La importancia de las pequeñas cosas. The importance of the little things. 小事的重要性



Esta semana he empezado a trabajar en el centro de salud de Soto del Real por jubilación de un compañero del equipo. Dejo mi consulta de Miraflores que he ocupado casi dos años agradecido por los estupendos profesionales del consultorio y por la amabilidad y acogida de los pacientes, muchos de ellos muy mayores. 

Lo primero que he hecho en mi nueva consulta es colocar mis libros, plantas y cuadros. Lo siguiente echar la mesa a un lado para poder atender a quien acuda sin barreras físicas. Al compartir el espacio con otro compañero de turno de tarde me tocará mover el mueble todos los días pero lo doy por bueno si eso me hace sentir más cómodo y facilita en algo la comunicación. 

En estos tiempos de tecnología e innovación sigo pensando en la importancia de las pequeñas cosas. Un gesto no cambiará nunca el sistema sanitario pero tal vez pueda producir efectos positivos e inesperados. 

Me seguiré descalzando al entrar para recordarme que mi labor ha de ir pegada a tierra y trataré de seguir mirando a la gente a los ojos para no olvidar que compartimos una misma mirada y una misma vida. 

Espero poder estar a la altura. 




The importance of the little things


This week I started working at Soto del Real primary health centre due to the retirement of a teammate.  I am leaving my Miraflores practice which I have occupied for almost two years, grateful for the great professionals in the office and for the kindness and welcome of the patients, many of them very old. 


The first thing I have done in my new practice is to place my books, plants and pictures. The next thing I do is set the table aside so that I can attend to anyone who comes in without any physical barriers. I share the space with another colleague from the afternoon shift, so I will have to move the table every day, but I will accept it with joy if it makes me feel more comfortable and facilitates communication. 


In these times of technology and innovation I still think about the importance of the little things. A gesture will never change the health system but it can perhaps produce positive and unexpected effects. 


I will continue to take off my shoes when I start working to remind myself that my suty must be grounded and I will try to continue to look at their faces so I don't forget that we share the same life.

I hope to be up to the task.

Falsos positivos, el talón de Aquiles sanitario. False positives, the sanitary Achilles' heel. 誤報,衛生致命傷。

Explicar matemáticas no se nos suele dar bien a los profesionales sanitarios pero en esta época de noticias falsas y medias verdades cada vez es más necesario. 

Pongamos un ejemplo reciente. Imaginen que elevamos al máximo la tensión político militar de un país. Imaginen que en un momento dado alguien tiene la duda de que un objeto volante pueda ser un avión enemigo. Imaginen que aprieta un botón y un misil derriba un avión de pasajeros. Esto es un falso positivo, etiquetar como nocivo algo que no lo es. 

En medicina lo hacemos con frecuencia dado que toda técnica diagnóstica, subrayo toda, produce falsos positivos. Desde las más sencillas a las más sofisticadas, con mayor o menor probabilidad, es un hecho objetivo que aumenta a medida que lo hace el número de pruebas que se llevan a cabo.

Como el mantra “más es mejor” contamina los servicios sanitarios, la ciudadanía reclama todas las pruebas posibles “porque yo lo valgo” sin saber el peligro que corren por someterse a ellas. Si una prueba se interpreta como positiva implicará otras pruebas o medidas. De este modo si el análisis de heces detectó sangre oculta se ordenará una colonoscopia y si la mamografía identifica una imagen extraña una punción biopsia y en ambos casos resección de lo que se considere sospechoso. 

Existe un gran debate con los programas de cribado que se aplican a población sana por ser de alguna manera una forma de elevar la tensión “político militar” sanitaria. Mientras más enfermedad se espere, más se encontrará (verdaderos positivos) y de paso generamos un buen puñado de efectos colaterales (faltos positivos).

A nadie le gustaría que a un familiar o a uno mismo le derribará un misil por error pero no es tan evidente que una prueba médica pueda potencialmente ser igual de peligrosa. Y si miramos la cifras de personas que mueren por reacciones a contrastes, perforaciones, sangrados, complicaciones y demás veremos que superan al año las de varios aviones. Si incluimos las personas a las que se les opera, amputa o somete a procesos innecesarios por falsos positivos la lista se alargaría muchísimo más. 


