Explicar la nueva sanidad. Explaining the new health care. 解釋新的醫療保健





Al final tendremos que ser los profesionales sanitarios quienes expliquemos en qué consiste la nueva sanidad. No esperen que políticos o gestores lo hagan. Es demasiado impopular, destruye muchos votos y cabrea sobremanera al personal. Disculpen el tono pero está la cosa caliente. 

Los hechos: llevamos meses enfrentándonos como sociedad globalizada a una pandemia. Cada país se intenta defender con políticas y recursos, dentro de estos con su sistema sanitario. En España contamos con un sistema de asistencia sanitaria precarizado y pobremente financiado que está muy debilitado por los recortes de los últimos años. Muchos profesionales sanitarios se sienten maltratados y están al borde de quemarse.

El problema: al lidiar con una enfermedad infecciosa muy contagiosa es necesario tomar medidas para evitar situaciones de peligro y proteger a la población. El uso de mascarillas, mantener la distancia y evitar aglomeraciones en sitios cerrados son ejemplos. Los centros sanitarios son sitios cerrados muy concurridos donde coinciden mucha gente enferma. Es fundamental que no se conviertan en focos de infección, para ello las salas de espera deben estar casi vacías. Esto implica modificar profundamente el tipo de asistencia, potenciando los canales de comunicación no presenciales y limitando la asistencia directa a lo imprescindible. 

Los ciudadanos están muy enfadados al ver que de un día para otro les han quitado de las manos el tipo de asistencia sanitaria que tenían: muy accesible (cita en el día o al día siguiente en el centro de salud), universal y gratuita. En lugar de poder ir a la consulta de la enfermera o la médica ven que les llaman por teléfono y se sienten engañados. 

Me temo que no hay trampa ni cartón. Por un lado el Sistema Sanitario no puede más, por otro la pandemia de coronavirus no permite que las cosas sean como antes. 

Habrá que acostumbrarse a que ya no es posible ir a urgencias a las dos de la madrugada por un grano o acudir sin cita al médico de cabecera porque llevo una hora con dolor de garganta. Las cuestiones de salud menores se deberán solucionar en casa sin acudir a ningún centro, como se ha hecho toda la vida, y para las más graves llamar por teléfono y preguntar qué hacer. Las urgencias vitales se seguirán atendiendo en el momento, pero me temo que solo estas estarán justificadas. Lo demás deberá esperar o deberá ser atendido de otra forma.

Esto no lo se lo contarán por televisión ni verán al señor ministro o al consejero de sanidad de turno explicándolo. Por eso como sanitarios se lo contamos claramente. Lo más inteligente es que no vuelque su enfado sobre los profesionales que están para cuidarle sino sobre los responsables que deben poner los medios y los fondos para que esto sea posible. 





Explaining the new health care

Since no one has done so, it will be up to us health professionals to explain what the new health system is all about. Do not expect politicians or managers to do this. It is too unpopular, it destroys many votes and it makes society very angry.

The facts: we have been facing a pandemic for months as a globalised society. Every country is trying to defend itself with policies and resources, within these with its national health system. In Spain we have a precarious and poorly funded health care system that is severely weakened by the cuts of recent years. Many health professionals feel mistreated and are on the verge of burning out.

The problem: when dealing with a highly contagious infectious disease it is necessary to take measures to avoid dangerous situations and protect the population. Wearing masks, keeping your distance and avoiding crowds indoors are examples. Healthcare facilities are busy indoor places where many sick people congregate. It is essential that they do not become sources of infection, for which reason waiting rooms must be almost empty. This implies a profound change in the type of assistance, strengthening non-presential communication channels and limiting direct assistance to what is essential. 

Citizens are very angry when they see that from one day to the next they have been deprived of the type of healthcare they had: very accessible (appointment on the day or the next day at the health centre), universal and free. Instead of being able to go to the nurse's or doctor's office, they see that they are called on the phone and feel cheated. 

I'm afraid this is not a scam. On the one hand the Health System can't stand it anymore, on the other hand the coronavirus pandemic doesn't allow things to be as they used to be. 

We will have to get used to the fact that it is no longer possible to go to the emergency room at two in the morning for a pimple or to go without an appointment to the family doctor because I have had a sore throat for an hour. Minor health issues should be solved at home without going to any centre, as has been done all your life, and for the more serious ones, you should phone and ask what to do. Vital emergencies will continue to be attended to at the moment, but I fear that only these will be justified. The rest will have to wait or be dealt with in another way.

