Quedarse en casa. Stay home. 呆在家裡。

No nos gusta la idea de quedarnos en casa a la fuerza. En una sociedad como la nuestra basada en el ruido y la prisa lo vivimos como castigo. Sin embargo hay situaciones que nos obligan a hacerlo, como una torcedura fortuita de tobillo o una pandemia global. 
Nos cuesta quedarnos quietos, dejar de hacer, dejar de consumir. Pero si nos atrevemos a hacerlo y a nos sumergimos en las calmadas aguas del aburrimiento tal vez encontremos cosas inesperadas.

En estos días de aumento de casos de infección por Coronavirus lo más adecuado es quedarse en casa. Minimizar nuestras interacciones sociales y permanecer dentro de lo límites de la república independiente de nuestro domicilio. 

Habrá tiempo de sobra para ver la tele (dosifiquen por favor), tal vez alguno se anime a leer y quizá surja alguna reflexión, toma de conciencia, alguna creatividad o idea.

Viene bien recordar que el silencio es uno de los grandes lujos modernos y que sin salir de casa se puede recorrer el mundo si uno se arma con las lecturas correctas. 

Mucho ánimo.






Stay home.

We don't like the idea of staying home by force. In a society like ours based on noise and haste, we live it as punishment. However, there are situations that force us to do so, such as a fortuitous sprained ankle or a global pandemic. 

It is difficult for us to stay still, to stop doing, to stop consuming. But if we dare to do it and dive into the calm waters of boredom we may find unexpected things.

In these days of increasing cases of Coronavirus infection, it is best to stay home. Minimize our social interactions and stay within the boundaries of the independent republic of our domicile. 

There will be plenty of time to watch TV, perhaps some will be encouraged to read and perhaps some reflection or awareness will arise, some creativity or idea.

It is good to remember that silence is one of the great modern luxuries and that without leaving your home you can travel the world if you arm yourself with the right readings. 

Have a good time.

我們不喜歡用武力待在家裡的想法。 在像我們這樣的社會中,我們以喧囂和喧囂為生。 但是,在某些情況下,我們不得不這樣做,例如偶然的腳踝扭傷或全球性大流行。

Sueños sucios Dirty dreams. 骯髒的夢。





Cada vez tenemos más clara la importancia de la higiene corporal pero seguimos sin cuidar otras facetas de nuestra vida más sutiles e invisibles, una de ellas el sueño. 

Lo habitual es que nos vallamos a la cama con los dientes limpios y los sueños sucios. Me explico, el ser humano lleva miles de años ajustando su ritmo sueño-vigilia según la luz solar. Desde la invención de la luz eléctrica hemos modificado dicho ajuste, más si cabe desde que disponemos de pantallas que además de ser una potente fuente de luz lo es de provisión de información. 

Lo más habitual es irnos a dormir tras haber consumido un tiempo de visualización de pantallas. El mercado lo sabe y sitúa en esas horas los contenidos audiovisuales más apetecibles que suelen incluir dosis de acción, entretenimiento y estímulos intensos. Nos acostamos pues sin haber dado tiempo a digerir esas potentes imágenes que teñirán inevitablemente el contenido onírico de la noche, dado que los sueños se valen de lo experimentado en la anterior jornada. 

Al ser lo soñado una realidad difícil de estudiar no tenemos mucha evidencia científica sobre cómo influye el contenido audiovisual en el sueño pero por lo que sabemos sí parece que hay relación tanto con el contenido onírico como con la calidad del sueño. Si se aumenta el nivel de alerta o ansiedad, el sueño perderá calidad, se hará más superficial y podrá volverse quebradizo con múltiples despertares nocturnos y menor sensación de haber descansado a la mañana siguiente. 