Por estas razones me parecería de mucha ayuda incluir en las hojas de consentimiento informado habituales una pequeña infografía donde se ilustre este tema para que quien tenga que someterse a una prueba diagnóstica sepa a qué probabilidades se somete tanto de posibles complicaciones como de falsos positivos. El consejo y el criterio del profesional sanitario seguirá siendo imprescindible pero quizá los números ayuden por sí mismos a más de uno.






False positives, the sanitary Achilles' heel.

Explaining mathematics is not usually easy to health professionals but in this age of fake news and half-truths it is increasingly necessary. 

Let's take a recent example. Imagine that we raise the political-military tension in a country to the maximum. Imagine that at some point someone has doubts that a flying object could be an enemy plane. Imagine that you push a button and a missile knocks down a passenger plane. This is a false positive: to label as harmful something that is not. 

In medicine we often do this because every diagnostic technique, I emphasize, produces false positives. From the simplest to the most sophisticated, with greater or lesser probability, it is an objective fact that increases as the number of tests carried out increases.

As the mantra "more is better" contaminates the health services, citizens demand all possible tests "because I am worth it" without knowing the danger they run by submitting to them. If a test is interpreted as positive it will imply other tests or measures. Thus, if the stool analysis detected occult blood, a colonoscopy will be ordered and if the mammography identifies a strange image, a puncture biopsy and in both cases a resection of what is considered suspicious. 

There is a great deal of debate with screening programs that are applied to healthy populations because they are somehow a way of raising the "political-military" health tension. The more disease you expect, the more you will find (true positives) and in the process we generate a good handful of side effects (false positives).

No one would like to see a family member shot down by mistake but it is not so obvious that a medical test can potentially be just as dangerous. And if we look at the numbers of people who die from reactions to contrast, punctures, bleedings, complications and so on, we see that they exceed the numbers of several crashed planes in a year. If we include the people who are operated on, amputated or subjected to unnecessary procedures for false positives the list would be much longer. 

For these reasons I think it would be very helpful to include in the usual informed consent forms a small computer graphic illustrating this issue so that anyone who has to undergo a diagnostic test knows what the chances are of both possible complications and false positives. The advice and judgement of the health professional will still be essential but perhaps the numbers themselves will help more than one.

Sobre la profilaxis del VIH con pastillas (pPrep)




  • El tratamiento preventivo del VIH con pastillas (pPrep) está disponible desde hace años. El Ministerio de Sanidad aprobó la financiación pública del medicamento hace un mes pero aún no está disponible en la mayoría de las Comunidades. En Cataluña se puede conseguir en muchos hospitales y en Madrid en el centro Sandoval (c/Sandoval 7) a partir de finales de diciembre. 
  • La pastilla se toma una vez al día y tras una semana de uso protege contra la infección al 99% si se toma diaria y con menor porcentaje si no es así. 
  • En España no está disponible en farmacias y quien la necesite debía comprarla por Internet (30-50€ al mes).
  • Es un tratamiento a considerar para personas que estén en riesgo de contactar con el virus y que no quieran usar preservativos, que es la opción en este momento más segura. 
  • Como médico me llama poderosamente la atención:


1. Que aún no esté disponible en farmacias (lo que obliga a comprarlo online con riesgo).
2. Que la mayoría de los profesionales sanitario no tengamos información al respecto.

¿Qué es la Profilaxis Pre-exposición (PrEP)?

  • Pre = antes (previa)
  • Exposición = tener contacto con el VIH
  • Profilaxis = tratamiento para prevenir que ocurra una infección
La profilaxis Pre-exposición (PrEP, siglas en inglés) es una estrategia de prevención de la infección con el VIH donde los individuos VIH negativos toman medicamentos anti-VIH antes de entrar en contacto con el VIH para reducir su riesgo de infectarse. Los medicamentos previenen que el VIH establezca una infección dentro del cuerpo.
PrEP ha demostrado reducir el riesgo de infección con el VIH a través del contacto sexual en los  hombres gay y bisexuales, mujeres transgénero y en hombres y mujeres heterosexuales, así como en personas que se inyectan drogas.
No protege contra otras infecciones de transmisión sexual (ITS) ni tampoco previene el embarazo. No es una cura para el VIH.

¿Para quién funciona la PrEP?