You will not be told this on television or see the minister or the health adviser on duty explaining it. That is why we, as health workers, are telling you clearly. The most intelligent thing is not to turn your anger on the professionals who are there to care for you, but on those responsible who must provide the means and funds to make this possible.

Trabajar por la salud. Working towards health. 為健康而努力






Una de las muchas paradojas del mundo sanitario es que pocos pueden decir que trabajan por la salud. La mayor parte del sistema está centrada en la enfermedad y además lo hace de una manera industrial. La estructura sanitaria está montada como una gran cadena de montaje con el fin de producir los máximos servicios sanitarios posibles. Más cirugías, más rehabilitaciones, más pruebas diagnósticas, más intervenciones, más tratamientos.

Dentro de este estado de cosas hay una pequeña aldea de irreductibles galos que todavía lucha por centrarse en la salud. La salud de las personas y de las comunidades. Este enfoque familiar y comunitario intenta tener en cuenta la realidad biológica de la persona pero también la psicológica, la social y la existencial. Pero no es nada fácil desarrollarlo con la profundidad necesaria dado que la Atención Primaria es la parte de la sanidad que más recortes ha sufrido y cuya financiación ha disminuido más.

Por una parte es necesario visibilizar la realidad pero quizás lo sea más visibilizar el sentido de ejercer una Medicina con mayúscula que ayude a la persona en tiempo de enfermar pero también en tiempo de salud. Una medicina de visión amplia que permita el establecimiento de una relación sólida entre profesionales y pacientes basada en la confianza y en la comunicación. Una medicina con parte científico-técnica y parte humana que no queme a sus profesionales para que estos no quemen a sus pacientes.

Working towards health

One of the many paradoxes of the health world is that few can claim to work towards health. Most of the system is focused on disease and it does so in an industrial way. The health structure is set up like a large assembly line in order to produce the maximum possible health services. More surgeries, more rehabilitations, more diagnostic tests, more interventions, more treatments.

Within this state of affairs there is a small village of irreducible Gauls who still struggle to focus on health. The health of individuals and communities. This family and community approach tries to take into account the biological reality of the person but also the psychological, social and existential reality. But it is not easy to develop it with the necessary depth given that Primary Care is the part of the National Health System that has suffered the most cuts and whose funding has decreased the most.

On the one hand, it is necessary to make reality visible, but perhaps more so to make visible the meaning of practicing Medicine with a capital "M" that helps people in times of illness but also in times of health. A medicine with a broad vision that allows the establishment of a solid relationship between professionals and patients based on trust and communication. A medicine with a scientific-technical part and a human part that does not burn its professionals so that they do not burn their patients.

This week the new residents have chosen their speciality. I welcome them all again and encourage them on the path they are starting.

Sanidad rural. Guía para alcaldes desesperados. Rural health. Guide for desperate mayors. 農村衛生。絕望的市長指南。






El mundo rural sufre multitud de amenazas y carencias. A nivel sanitario padece una anemia crónica de profesionales dado lo difícil que es cubrir las plazas más alejadas o complejas y a décadas de desfinanciación sanitaria.

En verano es siempre mucho peor. El personal de salud marcha de vacaciones y cada vez hay menos suplentes. Esto implica que muchas consultas permanecerán cerradas, lo que se suma a las que ya llevan tiempo sin servicio. 

Este problema afecta a los países vecinos que luchan contra él con incentivos que terminan atrayendo profesionales españoles cuyas condiciones son mucho peores. 

En el momento actual la situación es delicada por la acumulación de años de recortes sanitarios y el hartazgo de los profesionales que prefieren emigrar a lugares más ventajosos. 



¿Porqué es tan difícil encontrar profesionales sanitarios para zonas rurales?

1. Por que están más alejadas de los lugares de residencia de los profesionales y estos han de gastar más tiempo y gasolina en llegar. 

2. Por que con frecuencia son dispositivos pequeños donde hay que trabajar solo o con pocos apoyos.

3. Por la mayor dificultad técnica al tener que atender urgencias vitales y todo tipo de patología compleja.



¿Cómo incentivar que las zonas rurales cuenten con una asistencia sanitaria correcta?

