriva al nivel hospitalario solo lo complejo. ¿Pero qué pasa si sobrecargamos y anulamos a los médicos de cabecera? Pues que el sistema se acelera al aumentar la carga de trabajo hospitalaria: más procesos, más intervenciones diagnósticas, más tratamientos. Y en cada vuelta de tuerca la población se hace más dependiente del sistema y menos autosuficiente.

Si las personas que trabajan en el mundo sanitario no disponen de los recursos y el tiempo suficiente para desarrollar su labor sus respuestas no serán suficientes y el sistema público acaba colapsando y obligando a que muchas personas tengan que buscarse la vida con seguros privados de salud. De esta forma gana el mercado y pierde la sociedad y el individuo.

Esta situación no es sencilla de revertir. Hacen falta recursos pero sobretodo conciencia. Darnos cuenta de que la salud es algo profundamente importante que ha de ser cuidado y valorado pero no mercantilizado. Esto implica asumir la responsabilidad individual de cuidarse uno mismo y a nuestras familias a la vez que aprender a hacer un uso prudente de los recursos sanitarios. También implica cuidar la salud social con medidas de salud pública, cuidado del entorno y potenciación de las relaciones comunitarias.

La tecnología tal vez proporcione mejoras concretas y puntuales en el ámbito sanitario, habrá que aprovecharlas pero sin albergar la fantasía de que nos arreglarán la vida y solucionarán la enfermedad. Parece coherente invitar a que como sociedad hagamos una reflexión sobre qué es para nosotros la salud y qué hacer para mantenerla. Si no lo tenemos claro será el mercado quien responda.






Consuming health.


Turning health into a consumer product has ended up transforming healthcare into a huge business. Public health systems have not been able to keep out of this current and we are increasingly close to being swallowed up in a whirlpool of rapid spinning.

The market is speeding up all processes and decreasing tolerance for the inherent discomfort of life. This means more consultations and more consumption of health products. The fuse of the system is the family doctor who is supposed to solve 90% of health problems and refers only the complex to the hospital level. But what happens if we overload and overrule family doctors? Well, the system is accelerated by increasing the hospital workload: more processes, more diagnostic interventions, more treatments. And at every turn of the screw the population becomes more dependent on the system and less self-sufficient. 

If the people who work in the health sector do not have sufficient resources and time to carry out their work, their responses will not be sufficient and the public system ends up collapsing, forcing many people to seek their living through private health insurance. In this way, the market wins and society and the individual loses. 

This situation is not easy to reverse. We need resources, but above all awareness. We need to realise that health is something profoundly important that must be cared for and valued but not commodified. This means taking individual responsibility for looking after ourselves and our families while learning to make prudent use of health resources. It also means taking care of social health through public health measures, caring for the environment and enhancing community relations. 

Technology may provide specific and timely improvements in the health field, and we must take advantage of them, but without harbouring the fantasy that they will fix our lives and solve our illnesses. It seems coherent to invite us as a society to reflect on what health means to us and what we should do to maintain it. If we are not clear about this, it will be the market that will respond.

Silencio. Silence. 安靜

Tras muchos años escribiendo un blog y participando en redes sociales me voy dando cuenta de que cada vez hago menos ruido. Es inevitable mantener un mínimo, pero parece objetivo decir que abro menos la boca. Conozco muchos casos de compañeros que durante este tiempo desaparecieron, cansados, quemados o directamente contrariados. De un día para otro ya no estaban. Siempre entendí que es una posibilidad, una de las pocas que nos demuestra que tenemos algún pequeño grado de libertad en la recámara. En mi caso tengo varias razones, la primera tiene que ver con la pandemia que como a todos me alcanzó como una gran ola marina, me derribó, me dio bastantes vueltas y me hizo tragar una considerable cantidad de agua salada. Tras las marejada oceánica, o más bien de comparsa, vino un viento furioso y cansino de grito y de ruido. Los medios de comunicación, las pantallas y las redes sociales se pusieron como locas en una espiral de opiniones, descalificaciones, tontunas y demás que azuzaron el miedo y el enfado hasta límites nunca vistos. Ante tanto desvarío muchos nos tuvimos que activar en modo supervivencia, y esto implica cultivar y expandir silencio.