La PrEP ha demostrado reducir el riesgo de infección con el VIH en muchos estudios. El estudio iPrEx demostró que la PrEP reduce el riesgo de infección con el VIH entre los hombres gay y bisexuales y en las mujeres transgénero. Dos grandes estudios, Partners PrEP y TDF2, demostraron que PrEP también reduce el riesgo de infección con el VIH entre los hombres y mujeres heterosexuales. El estudio Bangkok Tenofovir demostró que la PrEP funciona para las personas que se inyectan drogas.




Efectividad

El uso de Truvada como PrEP provee una reducción del riesgo de infectarse del 96%  al 99% en las personas VIH negativas que toman las píldoras todos los días de acuerdo con  las indicaciones. Si se saltea una dosis diaria, el nivel de protección contra el VIH puede disminuir. Sólo funciona si toma el medicamento. Las personas que usan la PrEP correcta y consistentemente tienen niveles más altos de protección contra el VIH.
De acuerdo con el análisis de los datos del estudio iPrEx se descubrió que la PrEP es efectiva para:
  • Las personas que toman 7 píldoras de PrEP por semana, su nivel estimado de protección es del 99%
  • Las personas que toman 4 píldoras de PrEP por semana, su nivel estimado de protección es del 96%
  • Las personas que toman 2 píldoras de PrEP por semana, su nivel estimado de protección es del 76%
No existe suficiente data disponible para proveer una guía temporal específica que no sea de uso diario, por lo que la FDA recomienda que la PrEP se use diariamente para alcanzar el nivel más alto de protección.
Un par de cosas que deben tenerse en cuenta:
  • Al comenzar la PrEP, lleva al menos siete días para alcanzar altos niveles de protección contra el VIH.
  • Al suspender la PrEP, las personas deben continuar utilizando la PrEP durante cuatro semanas después de una exposición significativa.
  • La PrEP no protege contra otras infecciones de transmisión sexual (ITS) ni previene el embarazo. No es una cura para el VIH.




Efectos secundarios

Truvada como PrEP es generalmente seguro y bien tolerado. La mayoría de las personas que usan la PrEP reportan no experimentar efectos secundarios, pero se reportaron algunos efectos secundarios en los estudios clínicos. Los participantes del estudio iPrEx reportaron efectos secundarios que se encuadran en cuatro categorías principales (aquí ordenadas de más a menos comunes):
  1. Náusea: El 9% de quienes recibieron Truvada reportaron náuseas en el primer mes, en comparación con el 5% de los que recibieron placebo. Después del primer mes, no hubo diferencia en las tasas reportadas de náuseas entre los que recibieron Truvada y quienes recibieron placebo.
  2. Dolores de cabeza: El  4.5% de las personas que recibieron Truvada reportaron dolores de cabeza, en comparación con el 3.3% de quienes recibieron placebo.
  3. Pérdida de peso: El 2.2% de las personas que recibieron Truvada reportaron una pérdida de peso no intencional del 5%, en comparación con el 1.1% de los que recibieron placebo.
  4. Pequeños aumentos de la creatinina sérica: se sabe que Truvada causa pequeños aumentos en la creatinina sérica, una molécula de ocurrencia natural que es filtrada en los riñones. En este estudio, el 0.3% de quienes recibieron Truvada experimentaron aumentos moderados en la creatinina sérica que persistieron hasta el siguiente análisis. Los niveles de creatinina volvieron a bajar una vez que estos participantes abandonaron la PrEP. Cuatro de los cinco participantes retomaron la PrEP sin que volviera a aumentarles el nivel de creatinina. Los investigadores monitorearon la función renal durante la duración del estudio y no encontraron problemas serios.
Para la mayoría de las personas, estos efectos secundarios desaparecieron por si solos después de las primeras semanas de tomar Truvada, o cuando se suspendieron los medicamentos.

¿Quién es mi médico? Who's my doctor? 誰是我的醫生?

¿Quién es mi médico?


En esta época de hiperinformación a todos nos gusta saber quién tenemos delante en cada momento. Antes de consumir un bien o un servicio buscamos referencias, opiniones, valoraciones. Cuando estamos enfermos y necesitamos asistencia sanitaria también. Como médico conozco de primera mano que mis pacientes quieren saber quién soy. Buscan en Internet y encuentran cosas, fundamentalmente mi blog y cuenta de Twitter son mis principales tarjetas de visita, también mis vídeos y mi libro Diario de un médico descalzo

He de reconocer que es en el libro dónde más me mojo. Tuve la oportunidad de presentarlo en el pueblo donde actualmente desarrollo mi labor y es raro que semanalmente no reciba algún comentario del mismo. No es verdad que la gente no lea, todos leemos lo que verdaderamente nos interesa. Por eso le doy tanta importancia a este ejercicio de humildad que implica descalzarse ante las personas que uno sirve y mostrar un poco de aquella parte íntima de uno mismo: lo que uno piensa, siente, cree y espera. 