La recomendación es sencilla. La mejor higiene del sueño implica:

1. Cuidar los horarios y el hábito de ir a dormir a una misma hora. 
2. Evitar cenas copiosas inmediatamente antes de acostarse. 
3. Disminuir la intensidad de la luz eléctrica ambiental un tiempo antes de dormir.
4. Evitar el uso de pantallas la hora previa a acostarse. 


Es mucho más sano para el sueño acostumbrarse a leer, charlar tranquilamente o realizar alguna actividad no muy estimulante antes de irse a dormir. La costumbre de ver televisión en la cama puede terminar erosionando la calidad del sueño.

Dirty dreams.


The importance of body hygiene is becoming increasingly clear to us, but we still do not care about other more subtle and invisible facets of our lives, one of them being sleep.  

We usually go to bed with clean teeth and dirty dreams. I mean, human beings have been adjusting their sleep-wake rhythm according to sunlight for thousands of years. Since the invention of electric light, we have modified this adjustment, even more so since we have screens that, in addition to being a powerful source of light, also provide information. 

The most common thing is to go to sleep after having spent some time viewing screens. The market knows this and places the most appealing audiovisual content in those hours, which usually includes doses of action, entertainment and intense stimuli. So we go to bed without having given ourselves time to digest those powerful images that will inevitably colour the dreamlike content of the night, given that dreams are based on what we experienced the previous day.

1. Take care of the schedules and the habit of going to sleep at the same time.

Estaría muy bien que como sociedad aprendiéramos algo de las pandemias.

1. Los memes y los virus se transmiten igual. De persona a persona y son muy contagiosos. 
2. Hay memes y virus peligrosos y otros no tanto.
3. Merece la pena defenderse de ambos.  

    De los memes: minimizado el ruido de fondo, buscando información de calidad en cantidad justa y evitando fuentes de mala información. 

    De los virus:


  • Lavándonos las manos.
  • Cubriéndonos la boca si tosemos, con la flexura del codo o un pañuelo desechable.
  • No exponiéndonos innecesariamente a aglomeraciones o ambientes muy fríos.

Los periodistas y los medios de comunicación tendrían que tener una directriz clara de información veraz cuando se aborden estos temas. 

Los profesionales sanitarios recordar que son agentes de educación para la salud y eso implica algún curso de acción que visibilice información de calidad. 

La ciudadanía en general podría aprovechar estas situaciones para reforzar hábitos como el lavado de manos y el cubrirse la boca correctamente si tose delante de otros. 

Más educación y menos mascarillas por favor.

Not exposing ourselves unnecessarily to crowds or very cold environments.

Health professionals should remember that they are agents of health education and this implies some course of action that makes quality information visible. 



Citizens in general could take advantage of these situations to reinforce habits such as hand washing and covering their mouths correctly if they cough in front of others.

La importancia de las pequeñas cosas. The importance of the little things. 小事的重要性



Esta semana he empezado a trabajar en el centro de salud de Soto del Real por jubilación de un compañero del equipo. Dejo mi consulta de Miraflores que he ocupado casi dos años agradecido por los estupendos profesionales del consultorio y por la amabilidad y acogida de los pacientes, muchos de ellos muy mayores. 

Lo primero que he hecho en mi nueva consulta es colocar mis libros, plantas y cuadros. Lo siguiente echar la mesa a un lado para poder atender a quien acuda sin barreras físicas. Al compartir el espacio con otro compañero de turno de tarde me tocará mover el mueble todos los días pero lo doy por bueno si eso me hace sentir más cómodo y facilita en algo la comunicación. 

En estos tiempos de tecnología e innovación sigo pensando en la importancia de las pequeñas cosas. Un gesto no cambiará nunca el sistema sanitario pero tal vez pueda producir efectos positivos e inesperados. 

Me seguiré descalzando al entrar para recordarme que mi labor ha de ir pegada a tierra y trataré de seguir mirando a la gente a los ojos para no olvidar que compartimos una misma mirada y una misma vida. 

Espero poder estar a la altura. 