1. Dimensionando correctamente las plantillas. 

2. Incentivando el tiempo trabajado con algún suplemento de puntuación de cara a oposiciones o traslados.

3. Con incentivos económicos de parte de la Consejería de Salud de la Comunidad Autónoma.

4. Con incentivos del ayuntamiento. Antiguamente los ayuntamientos ofrecían vivienda gratuita y otras ventajas a docentes y sanitarios. Hoy tal vez podrían ofrecer vehículo, dietas de transporte, bonos de gasolina, carga eléctrica de coches u otros servicios. 

5. Abriendo y potenciando procesos de participación comunitaria. El consultorio local no debería ser el centro de la salud de la comunidad, sino un apoyo más. Las dinámicas de salud comunitaria lideradas por la propia comunidad pueden potenciar la red de apoyos y favorecer los autocuidados. 

Por otra parte redimensionar el contrato sociedad-medicina puede ser una opción interesante para que no todo malestar acabe en una consulta sanitaria. 


¿Qué más se puede hacer?

Actualmente España no cuenta con un plan de incentivación del mundo rural más allá de lo bien que queda en los discursos. Mientras no haya políticas y presupuestos todo seguirá igual. 

Los ayuntamientos podrían:

1. Asociarse para presionar a sus representantes en la Comunidad Autónoma y Consejería de Sanidad y visitarles con periodicidad para recordar la necesidad de cumplir los compromisos dados.

2. Pedir inspecciones ante la magistratura de trabajo (especialmente en verano).

3. Visibilizar en redes sociales y en prensa local y nacional su situación de indefensión.

4. Presentar las quejas que se consideren necesarias en los organismos públicos nacionales, internacionales y ONGs.



También parece necesario abrir un debate social local y general. Por un lado cada pueblo y territorio necesita repensar cómo adaptarse a una situación de déficit estructural de recursos y por otro la ciudadanía en general debería plantearse la inviabilidad de un contrato sociedad-medicina tipo supermercado y en su lugar buscar otros basado en las necesidades reales de salud. No solo se trata de pedir más recursos sino de plantearnos en profundidad cómo manejarnos con los que tenemos e incluso con menos.

3. Because of the greater technical difficulty in attending to vital emergencies and all types of complex pathology.




How can we encourage rural areas to have correct health care?


1. By correctly sizing the templates. 

2. By encouraging the time worked with some supplemental score for competitions or transfers.

3. With economic incentives from the Regional Health Systems of the Autonomous Community.

4. With incentives from the city council. In the past, the city council offered free housing to teachers and health workers. Today it could offer vehicle, transport allowances, petrol bonuses, electric vehicle charging or other services. 

5. With processes of community participation. The local office should not be the center of community health, but rather a support for it. Community health dynamics led by the community itself can strengthen the support network and encourage self-care. 

On the other hand, redimensioning the society- medicine contract can be an interesting option so that not all discomfort ends up in a health consultation. 


What else can be done?

Currently Spain does not have a plan to encourage the rural world beyond good words in the political speeches. As long as there are no policies or budgets, everything will remain the same. 

The city councils could join forces to put pressure on their representatives in the Autonomous Community. It also seems necessary to open a local and general social debate. On the one hand, each town and territory needs to rethink how to adapt to a situation of structural deficit in resources, and on the other hand, citizens in general should consider the viability of a society-medicine contract like a supermarket instead of one based on real health needs.

El peligro de que los sanitarios entren en modo supervivencia. The danger of health professionals going into survival mode. 卫生专业人员进入生存模式的危险。





Tratar con el dolor y la enfermedad ajena no es sencillo para nadie. Tampoco para los que tienen formación especializada. El sufrimiento impregna la parte invisible del alma humana, tanto del que lo padece como del que lo acompaña.

Por eso es necesario que los profesionales sanitarios tengan una formación técnica y humana completa y trabajen con medios que les permitan ofrecer un servicio de máxima calidad. Lo que vemos habitualmente es que las instituciones sanitarias conciben la calidad desde un punto de vista exclusivamente científico-técnico y se centran en proveer medios técnicos, farmacológicos e informáticos. La parte humana se obvia y, pese a estar presente en el discurso oficial, no se cuidan las condiciones laborales ni de protección de sobrecargas de los profesionales. En ocasiones los profesionales trabajan en clara situación de maltrato, como los residentes que encadenan tres turnos de trabajo seguidos en sus guardias o los médicos de familia obligados a atender a más de 60 pacientes en una jornada.