No es lo normal. Lo que se suele mostrar en público de uno mismo suele ser una pose, una careta, que construimos con selfies, imágenes y un relato sobre dimensionado de lo que entendemos es nuestra mejor versión. Pero la gente suele diferenciar bien lo que es postureo de lo que no lo es. Por eso al escribir o construir un contenido que hacer público trato de aplicar la sencilla regla de tratar de ser quien soy, cosa que no siempre es fácil.

En la facultad de medicina no se nos enseña a comunicarnos fuera de la consulta. No se nos dan pautas para usar redes sociales ni tampoco para escribir libros sobre nuestra visión de la vida. Se nos orienta más bien a construir un curriculum formativo y profesional basado en méritos académicos, investigadores o laborales. A los pacientes no es lo que más les interesa de nosotros. Quieren saber si somos competentes pero también si somos humanos. Si somos buenos clínicos pero también si escuchamos correctamente, miramos a los ojos o tratamos con respeto. Lo van averiguando según nos van conociendo pero algunos necesitan saber más para decidir si depositar su confianza o su problema en nuestras manos. Por eso es un excelente complemento a la entrevista personal en consulta disponer de otros medios que ayuden a mostrar quienes somos. 

El clínico siempre debe hacerse las preguntas: quién tengo delante, cuál es su verdadero problema, a la par que el paciente se pregunta: quién es mi médico. Mientras más fácil se lo pongamos tal vez nos resulte más sencillo encontrar las respuestas que buscamos. Al fin y al cabo la medicina se basa en comunicación y para que sea de calidad esta debe apoyarse en confianza y esta a su vez en conocimiento mutuo.








Who's my doctor?


In this age of hyperinformation we all like to know who is in front of us at all times. Before consuming a good or a service we look for references, opinions, evaluations. When we are sick and we need health care also. As a doctor I know first hand that my patients want to know who I am. They search the Internet and find things, mainly my blog and Twitter account are my main business cards, also my videos and my book Diary of a Barefoot Doctor

I have to admit that it is in the book where I show more who am I. I had the opportunity to present it in the Villaba where I currently work and it is rare that I don't receive any comment about it every week. It is not true that people do not read, we all read what really interests us. That's why I attach so much importance to this exercise in humility that involves taking off your shoes in front of the people you serve showing a little of that intimate part of yourself: what you think, feel, believe and expect. 

It is not normal. What is usually shown in public of oneself is usually a pose, a mask, that we build with selfies, images and a story about the dimension of what we understand is our best version. But people usually differentiate well what is a más and what is not. That's why when I write or build content that I make public I try to apply the simple rule of trying to be who I am, which is not always easy.

In medical school we are not taught to communicate outside the practice. We are not given guidelines for using social networks or for writing books about our vision of life. Rather, we are guided to construct a formative and professional curriculum based on academic, research or labor merits. Patients are not the most interested in that. They want to know if we are competent but also if we are human. If we are good clinicians but also if we listen correctly, look into their eyes or treat with respect. They find out as they get to know us, but some need to know more to decide whether or not to put their trust or their problem in our hands. That is why it is an excellent complement to the personal interview in consultation to have other means that help to show who we are. 

The clinician should always ask himself the questions: who is in front of me, what is his real problem, at the same time as the patient asks himself: who is my doctor. The easier we make it, the easier it may be for us to find the answers we are looking for. After all, medicine is based on communication and for it to be of quality it must be based on trust and this in turn on mutual knowledge.

Cuando nos sentimos mal hacemos lo posible por zafarnos del problema. Da igual que sea por una desavenencia de pareja, con el jefe, con padres o hijos, con alguna amistad, con un cliente, por una pérdida, por una enfermedad, por un futuro incierto...  Las reacciones básicas suelen ser dos:

1. Huida.
2. Ataque.


La más común es la primera para la que usamos toda nuestra capacidad de distracción, despiste, negación... Hacemos grandes esfuerzos para quitar de encima de la mesa el problema en cuestión y meterlo en un cajón o en algún armario fuera de la vista para que no moleste. Los marrones pequeños caben, los grandes no. Como el mercado nos conoce bien ofrece una infinita lista de posibilidades para mantenernos entretenidos: series, películas, programas de televisión, youtube, redes sociales, páginas de Internet, música... Además de todos los productos y servicios que son susceptibles de mercantilizarse. El universo misteriosamente confabula para que compremos algo cuando estamos mal.