The importance of the little things


This week I started working at Soto del Real primary health centre due to the retirement of a teammate.  I am leaving my Miraflores practice which I have occupied for almost two years, grateful for the great professionals in the office and for the kindness and welcome of the patients, many of them very old. 


The first thing I have done in my new practice is to place my books, plants and pictures. The next thing I do is set the table aside so that I can attend to anyone who comes in without any physical barriers. I share the space with another colleague from the afternoon shift, so I will have to move the table every day, but I will accept it with joy if it makes me feel more comfortable and facilitates communication. 


In these times of technology and innovation I still think about the importance of the little things. A gesture will never change the health system but it can perhaps produce positive and unexpected effects. 


I will continue to take off my shoes when I start working to remind myself that my suty must be grounded and I will try to continue to look at their faces so I don't forget that we share the same life.

I hope to be up to the task.

Falsos positivos, el talón de Aquiles sanitario. False positives, the sanitary Achilles' heel. 誤報,衛生致命傷。

Explicar matemáticas no se nos suele dar bien a los profesionales sanitarios pero en esta época de noticias falsas y medias verdades cada vez es más necesario. 

Pongamos un ejemplo reciente. Imaginen que elevamos al máximo la tensión político militar de un país. Imaginen que en un momento dado alguien tiene la duda de que un objeto volante pueda ser un avión enemigo. Imaginen que aprieta un botón y un misil derriba un avión de pasajeros. Esto es un falso positivo, etiquetar como nocivo algo que no lo es. 

En medicina lo hacemos con frecuencia dado que toda técnica diagnóstica, subrayo toda, produce falsos positivos. Desde las más sencillas a las más sofisticadas, con mayor o menor probabilidad, es un hecho objetivo que aumenta a medida que lo hace el número de pruebas que se llevan a cabo.

Como el mantra “más es mejor” contamina los servicios sanitarios, la ciudadanía reclama todas las pruebas posibles “porque yo lo valgo” sin saber el peligro que corren por someterse a ellas. Si una prueba se interpreta como positiva implicará otras pruebas o medidas. De este modo si el análisis de heces detectó sangre oculta se ordenará una colonoscopia y si la mamografía identifica una imagen extraña una punción biopsia y en ambos casos resección de lo que se considere sospechoso. 

Existe un gran debate con los programas de cribado que se aplican a población sana por ser de alguna manera una forma de elevar la tensión “político militar” sanitaria. Mientras más enfermedad se espere, más se encontrará (verdaderos positivos) y de paso generamos un buen puñado de efectos colaterales (faltos positivos).

A nadie le gustaría que a un familiar o a uno mismo le derribará un misil por error pero no es tan evidente que una prueba médica pueda potencialmente ser igual de peligrosa. Y si miramos la cifras de personas que mueren por reacciones a contrastes, perforaciones, sangrados, complicaciones y demás veremos que superan al año las de varios aviones. Si incluimos las personas a las que se les opera, amputa o somete a procesos innecesarios por falsos positivos la lista se alargaría muchísimo más. 


Por estas razones me parecería de mucha ayuda incluir en las hojas de consentimiento informado habituales una pequeña infografía donde se ilustre este tema para que quien tenga que someterse a una prueba diagnóstica sepa a qué probabilidades se somete tanto de posibles complicaciones como de falsos positivos. El consejo y el criterio del profesional sanitario seguirá siendo imprescindible pero quizá los números ayuden por sí mismos a más de uno.

Explaining mathematics is not usually easy to health professionals but in this age of fake news and half-truths it is increasingly necessary. 

Let's take a recent example. Imagine that we raise the political-military tension in a country to the maximum. Imagine that at some point someone has doubts that a flying object could be an enemy plane. Imagine that you push a button and a missile knocks down a passenger plane. This is a false positive: to label as harmful something that is not. 

In medicine we often do this because every diagnostic technique, I emphasize, produces false positives. From the simplest to the most sophisticated, with greater or lesser probability, it is an objective fact that increases as the number of tests carried out increases.