Es cierto que el entrenamiento de los profesionales permite que estos puedan asumir aumentos puntuales de demanda asistencial, pero no son de piedra. Nadie aguanta una sobrecarga permanente sin entrar en modo supervivencia. Y si sobrevivir se convierte en la prioridad, el paciente, el sistema y todo lo demás será secundario. Esto no beneficia a nadie pero cada vez es más frecuente porque son más los profesionales que se sienten sobrepasados, ninguneados, sobrecargados y abusados. La situación es global, pasa en todos los países, y tiene que ver con la dinámica social que ha convertido la salud en un bien de consumo y los sistemas sanitarios en grandes supermercados de servicios que cada vez han de producir más. Si las necesidades de salud siguen creciendo a la par que lo hacen la edad media y la complejidad de los pacientes, podrán imaginar que si los recursos no aumentan serán los profesionales quienes se carguen sobre sus hombros el aumento progresivo de trabajo.

El problema es que en modo supervivencia cada profesional se quita de encima los problemas lo más rápido que puede. Se prescriben más pruebas diagnósticas y más tratamiento, se deriva más de un profesional a otro. Se mantiene el problema en superficie sin que nadie coja el toro por los cuernos. Esto desencadena cadenas gasto inasumible, pérdidas de tiempo y de recursos, y pocos resultados.
Cuando es la Atención Primaria lo más abusado y menos cuidado el problema aumenta al ser en este nivel donde más información blanda se tiene del paciente y donde se pueden manejar el 90% de sus problemas si se dieran las condiciones adecuadas. Estrangular este nivel asistencial es condenar al fracaso la totalidad del sistema y es lo que estamos viendo año tras año con el asombro de unos, el consentimiento de otros y la dejadez o estulticia de alguno más.

Permitir que un médico de familia tenga que asumir en verano cinco consultas a la vez debería ser delito. Permitir que haya centros de salud cuyos médicos tengan que asumir las consultas no suplidas de compañeros durante meses no debería tolerarse.

Pero es lo que hay, y cada vez más profesionales quemados, con la moral por los suelos o agotados. Entramos en modo supervivencia, no pidan empatía, humanismo ni florituras. No las va a haber.







The danger of health professionals going into survival mode.


Dealing with the pain and illness of others is not easy for anyone. Nor is it easy for those with specialized training. Suffering permeates the invisible part of the human soul, both of the sufferer and of those who accompany him. 

That is why it is necessary for health professionals to have a complete technical and human formation and to work with means that allow them to offer a service of maximum quality. What we usually see is that health institutions conceive of quality from an exclusively scientific-technical point of view and focus on providing technical, pharmacological and IT means. The human side is forgotten, in spite of being present in the official discourse, the working conditions and overload protection of the professionals are not taken care of. Sometimes the professionals work in a clearly abusive situation, such as residents who work three shifts in a row on their guards or family doctors who are forced to attend to more than 60 patients in a day. 

It is true that the training of the professionals allows them to take on occasional increases in the demand for care, but they are not made of stone. No one can withstand a permanent overload without going into survival mode. And if survival becomes the priority, the patient, the system and everything else will be secondary. This does not benefit anyone but it is increasingly frequent because more and more professionals feel overloaded, unaware, overburdened and abused. The situation is global, it happens in all countries, and it has to do with the social dynamics that have turned health into a consumer good and the health systems into large service supermarkets that have to produce more and more. If health needs continue to grow at the same rate as the average age and complexity of patients, you can imagine that if resources do not increase, it will be the professionals who will have to shoulder the progressive increase in work. 

The problem is that in survival mode each professional gets rid of the problems as quickly as possible. More diagnostic tests and more treatment are prescribed, more are referred from one professional to another. The problem is kept on the surface without anyone taking the bull by the horns. This triggers chains of unconscionable expense, loss of time and resources, and poor results. 
When it is the Primary Care that is most abused and least cared for, the problem increases as it is at this level where more soft information is held on the patient and where 90% of their problems can be managed if the right conditions are given. To strangle this level of care is to condemn the whole system to failure and this is what we are seeing year after year with the astonishment of some, the consent of others and the negligence or silliness of some others. 