Las adicciones también tienen mucho que decir aquí. Cuando uno está chungo es fácil caer por la pendiente resbaladiza del alcohol, las drogas (incluidos medicamentos), el juego, las conductas de riesgo, la sexualidad compulsiva... Hecho que también reporta pingues beneficios a corporaciones y personas sin muchos escrúpulos.

Cada vez es más frecuente acudir al médico en estos casos que, como estará sobrecargado de trabajo hasta las trancas, nos despachará con un diagnóstico de ansiedad/depresión, una cajita de pastillas y quizá una derivación a los servicios de salud mental donde, si terminamos yendo, nos darán más pastillas.

Es verdad que cualquier chungo termina disolviéndose con el tiempo. Pero ¿quién es el majo que sabe esperar en estos tiempos de prisa permanente? Nos tomamos las pastillas y lo que nos den. Lo malo es que no hay medicamento que consiga frenar la cabeza, y la nuestra sigue en modo lavadora centrifugando a toda pastilla el problema de marras. Agotador.

Terminamos yendo al psicólogo o al terapeuta alternativo. El primero nos dará una lista de consejos, el segundo nos hará alguna terapia y nos venderá una nueva caja de pastillas (naturales) o algo similar. Tampoco con eso se nos va el problema.

La mala noticia: los problemas no se van. Pasa como con la sombra, uno siempre la tiene, a veces se ve más otras menos pero suele quedarse pegada a uno de forma vitalicia tirada por el suelo como aquella serpiente que hace bastante tiempo expulsaron de un remoto jardín.

A mis pacientes les suelo recordar que hay otra estrategia vital más sofisticada que la huida y el ataque: la toma de consciencia. Uno siempre tiene la posibilidad de contemplar cualquier contenido de la consciencia, por muy feo o terrible que parezca, sin sufrir daño alguno. Podemos mirar nuestro problema de frente aunque sea precisamente lo que evitamos con todas nuestras fuerzas. La buena noticia: atrevernos a mirar un problema no lo vaporiza pero nos permite ponerlo en su sitio y asignarle el peso que le corresponde. 

Cuando el Buda tras una vida intensa de búsqueda y huida se sienta al fin bajo el árbol del Bhodi y se atreve a contemplar todos sus demonios y tentaciones sin reaccionar a ellas ni escapar termina iluminándose que es como decir: entendiendo que no hay absolutamente nada que no sea susceptible de ser sostenido con la atención. Lo mismo le pasó al Cristo cuando se metió en su desierto 40 días y se enfrento a los mismos demonios y tentaciones. Salió de allí con una nueva comprensión. Pongan ustedes el profeta, místico o héroe preferido y comprobaran que la historia se repite.

En ambos casos los problemas y sombras no desaparecieron. Pero dejaron de fastidiar. Ya no era necesario seguir huyendo o atacando(se) y por ende a los demás.

Hasta que consigamos iluminarnos dejo diez pistas que creo pueden venir bien a una gran mayoría:

  6. Mindfulness
  7. Añadir más "minutos buenos" al día
  8. Relajación
  9. Apoyo familiar/amigos

1, EJERCICIO 

La palabra clave es movimiento. Basta con caminar media hora al día al aire libre, pruébenlo y verán que tiene efectos casi mágicos. Si consiguen perseverar tal vez terminen apuntándose a ese selecto club de paseantes que son capaces de observar la vida y sus visicitudes desde la perspectiva del que se atreve a levantarse de la silla. 

2. NATURALEZA

Un animal salvaje es feliz si está libre y desdichado si le meten en una jaula. ¿Cuántas horas pasas al día en la tuya? Seguramente muchas. Aunque tu jaula sea de oro, cómoda, agradable, climatizada... no deja de ser una jaula. Libérate un rato y si tienes un parque, montaña o campo cerca prueba a respirar un poco de aire fresco y darte una vuelta. 