As the mantra "more is better" contaminates the health services, citizens demand all possible tests "because I am worth it" without knowing the danger they run by submitting to them. If a test is interpreted as positive it will imply other tests or measures. Thus, if the stool analysis detected occult blood, a colonoscopy will be ordered and if the mammography identifies a strange image, a puncture biopsy and in both cases a resection of what is considered suspicious. 

There is a great deal of debate with screening programs that are applied to healthy populations because they are somehow a way of raising the "political-military" health tension. The more disease you expect, the more you will find (true positives) and in the process we generate a good handful of side effects (false positives).

No one would like to see a family member shot down by mistake but it is not so obvious that a medical test can potentially be just as dangerous. And if we look at the numbers of people who die from reactions to contrast, punctures, bleedings, complications and so on, we see that they exceed the numbers of several crashed planes in a year. If we include the people who are operated on, amputated or subjected to unnecessary procedures for false positives the list would be much longer. 

For these reasons I think it would be very helpful to include in the usual informed consent forms a small computer graphic illustrating this issue so that anyone who has to undergo a diagnostic test knows what the chances are of both possible complications and false positives. The advice and judgement of the health professional will still be essential but perhaps the numbers themselves will help more than one.

Sobre la profilaxis del VIH con pastillas (pPrep)




  • El tratamiento preventivo del VIH con pastillas (pPrep) está disponible desde hace años. El Ministerio de Sanidad aprobó la financiación pública del medicamento hace un mes pero aún no está disponible en la mayoría de las Comunidades. En Cataluña se puede conseguir en muchos hospitales y en Madrid en el centro Sandoval (c/Sandoval 7) a partir de finales de diciembre. 
  • La pastilla se toma una vez al día y tras una semana de uso protege contra la infección al 99% si se toma diaria y con menor porcentaje si no es así. 
  • En España no está disponible en farmacias y quien la necesite debía comprarla por Internet (30-50€ al mes).
  • Es un tratamiento a considerar para personas que estén en riesgo de contactar con el virus y que no quieran usar preservativos, que es la opción en este momento más segura. 
  • Como médico me llama poderosamente la atención:


1. Que aún no esté disponible en farmacias (lo que obliga a comprarlo online con riesgo).
2. Que la mayoría de los profesionales sanitario no tengamos información al respecto.

¿Qué es la Profilaxis Pre-exposición (PrEP)?

  • Pre = antes (previa)
  • Exposición = tener contacto con el VIH
  • Profilaxis = tratamiento para prevenir que ocurra una infección
La profilaxis Pre-exposición (PrEP, siglas en inglés) es una estrategia de prevención de la infección con el VIH donde los individuos VIH negativos toman medicamentos anti-VIH antes de entrar en contacto con el VIH para reducir su riesgo de infectarse. Los medicamentos previenen que el VIH establezca una infección dentro del cuerpo.
PrEP ha demostrado reducir el riesgo de infección con el VIH a través del contacto sexual en los  hombres gay y bisexuales, mujeres transgénero y en hombres y mujeres heterosexuales, así como en personas que se inyectan drogas.
No protege contra otras infecciones de transmisión sexual (ITS) ni tampoco previene el embarazo. No es una cura para el VIH.

¿Para quién funciona la PrEP?

La PrEP ha demostrado reducir el riesgo de infección con el VIH en muchos estudios. El estudio iPrEx demostró que la PrEP reduce el riesgo de infección con el VIH entre los hombres gay y bisexuales y en las mujeres transgénero. Dos grandes estudios, Partners PrEP y TDF2, demostraron que PrEP también reduce el riesgo de infección con el VIH entre los hombres y mujeres heterosexuales. El estudio Bangkok Tenofovir demostró que la PrEP funciona para las personas que se inyectan drogas.