La segunda razón tiene que ver con la impotencia al relacionarme con mi trabajo y con la sociedad. Esa sensación de no poder hacer pese a ver el desastre aproximarse. Esa comprensión que te obliga a entender que el barco se hundirá pase lo que pase. Lo he visto de primera mano en mi centro de salud. Ni siquiera una pandemia como la que hemos vivido ha servido para que nos refuercen, potencien o reformen. No hubo rastreadores, fuimos nosotros, no hubo refuerzos de médicos y enfermeras, tuvimos que desdoblarnos y deslomarnos, no hubo mejoras de gestión, ni reducción de la burocracia, ni nuevos planteamientos estratégicos, hemos seguido haciendo lo que buenamente hemos podido. La sanidad se ha manifestado a las claras como una sanidad zombi, ni viva ni muerta. Fiel reflejo de una sociedad que a su vez avanza en un proceso de “zombificación” y en la que las instituciones y la política se van alejando de la vida para convertirse en entes grises cada vez más incapaces de dar el servicio para el que fueron encomendadas.

Y la tercera razón que pude argüir tiene que ver conmigo. Con la profunda necesidad de callar para alejarme del bullicio de la costa y remar mar adentro. Y mar adentro, como en el desierto, uno inevitablemente tiene que vérselas con sigo mismo. En esa bastedad vacía descubrimos que estamos habitados por una presencia que nos trasciende, algo difícil de nombrar pero dotado de una fuerza fundante que mana como fuente y que al beberla nos regala fuerza, vida y sentido. 


En estos tiempos lo fácil es unirse a la turba y gritar sin parar. No me busquen ahí. Nunca fue mi sitio. Si quieren encontrarme estaré por despoblado, caminando quizá mirando nubes. 






Silence

After many years writing a blog and participating in social networks I am realizing that I am making less and less noise. It is inevitable to maintain a minimum, but it seems objective to say that I open my mouth less. I know many cases of colleagues who during this time disappeared, tired, burned out or directly disgruntled. From one day to the next they were gone. I always understood that it is a possibility, one of the few that shows us that we have some small degree of freedom. In my case I have several reasons, the first has to do with the pandemic that, like everyone else, hit me like a big ocean wave, knocked me down, tossed me around a lot and made me swallow a considerable amount of salt water. After the oceanic tidal waves, or rather as a companion, came a furious and tiring wind of shouting and noise. The media, the screens and the social networks went crazy in a spiral of opinions, disqualifications, nonsense and others that whipped up fear and anger to unprecedented limits. Faced with so much raving, many of us had to activate survival mode, and this implies cultivating and expanding silence.

The second reason has to do with the impotence when relating to my work and to society. That feeling of not being able to do anything despite seeing disaster approaching. That realization that forces you to understand that the ship will sink no matter what. I have seen it firsthand in my health center. Not even a pandemic like the one we have lived through has served to strengthen, empower or reform us. There were no trackers, it was us, there were no reinforcements of doctors and nurses, we had to split up and get out of the way, there were no improvements in management, no reduction in bureaucracy, no new strategic approaches, we have continued to do what we have been able to do. The healthcare system has clearly shown itself to be a zombie healthcare system, neither alive nor dead. A true reflection of a society which in turn is advancing in a process of "zombification" and in which institutions and politics are moving away from life to become gray entities increasingly incapable of providing the service for which they were entrusted.

And the third reason I could argue has to do with me. With the deep need to be quiet in order to get away from the hustle and bustle of the coast and row out to sea. And out to sea, as in the desert, one inevitably has to deal with oneself. In that empty wasteland we discover that we are inhabited by a presence that transcends us, something difficult to name but endowed with a founding force that flows like a fountain and that when we drink it gives us strength, life and meaning. 

In these times it is easy to join the mob and shout without stopping. Don't look for me there. It was never my place. If you want to find me, I'll be in the wilderness, walking, maybe looking at clouds.