3. MASCOTAS. 

Acariciar un animal es una fuente de buen rollo. Eso lo saben bien los que los tienen. Si no es el caso pide a algún amigo que te dejen acariciar el suyo, o a alguna persona que saque de paseo a su perro por la calle. Seguro que te dejan y verás que bien te sientes. 

4. AYUDAR A OTROS

Cuando uno está chungo es inevitable quedarse mirando el propio ombligo. Y claro, nos ponemos peor. Ayudar a otros nos permite descentrarnos un poquito de nuestro problema circular y de paso ponerlo en perspectiva. Los demás habitualmente están mucho peor. Si no tienen nadie a mano basta con darse una vuelta por el barrio y hablar un rato con alguna persona sin techo o que pida a la salida de un super. O darse una vuelta por una residencia de ancianos. 

5. AFICIONES

Las aficiones son aquellas cosas que nos mola hacer. Tienen la facultad de sacar nuestro lado bueno y hacernos pasarlo bien y divertirnos. También nos permiten canalizar la creatividad y esta es una de las medicinas más potentes que existen para tener buen rollo. 

6. ATENCIÓN PLENA

El mal rollo nos lleva al pasado (culpa) y al futuro (incertidumbre y miedo) robándonos el presente. Pero solo se puede encontrar serenidad en el aquí y el ahora que es lo único que podemos manipular. Tomar plena consciencia de lo que hacemos en cada momento es una manera de poner los pies en el suelo. Sentir el cuerpo, la respiración en este instante. Notar como la cabeza se va llenando y vaciando de sentimientos, ideas y contenidos. Saborear lo que meto en la boca o lo que mi piel nota en este instante. Es básico tener paciencia con nosotros mismos y volver al presente cada vez que me doy cuenta de que he vuelto al pensamiento rumiante. 
Técnicas como el yoga, el mindfulness, la meditación y otras muchas pueden tal vez ayudar. Echa un vistazo a Internet, o mejor aún, prueba algo ahora mismo.

7. AÑADIR MÁS MINUTOS BUENOS AL DÍA. 

Por muy mala que esté la cosa siempre hay algún instante bueno en cada día. Sentir el placer del agua de la ducha, notar la ropa limpia al vestirnos, disfrutar del sabor de la comida... Pero no es nada sencillo encontrarlos cuando nuestra cabeza tiene una marcada tendencia a meterse en el estiércol. Quizá la poesía y la música puedan servirte de ayuda.

8. RELAJACIÓN

Los problemas suelen asociar tensión muscular a sumar a la cognitiva y emocional. Las contracturas duelen. Un poco de fisioterapia casera aplicada sabiamente a la maltrecha anatomía con calorcito y masaje suave puede venir bien. Échale morro y pídeselo a alguien, si no es posible usa tus manos. En paralelo podemos añadir alguna técnica sencilla de relajación o respiración consciente que podemos copiar de algún vídeo o audio sacado de Internet.

9. APOYO FAMILIAR Y DE AMIGOS

Pedir ayuda a los que nos quieren o simplemente desahogarnos un rato hablando, dando una vuelta o pasando tiempo juntos no tiene precio. No es fácil tener el valor para enseñar nuestras heridas y preocupaciones. Tampoco saber medir hasta dónde mostrar para no sobrecargar a los nuestros con un chorreo inacabable de queja permanente que al final resulta tóxico para todos y que los hará huir.

10. FILOSOFÍA

Tomarse las cosas con filosofía significa no darse tanta importancia. Cuando vagamos perdidos en el bosque umbrío no hay nada mejor que buscar una altura para tratar de ver las cosas con perspectiva, buscar alguna referencia en el paisaje y recuperar la orientación. Salir del "pobre de mi" y de las actitudes catastrofistas y autodestructivas para con humildad reconocer que la vida siempre tiene algún sentido aunque en ocasiones nos resulte complicado encontrarlo. A otros quizá les sirva un código moral o una creencia religiosa, todo puede valer para encontrar sentido a la difícil pregunta de por qué a la gente buena les pasan cosas malas. 



Los decálogos y las listas son una manera como otra cualquiera de perder el tiempo. Sirven para muy poco. En cualquier caso aquí les dejo esta que está redactada con buena intención y bastante ingenuidad. Me paso el día rodeado de gente que está fatal, tengo la consulta atestada y no me da la vida para contarlo todo en los seis minutos que tengo para atender cada paciente. Espero por lo tanto que esto pueda servir de ayuda. Si es así compártelo, que seguro tienes también un montón de gente chunga alrededor.