Efectividad

El uso de Truvada como PrEP provee una reducción del riesgo de infectarse del 96%  al 99% en las personas VIH negativas que toman las píldoras todos los días de acuerdo con  las indicaciones. Si se saltea una dosis diaria, el nivel de protección contra el VIH puede disminuir. Sólo funciona si toma el medicamento. Las personas que usan la PrEP correcta y consistentemente tienen niveles más altos de protección contra el VIH.
De acuerdo con el análisis de los datos del estudio iPrEx se descubrió que la PrEP es efectiva para:
  • Las personas que toman 7 píldoras de PrEP por semana, su nivel estimado de protección es del 99%
  • Las personas que toman 4 píldoras de PrEP por semana, su nivel estimado de protección es del 96%
  • Las personas que toman 2 píldoras de PrEP por semana, su nivel estimado de protección es del 76%
No existe suficiente data disponible para proveer una guía temporal específica que no sea de uso diario, por lo que la FDA recomienda que la PrEP se use diariamente para alcanzar el nivel más alto de protección.
Un par de cosas que deben tenerse en cuenta:
  • Al comenzar la PrEP, lleva al menos siete días para alcanzar altos niveles de protección contra el VIH.
  • Al suspender la PrEP, las personas deben continuar utilizando la PrEP durante cuatro semanas después de una exposición significativa.
  • La PrEP no protege contra otras infecciones de transmisión sexual (ITS) ni previene el embarazo. No es una cura para el VIH.




Efectos secundarios

Truvada como PrEP es generalmente seguro y bien tolerado. La mayoría de las personas que usan la PrEP reportan no experimentar efectos secundarios, pero se reportaron algunos efectos secundarios en los estudios clínicos. Los participantes del estudio iPrEx reportaron efectos secundarios que se encuadran en cuatro categorías principales (aquí ordenadas de más a menos comunes):
  1. Náusea: El 9% de quienes recibieron Truvada reportaron náuseas en el primer mes, en comparación con el 5% de los que recibieron placebo. Después del primer mes, no hubo diferencia en las tasas reportadas de náuseas entre los que recibieron Truvada y quienes recibieron placebo.
  2. Dolores de cabeza: El  4.5% de las personas que recibieron Truvada reportaron dolores de cabeza, en comparación con el 3.3% de quienes recibieron placebo.
  3. Pérdida de peso: El 2.2% de las personas que recibieron Truvada reportaron una pérdida de peso no intencional del 5%, en comparación con el 1.1% de los que recibieron placebo.
  4. Pequeños aumentos de la creatinina sérica: se sabe que Truvada causa pequeños aumentos en la creatinina sérica, una molécula de ocurrencia natural que es filtrada en los riñones. En este estudio, el 0.3% de quienes recibieron Truvada experimentaron aumentos moderados en la creatinina sérica que persistieron hasta el siguiente análisis. Los niveles de creatinina volvieron a bajar una vez que estos participantes abandonaron la PrEP. Cuatro de los cinco participantes retomaron la PrEP sin que volviera a aumentarles el nivel de creatinina. Los investigadores monitorearon la función renal durante la duración del estudio y no encontraron problemas serios.
Para la mayoría de las personas, estos efectos secundarios desaparecieron por si solos después de las primeras semanas de tomar Truvada, o cuando se suspendieron los medicamentos.

¿Quién es mi médico?


En esta época de hiperinformación a todos nos gusta saber quién tenemos delante en cada momento. Antes de consumir un bien o un servicio buscamos referencias, opiniones, valoraciones. Cuando estamos enfermos y necesitamos asistencia sanitaria también. Como médico conozco de primera mano que mis pacientes quieren saber quién soy. Buscan en Internet y encuentran cosas, fundamentalmente mi blog y cuenta de Twitter son mis principales tarjetas de visita, también mis vídeos y mi libro Diario de un médico descalzo

He de reconocer que es en el libro dónde más me mojo. Tuve la oportunidad de presentarlo en el pueblo donde actualmente desarrollo mi labor y es raro que semanalmente no reciba algún comentario del mismo. No es verdad que la gente no lea, todos leemos lo que verdaderamente nos interesa. Por eso le doy tanta importancia a este ejercicio de humildad que implica descalzarse ante las personas que uno sirve y mostrar un poco de aquella parte íntima de uno mismo: lo que uno piensa, siente, cree y espera. 