It should be a crime to allow a family doctor to have to take on five consultations at a time in the summer. Allowing health centres to have doctors take over unsupplied consultations from colleagues for months on end should not be tolerated. 

But there are, and more and more professionals who are burned out, morally bankrupt or exhausted. We're going into survival mode, so don't ask for empathy, humanism or frills. There won't be any.

Who will lead the health care?

Many of us have been wondering for a long time if we will ever see someone who will adequately lead the health sistema. And the tricky thing is to get to know what the hell means in a proper way. For some, it's keeping costs down and maintaining the freedom to choose a private service for whomever they want. For others, it is that the service should be forever universal and free. The consensus is similar a Gordian knot. 

Therefore, there will be no leaders to suit everyone. Nor will we be able to reach an agreement if we define them as defenders of the common good.

I expect nothing from the management structures. I have been on the NHS boat for many years now and I have heard only good words and seen very few gestures. Nor do I expect anything from professional organisations, which are dense, dusty and unfriendly to change and innovation. 

I hope that if there is a value proposition, it will come from the health professionals, as we have seen in this pandemic in which health managers and organisations have lagged behind the care providers by weeks. 

The problem is that with the current level of overload, professionals are focused on a single priority: survival. And this, I am afraid, does not benefit anyone, not society, which is aware that its health care facilities are burnt out and on the verge of exhaustion, not the managers who see that it is impossible to make changes, not the politicians who, despite announcing new hospitals, see the system crumble. 

What strength is left for a family doctor when at the end of her day she leaves exhausted because she has seen her patients, those of her colleague who is on leave without a substitute, those without an appointment, the home visitsand the telephone calls? What energy is left for the ICU nurse with the units full? What encouragement do I give to those who support the emergencies and see how more and more people are coming?

Cannon fodder. It's sad to realize that we all are. We're gonna need to hit rock bottom to make the decision to step up. Swarming societies may not help, but that does not mean that all doors will be closed to us. Everyone can make a revolution at the edges of their own inner world. That is where the future lies. If we manage to make contact with ourselves and see that we need very little to survive and that the potential for human growth that is open to us is almost infinite, we may have a chance. 






¿Quién liderará la sanidad?

Muchos llevamos tiempo preguntándonos si llegaremos a ver a alguien que lidere de una manera adecuada la sanidad. Y lo complicado es llegar a saber qué diablos es eso de una manera adecuada. Para unos es que los costes se mantengan contenidos y mantener la libertad de elegir un servicio privado para el que quiera. Para otros que el servicio sea por siempre universal y gratuito. El consenso no es que sea difícil sino que ha devenido en nudo gordiano. 

Por tanto no habrá líderes a gusto de todos. Tampoco si los definimos como defensores del bien común, termino para el que de nuevo no nos conseguiremos poner de acuerdo.

No espero nada de las estructuras de gestión. Son ya muchos años en el barco y solo he escuchado buenas palabras y visto muy pocos gestos. Tampoco de las organizaciones profesionales, densas, polvorientas y poco amigas al cambio y la innovación. 

Abrigo la esperanza de que si hay alguna propuesta de valor esta saldrá de los profesionales como hemos visto en esta pandemia en la que las gerencias y organizaciones sanitarias han ido semanas por detrás que los asistenciales. 

El problema es que con el nivel actual de sobrecarga los profesionales se centran en una única prioridad: sobrevivir. Y esto me temo no beneficia a nadie, ni a la sociedad que consiente que sus sanitarios estén quemados y al borde de la extenuación, ni a los gestores que ven que es imposible hacer cambios, ni a los políticos que pese a anunciar nuevos hospitales ven como el sistema se desmorona. 

¿Qué fuerza le queda a una médica de familia cuando al terminar su jornada sale agotada por haber visto sus pacientes, los de la compañera que está de baja sin suplente, los sin cita, los domicilios y las llamadas telefónicas? ¿Qué energía le queda a la enfermera de UCI con las unidades llenas? ¿Qué animo a los que sostienen las urgencias y ven como cada vez acude más gente?

Carne de cañón. Es triste darse cuenta de que todos lo somos. Vamos a necesitar tocar fondo para tomar la decisión de dar un paso al frente. Las sociedades convertidas en enjambre quizá no ayuden pero no por ello se nos cerrarán todas las puertas. Cada cual puede llevar a cabo una revolución en los límites de su mundo interno. Será ahí donde nos juguemos el futuro. Si conseguimos tomar contacto con nosotros mismos y comprobar que para subsistir hace falta bien poco y que las potencialidades de crecimiento humano que se nos abren son casi infinitas, tal vez tengamos alguna posibilidad.