Decalogue for hard times


When we feel bad we do our best to get out of the problem. It doesn't matter if it is due to a disagreement between the couple, with the boss, with parents or children, with some friendship, with a client, for a loss, for an illness, for an uncertain future...  There are usually two basic reactions:


1. Flight.

2. Fight.



The most common is the first for which we use all our capacity for distraction, absent-mindedness, denial... We make great efforts to remove the problem from the table and put it in a drawer or some closet out of sight so that it does not disturb. Small problems fit, big ones don't. As the market knows us well, it offers an infinite list of possibilities to keep us entertained: series, movies, television programs, youtube, social networks, Internet pages, music... In addition to all the products and services that are likely to be marketed. The universe mysteriously conspires to sell us something when we are wrong.


Addictions also have a lot to say here. When you're cranky it's easy to fall down the slippery slope of alcohol, drugs (including medications), gambling, risky behaviors, compulsive sexuality... It also brings profits to corporations and unscrupulous people.



It is more and more frequent to go to the doctor in these cases that will dispatch us with a diagnosis of anxiety/depression, a box of pills and perhaps a referral to mental health services where, if we end up going, they will give us more pills.



It's true that any issue ends up dissolving over time. But who is the nice one who knows how to wait in these times of permanent haste? We take the pills and whatever they give us. The bad thing is that there is no medicine that can slow down the mind, and ours is still in washing machine mode centrifuging the whole trouble. Exhausting.



We end up going to the psychologist or the alternative therapist. The first will give us a list of advice, the second will do some therapy and sell us a new box of pills (eco) or something similar. We won't rid off the problem either.



The bad news: problems don't go away. It happens as with the shadow, you always have it, sometimes you see more others less but it usually stays stuck to one for life thrown on the ground like that snake that a long time ago expelled from a remote garden.



I often remind my patients that there is another more sophisticated life strategy than flight or fight: awareness. One always has the possibility of contemplating any content of consciousness, no matter how ugly or terrible it may seem, without suffering any harm. We can look our problem head-on even if it is precisely what we avoid with all our might. The good news: daring to look at a problem does not vaporize it, but it allows us to put it in its place and give it the weight it deserves.



When the Buddha, after an intense life of search and flight, finally sits under the Bhodi tree and dares to contemplate all his demons and temptations without reacting to them or escaping, he ends up illuminating himself as if to say: understanding that there is absolutely nothing that cannot be sustained with attention. The same thing happened to Christ when he went into his desert 40 days and faced the same demons and temptations. He left there with a new understanding. Put the prophet, mystic or favourite hero and you will see that history repeats itself.



In both cases the problems and shadows did not disappear. But they stopped teasing. It was no longer necessary to keep on running or attacking oneself and therefore the others.



Until we get enlightened I leave ten clues that I think can come in handy to a large majority:

1. EXERCISE



The key word is movement. Just walk half an hour a day in the open air, try it and you will see that it has almost magical effects. If you manage to persevere you may end up joining the select club of walkers who are capable of observing life and its ups and downs from the perspective of the one who dares to get up from the chair.



2. NATURE



A wild animal is happy if it is free and unhappy if it is put in a cage. How many hours a day do you spend in yours? Probably many. Even if your cage is made of gold, comfortable, pleasant, heated... it is still a cage. Get free for a while and if you have a park, mountain or field nearby try to breathe some fresh air and take a walk.



3. PETS.

Caressing an animal is a source of good vibrations. That is well known to those who have them. If this is not the case, ask a friend to let you pet his, or someone to take his dog for a walk in the street. I'm sure they'll let you and you'll see how good you feel.



4. HELPING OTHERS



When one is feeling bad it is inevitable to stare at one's own navel. And of course, we get worse. Helping others allows us a little bit of our circular problem and to put it into perspective. The others are usually much worse off. If you don't have anyone at hand, you can go for a walk around the neighbourhood and talk for a while with some homeless person or someone who asks at the exit of a supermarket. Or take a walk around an old people's home.



5. HOBBIES



Hobbies are those things we like to do. They have the ability to bring out our good side and make us have a good time and have fun. They also allow us to channel creativity and this is one of the most potent medicines there is to have good vibes.