No es lo normal. Lo que se suele mostrar en público de uno mismo suele ser una pose, una careta, que construimos con selfies, imágenes y un relato sobre dimensionado de lo que entendemos es nuestra mejor versión. Pero la gente suele diferenciar bien lo que es postureo de lo que no lo es. Por eso al escribir o construir un contenido que hacer público trato de aplicar la sencilla regla de tratar de ser quien soy, cosa que no siempre es fácil.

En la facultad de medicina no se nos enseña a comunicarnos fuera de la consulta. No se nos dan pautas para usar redes sociales ni tampoco para escribir libros sobre nuestra visión de la vida. Se nos orienta más bien a construir un curriculum formativo y profesional basado en méritos académicos, investigadores o laborales. A los pacientes no es lo que más les interesa de nosotros. Quieren saber si somos competentes pero también si somos humanos. Si somos buenos clínicos pero también si escuchamos correctamente, miramos a los ojos o tratamos con respeto. Lo van averiguando según nos van conociendo pero algunos necesitan saber más para decidir si depositar su confianza o su problema en nuestras manos. Por eso es un excelente complemento a la entrevista personal en consulta disponer de otros medios que ayuden a mostrar quienes somos. 

El clínico siempre debe hacerse las preguntas: quién tengo delante, cuál es su verdadero problema, a la par que el paciente se pregunta: quién es mi médico. Mientras más fácil se lo pongamos tal vez nos resulte más sencillo encontrar las respuestas que buscamos. Al fin y al cabo la medicina se basa en comunicación y para que sea de calidad esta debe apoyarse en confianza y esta a su vez en conocimiento mutuo.








Who's my doctor?


In this age of hyperinformation we all like to know who is in front of us at all times. Before consuming a good or a service we look for references, opinions, evaluations. When we are sick and we need health care also. As a doctor I know first hand that my patients want to know who I am. They search the Internet and find things, mainly my blog and Twitter account are my main business cards, also my videos and my book Diary of a Barefoot Doctor

I have to admit that it is in the book where I show more who am I. I had the opportunity to present it in the Villaba where I currently work and it is rare that I don't receive any comment about it every week. It is not true that people do not read, we all read what really interests us. That's why I attach so much importance to this exercise in humility that involves taking off your shoes in front of the people you serve showing a little of that intimate part of yourself: what you think, feel, believe and expect. 

It is not normal. What is usually shown in public of oneself is usually a pose, a mask, that we build with selfies, images and a story about the dimension of what we understand is our best version. But people usually differentiate well what is a más and what is not. That's why when I write or build content that I make public I try to apply the simple rule of trying to be who I am, which is not always easy.

In medical school we are not taught to communicate outside the practice. We are not given guidelines for using social networks or for writing books about our vision of life. Rather, we are guided to construct a formative and professional curriculum based on academic, research or labor merits. Patients are not the most interested in that. They want to know if we are competent but also if we are human. If we are good clinicians but also if we listen correctly, look into their eyes or treat with respect. They find out as they get to know us, but some need to know more to decide whether or not to put their trust or their problem in our hands. That is why it is an excellent complement to the personal interview in consultation to have other means that help to show who we are. 

The clinician should always ask himself the questions: who is in front of me, what is his real problem, at the same time as the patient asks himself: who is my doctor. The easier we make it, the easier it may be for us to find the answers we are looking for. After all, medicine is based on communication and for it to be of quality it must be based on trust and this in turn on mutual knowledge.
