La desaparición de los generalistas. The disappearance of the generalists. 普通人的消失。






Nuestro tiempo líquido nos ofrece múltiples ventajas. Es posible acceder a infinitas bibliotecas digitales, escuchar cualquier canción que deseemos, ver cualquier película, contactar al instante con quien queramos. Nos pone en el supermercado infinitos productos y con frecuencia a precios sorprendentes. Pero luego nos lo cobra caro con contaminación, ruido de fondo e incertidumbre. Es mucho más sencillo pensar en distopías que en utopías, más fácil quejarse de la propia situación que bendecirla, más frecuente sentirse estresado que en paz. 


Por estas razones y por otras nadie quiere aceptar más incertidumbre de la necesaria y los jóvenes prefieren ser especialistas que generalistas dado que construir una carrera sobre un estrecho margen de conocimiento y habilidad limita las posibilidades y el caos. Mantener el horizonte abierto es mucho más complejo y agobiante dado que nunca será controlable. 


Resulta que yo soy un médico de familia generalista. Atiendo niños, jóvenes, adultos y ancianos. Me enfrento a todo tipo de situaciones, crisis vitales y enfermedades. A nacimientos y muertes. A la alegría y al horror. Y al mirar a mi alrededor, tanto cerca como en otros países, veo como muchos colegas prefieren colgar el generalismo en una percha y dedicarse a otras cuestiones más precisas, algo que entiendo perfectamente en los tiempos que corren. No les culpo dado que no puedo decir que no termine claudicando al ser la corriente tan fuerte, pues el problema de fondo es social, la gente está cada vez más nerviosa y perdida, y acaba en una consulta de un generalista pidiendo ayuda. Lo que antes atendían los sacerdotes acaba en consulta, los secretos que antes escuchaban los amigos o familiares también, los agobios que antes se dirimían en grupo o en la comunidad también. Y eso no hay quien lo pueda sostener.  Por eso preveo un futuro cercano con menos generalistas, con mucha población sin acceso a ellos, más perdidos quizá. 


La gente común tampoco lleva nada bien la incertidumbre. Un catarro que empieza los impulsa a acudir ayuda médica en horas, un grano, una marchita, una picadura de avispa. Recuerdo de niño las veces que a mi o a mis hermanos nos picaron. Duele de verdad, pero jamás nos llevaron al médico por eso. Cada año atiendo cientos de personas que no sabían qué hacer con su picadura. Imagino que mis colegas de urgencias saben de lo que hablo. Esto no hay sistema sanitario que lo contenga. Por eso colapsarán y no tendrán más remedio que crear muros que en lugar de usar la educación usarán copagos. 


Las sociedades y las organizaciones precisan de generalistas, la sanidad también. Y es una pena que se siga sin valorar y proteger esa visión, la única capaz de poner cada cosa en su sitio, planificar una estrategia sensata, llevar una armada a buen puerto. No porque sepa más sino por que se atreve a mirar los problemas de una forma abierta asumiendo la incomodidad de una mayor incertidumbre. Algo que generalmente, pocos quieren hacer.

Our liquid time offers us multiple advantages. We can access infinite digital libraries, listen to any song we want, watch any film, instantly contact anyone we want. It puts infinite products in the supermarket and often at surprising prices. But then it charges us dearly with pollution, background noise and uncertainty. It is much easier to think in dystopias than in utopias, easier to complain about one's situation than to bless it, more frequent to feel stressed than to feel at peace. 

For these reasons and others, no one wants to accept more uncertainty than necessary, and young people prefer to be specialists rather than generalists, since building a career on a narrow margin of knowledge and skill limits possibilities and chaos. Keeping the horizon open is much more complex and burdensome as it will never be controllable. 

I happen to be a general practitioner. I see children, young people, adults and the elderly. I deal with all kinds of situations, life crises and illnesses. Births and deaths. Joy and horror. And looking around me, both at home and abroad, I see how many colleagues prefer to hang generalism on a hanger and dedicate themselves to other, more precise issues, something that I fully understand in this day and age. I do not blame them, since I cannot say that they do not end up giving up because the current is so strong, because the underlying problem is social, people are increasingly nervous and lost, and end up in a generalist's office asking for help. What used to be attended to by priests ends up in the consulting room, the secrets that used to be heard by friends or family members, and the worries that used to be solved in a group or in the community. And there is no one who can sustain that.  That is why I foresee a near future with fewer generalists, with many people without access to them, perhaps more lost. 

Ordinary people don't cope well with uncertainty either. A cold that starts to drive them to seek medical help within hours, a pimple, a pimple, a wasp sting. I remember as a child how many times I or my siblings were stung. It really hurt, but we were never taken to the doctor for it. Every year I see hundreds of people who didn't know what to do about their sting. I imagine my colleagues in the emergency department know what I'm talking about. There is no health system that can contain this. So they will collapse and they will have no choice but to create walls that instead of using education will use co-payments. 

Societies and organisations need generalists, and so does healthcare. And it is a shame that this vision, the only one capable of putting everything in its place, of planning a sensible strategy, of bringing an army to a safe port, is still not valued and protected. Not because it knows more, but because it dares to look at problems in an open way, assuming the discomfort of greater uncertainty. Something that, generally speaking, few want to do.

我正好是一名普通医生。我看到儿童、年轻人、成年人和老年人。我处理各种情况、生活危机和疾病。出生和死亡。喜悦和惊恐。而当我环顾四周,无论是在国内还是国外,我看到许多同事都喜欢把泛泛而谈挂在嘴边,而专注于其他更精确的问题,在这个时代我完全理解。我不怪他们,因为我不能说他们最终没有放弃,因为水流太大,因为根本问题是社会性的,人们越来越紧张和迷失,最后在普通医生的办公室里寻求帮助。过去由牧师负责的事情最终在咨询室里进行,过去由朋友或家人听到的秘密,以及过去在小组或社区中解决的烦恼。而且没有人能够维持这一点。 这就是为什么我预见到在不久的将来,通才会越来越少,很多人没有机会接触到通才,也许会有更多人迷失。

普通人也不善于应对不确定性。一次感冒,在几个小时内就开始驱使他们寻求医疗帮助,一个痘痘,一个疙瘩,一个马蜂蜇伤。我记得小时候,我或我的兄弟姐妹有多少次被蜇伤。这真的很疼,但我们从未因此被带去看医生。每年我都会看到数以百计的人,他们不知道该如何处理他们的刺伤。我想象我在急诊科的同事们知道我在说什么。没有任何卫生系统能够遏制这种情况。因此,他们将崩溃,他们将别无选择,只能创建墙,而不是使用教育将使用共同支付。

社会和组织需要通才,医疗保健也是如此。而令人遗憾的是,这种眼光,这种唯一能够将一切归位、规划合理的战略、使军队圆满完成任务的眼光,仍然没有得到重视和保护。不是因为它知道得更多,而是因为它敢于以开放的方式看待问题,承担更大的不确定性带来的不适感。一般来说,很少有人愿意做这样的事情。

Nadar y avanzar en la incertidumbre es francamente muy difícil e inquietante.

Uno de los factores importantes del porqué los nuevos licenciados de medicina no suelen escoger la especialidad de MFyC. Hay otros, anodina presencia de la especialidad en la carrera, historico maltrato administrativo a la especialidad en los ultimos 20 años, ..... pero éste más sutil que tan bien describe en el post es sin duda significativo. Muy buena reflexión.
Tengo infinitas dudas de como la sociedad y los profesionales vamos a medio salvar la especialidad más necesaria de todas.