6. FULL ATTENTION



Evil leads us to the past (guilt) and to the future (uncertainty and fear) by stealing the present. But serenity can only be found in the here and now which is the only thing we can manipulate. Becoming fully aware of what we are doing in each moment is a way of putting our feet on the ground. To feel the body, the breath in this instant. Notice how the head fills up and empties of feelings, ideas and contents. Taste what I put in my mouth or what my skin notices at this moment. It is basic to have patience with ourselves and to return to the present every time I realize that I have returned to ruminant thought.

Techniques such as yoga, mindfulness, meditation and many others can perhaps help. Take a look at the Internet, or better yet, try something right now.



7. ADD MORE GOOD MINUTES TO THE DAY.



No matter how bad it is, there is always some good moment in every day. To feel the pleasure of the water of the shower, to notice the clean clothes when we dress, to enjoy the flavor of the food... But it is not easy to find them when our head has a marked tendency to get into the manure. May be some poetry or music can help.


8. RELAXATION


The problems usually associate muscular tension to add to the cognitive and emotional. Contractures hurt. A little homemade physiotherapy wisely applied to the battered anatomy with warmth and gentle massage may come in handy. Nose and ask someone, if it is not possible use your hands. In parallel we can add some simple technique of relaxation or conscious breathing that we can copy from some video or audio taken from the Internet.


9. SUPPORT FROM FAMILY AND FRIENDS


Asking for help from those who love us or simply letting off steam for a while by talking, going for a walk or spending time together is priceless. It is not easy to have the courage to show our wounds and worries. Nor do we know how to measure how far to show so as not to overburden our own with an endless stream of permanent complaint that in the end is toxic to everyone and will make them flee.


10. PHILOSOPHY


Taking things philosophically means not giving oneself so much importance. When we wander lost in the shady forest there is nothing better than looking for a height to try to see things with perspective, to look for some reference in the landscape and to recover the orientation. Coming out of the "poor me" and the catastrophic and self-destructive attitudes in order to humbly recognize that life always has some meaning, even though sometimes it is difficult for us to find it. Others may be served by a moral code or a religious belief, all of which may make sense of the difficult question of why bad things happen to good people.



Decalogues and lists are just another way of wasting time. They are of little use. In any case here I leave you this one that is written with good intention and quite naïve. I spend the day surrounded by people who feels terrible, my practice is overcrowded and I can´t tell everything I want in the six minutes I have to attend each patient. I therefore hope that this can help. If that's the case, I'm sure you also have a lot of needy people around you to share this clues.










艱難時期的十誡





當我們心情不好時,我們會盡力擺脫這個問題。如果是因為這對夫妻,老闆,父母或孩子,與朋友,客戶,失去,疾病,不確定的未來之間存在分歧,這無關緊要......通常有兩個基本反應:

成癮在這裡也有很多話要說。當你發脾氣時,很容易陷入酒精,藥物(包括藥物),賭博,危險行為,強迫性行為的滑坡......這也給公司和不法分子帶來了利潤。







在這些病例中去看醫生會越來越頻繁地向我們發出焦慮/抑鬱症的診斷,一盒藥片以及可能轉介到心理健康服務的地方,如果我們最終去,他們會給我們更多的藥片。

在這兩種情況下,問題和陰影都沒有消失。但是他們停止了戲弄。不再需要繼續跑步或攻擊自己,而不是其他人。

哲學





1.運動



關鍵詞是運動。只需每天在露天走半小時,嘗試一下,你會發現它幾乎具有神奇的效果。如果你能夠堅持到底,你最終可能會加入精選的步行者俱樂部,他們能夠從敢於從椅子上站起來的人的角度觀察生活和起伏。



2.自然



如果野生動物被放入籠子中,它是自由而不快樂的,它會很開心。你每天花多少時間在你的身上?可能很多。即使你的籠子是由金製成的,舒適,舒適,加熱......它仍然是一個籠子。獲得一段時間的免費,如果你有一個公園,山脈或附近的場地嘗試呼吸新鮮空氣,散步。

8.放鬆

從哲學角度講事物意味著不要給予自己如此重要的意義。當我們在陰涼的森林中迷失時,沒有什麼比尋找一個高度試圖用透視看東西,尋找景觀中的一些參考並恢復方向更好的了。走出“可憐的我”和災難性和自我毀滅的態度,以便謙卑地認識到生活總是有一些意義,即使有時我們很難找到它。其他人可能會受到道德準則或宗教信仰的影響,所有這些都可以理解為什麼壞事發生在好人身上的難題。