Burocracia y Covid-19 en Madrid





Transcribo un tuit de mi compañero Jose María Molero por su interés:


El procedimiento de @SaludMadrid de detección precoz y diagnóstico de nuevos casos de infección #COVIDー19 en AP, ante un caso confirmado parece ciencia ficción pero no lo es:


  1. Hay que crear 2 episodios en la historia clínica electrónica,
  2. Cumplimentar 3 protocolos,
  3. Cambiar el código CIE 10 para la gestión de la Incapacidad Transitoria
  4. Hacer test PCR en menos de 24 horas,
  5. Comunicar el resultado,
  6. Hacer seguimiento telefónico del caso,
  7. Identificar contactos sospechosos,
  8. Tramitar las bajas de los contactos si son trabajadores,
  9. Realizar PCR a los contactos si tienen síntomas
  10. y si es positiva actuar con ellos como nuevos casos confirmados (volver al paso 1)...


Además de seguir con la consulta, urgencias, domicilios...



Como sean muchos vamos dados... de momento no hemos recibido en Atención Primaria de Madrid ningún refuerzo conocido, ni médicos rastreadores ni nada... sigo confiando en que vengan pero...

La Atención Primaria ante la nueva normalidad, ¿alguna vez fue normal? Primary Care in the face of the new normality, was it ever normal? 面對新的常態,初級保健曾經是正常的嗎?





La sanidad española ha ido experimentado modificaciones con el tiempo que han tendido a reforzar con tecnología, financiación y recursos humanos los hospitales desatendiendo los servicios de Atención Primaria, Salud Pública y Salud Mental comunitaria.

Han pasado muchos años desde la reforma de la Atención Primaria de 1984. Y largos los ríos de tinta aportando ideas y pidiendo ajustes y mejoras. Mucho ruido y pocas nueces por cierto. 

Los hechos nos dicen que el cambio de modelo no llegó a cuajar del todo sobre todo por falta de apoyo político y financiación pero también por la inercia del sistema, la impotencia o dejadez de los profesionales y el sobreuso de la misma por parte de la ciudadanía. 

Pese a los esfuerzos de un gran número de profesionales abnegados nuestra Atención Primaria nunca fue normal del todo, más bien algo sub-normal, si me permiten la ironía. Aunque algunos centros de salud de gran tamaño han podido desarrollar su labor con ciertas garantías otros muchos, así como un gran porcentaje de consultorios locales, han sufrido un permanente estado de penuria y sobrecarga de larga duración.

¿Qué esperar para el futuro cercano de nueva normalidad?

Pues me temo que mucho deberían cambiar las cosas para dejar atrás la subnormalidad previa y avanzar hacia algo mejor. Si nada lo remedia iremos otra vez para atrás con una crisis económica que hará difícil conseguir una financiación sanitaria que nos iguale a los países de nuestro entorno. 

Y en cuanto a los cambios y ajustes que no requieren grandes presupuestos (minimizar burocracia, mejorar la coordinación entre centros y niveles, sistemas de información, historia clínica electrónica mejorada, autonomía de gestión de los centros, etc...) me gustaría ser optimista, dejémoslo ahí.

The facts tell us that the change of model did not fully take hold, above all due to a lack of political support and funding, but also due to the inertia of the system, the impotence or neglect of the professionals and the overuse of the system by the citizens. 


Despite the efforts of a large number of dedicated professionals, our Primary Care was never completely normal, rather something sub-normal, if I may be ironic. Although some large local health centres have been able to carry out their work with certain guarantees, many others, as well as a large percentage of small rural clinics, have suffered a permanent state of hardship and long-term overload.


What to expect for the near future of new normality?


Well, I am afraid that a lot should change in order to leave behind the previous subnormality and move towards something better. If nothing remedies it, we will go back again with an economic crisis that will make it difficult to obtain health financing that will put us on a par with the countries around us. 


And as for the changes and adjustments that do not require large budgets (minimizing bureaucracy, improving coordination between centres and levels, information systems, improved electronic medical records, management autonomy of centres, etc...) I would like to be optimistic, let's leave it at that.
