Navegar la incertidumbre. Navigating uncertainty. 應對不確定性

La verdadera pandemia se llama incertidumbre y tiene que ver con la falta de seguridad ante la posibilidad de enfermar, de insuficiente confianza en las instituciones y responsables y de poca certeza ante un futuro impredecible que provoca gran inquietud. 

Es una situación de crisis social similar a las de otras épocas con la peculiaridad de que en esta ocasión es global y audiovisual, lo que aumenta la sincronía y el contagio emocional. Muchos de los que la vivimos la experimentamos como algo novedoso, intenso e inédito, lo que aumenta más la percepción de gravedad y agobio.

Las circunstancias de las pandemias epidémicas suelen hacer que estas sean muy difícilmente controlables. Normalmente se requiere mucho tiempo para que las aguas vuelvan a su cauce. Esto es muy difícil de asumir para una sociedad infantilizada acostumbrada a conseguir lo que desea con inmediatez. En esta ocasión el problema no se va a poder arreglar únicamente poniendo dinero encima de la mesa. Nos las estamos viendo con un fenómeno natural y a la naturaleza le importan un bledo nuestras finanzas.

Si observamos el nivel de gobernanza de los distintos territorios veremos que pese a las consabidas excepciones se ha generalizado el malestar ante una evidente falta de liderazgo. Lo habitual es que los gestores políticos señalen a otros como culpables de la situación y nadie asuma responsabilidad alguna ni actúe en consecuencia. Esto suele enfadar a la ciudadanía que termina irritada tanto si se toman medidas, que suelen ser impopulares, como si no se toman, lo que se percibe como dejadez. 

Los cursos de acción que puede tomar el político son principalmente coercitivos: limitación de derechos, movilidad, escolarización... Y muy limitados a la hora de promover la responsabilidad del ciudadano y las conductas cívicas, que lógicamente dependen de cada cual. Tampoco hay gran capacidad de trasvasar recursos de una institución a otra como por ejemplo reforzar la sobrecargada sanidad con recursos de protección civil, ayuntamientos u otros trabajadores cuando estos estuvieron confinados. 

La complejidad de la situación obliga a rescatar a nivel personal y comunitario dos importantes valores como son la creatividad y la flexibilidad. Será necesarios hacer cambios y ajustes en nuestra conducta personal y familiar a la hora de relacionarnos con los espacios públicos y con los demás. Variar costumbres, evitando algunas y rescatando o reforzando otras. Aprender modos sociales seguros y adaptarnos a tener menos interacción social presencial. 

A nivel comunitario será fundamental potenciar las redes de cuidado y protección social así como la sanitaria. Toda asociación de personas formal o informal puede  aportar valor, desde ONGs, parroquias/Cáritas, asociaciones de vecinos, clubes deportivos, grupos culturales, etc... poniendo el foco en los más vulnerables, excluidos, mayores y enfermos. La coordinación de estos agentes con los Servicios Sociales de zona sería muy deseable, lo que implicará mejorar los puentes de comunicación.

Será necesario dar respuestas a la formación académica de los niños que tal vez no pueda ser del todo escolarizada y deba manejarse familiarmente o en pequeños grupos de clases particulares. También a la asistencia sanitaria que claramente no podrá recaer únicamente en los recursos limitados del barrio o pueblo que tal vez puedan verse reforzados por algún tipo de voluntariado, o medios del ayuntamiento, protección civil u otros recursos. Y sobre todo tomando conciencia de que lo correcto es solicitar asistencia sanitaria únicamente cuando sea imprescindible. 

Todos estos cambios requerirán la implicación de grandes mayorías, lo que no será fácil sin la participación de los líderes naturales de los distintos ámbitos y territorios. Un liderazgo basado en el ejemplo y el servicio. De poco nos van a servir ahora la queja y la protesta cuando estamos navegando dentro de la tormenta. Es necesario cambiar la actitud y recordar que precisamente ahora es cuando más necesidad tiene la sociedad de sus servidores públicos sean estos limpiadores, profesores, agentes de seguridad, sanitarios, funcionarios, gestores o políticos. Incluyendo también a todos los que desde el ámbito privado pueden aportar valor social, poniendo temporalmente este por encima del beneficio económico.

Aportar servicio y cuidado a los demás es el camino más seguro para disminuir la incertidumbre de este tiempo. Cada cual deberá hacer una reflexión personal que le ayude a situarse en este contexto para que en lugar de permanecer inmóviles dentro de un confinamiento impuesto nos atrevamos a convertirnos en agentes que velan y trabajan por el bien de otros. 

Es fundamental aceptar que de este embrollo no nos va a sacar el estado ni ningún salvador externo, saldremos todos cuando se cumpla el tiempo que la naturaleza imponga. Lo que sí está en nuestra mano es el cómo. Lo podremos hacer incendiados en miedo o en ira ante el naufragio o remangados y con las manos metidas en faena mientras achicamos el agua de las muchas grietas que tiene el casco de nuestras sociedades. 




Navigating uncertainty


The real pandemic is called uncertainty, and it has to do with a lack of security in the face of the possibility of falling ill, insufficient trust in institutions and officials, and little certainty in the face of an unpredictable future that causes great concern. 

It is a situation of social crisis similar to those of other times with the peculiarity that on this occasion it is global and audiovisual, which increases synchrony and emotional contagion. Many of us are feeling it as something new, intense and unprecedented, which further increases the perception of gravity and distress.

The circumstances of epidemic pandemics often make them very difficult to control. It usually takes a long time for the waters to return to their course. This is very difficult to assume for an infantilized society accustomed to getting what it wants immediately. This time the problem will not be solved just by putting money on the table. We are dealing with a natural phenomenon and nature doesn't give a damn about our finances.

If we look at the level of governance of different countries, we see that, despite the well-known exceptions, there is widespread unease about an obvious lack of leadership. It is common for political managers to point to others as the culprits of the situation and no one takes responsibility or acts accordingly. This often angers citizens who end up being irritated whether action is taken, which is often unpopular, or not taken, which is perceived as neglect. 

The courses of action that the politician can take are mainly coercive: limitation of rights, mobility, schooling And very limited when it comes to promoting citizen responsibility and civic behaviour, which logically depend on each individual. Nor is there much capacity to transfer resources from one institution to another, such as reinforcing overburdened health care with resources from civil protection, town councils or other workers when these were confined. 

The complexity of the situation means that two important values, creativity and flexibility, have to be rescued at personal and community level. It will be necessary to make changes and adjustments in our personal and family behaviour when relating to public spaces and others. Varying customs, avoiding some and rescuing or reinforcing others. Learn safe social modes and adapt to having less face-to-face social interaction. 

At the community level it will be fundamental to strengthen the networks of care and social protection as well as health. Any formal or informal association of people can provide value, from NGOs, parishes/charitable organizations, neighborhood associations, sports clubs, cultural groups, etc... focusing on the most vulnerable, excluded, elderly and sick. The coordination of these agents with the Social Services of the area would be very desirable, which will imply improving new bridges of communication.

It will be necessary to respond to the academic training of children which may not be entirely schoolable and must be handled by families or in small groups of private or voluntary classes. Also to health care, which clearly cannot fall solely on the limited resources of the neighbourhood or village, which can perhaps be reinforced by some kind of voluntary work, or means from the town hall, civil protection or other resources. And above all, being aware that the right thing to do is to ask for health care only when it is essential. 

All these changes will require the involvement of large majorities, which will not be easy without the participation of the natural leaders of the different areas and territories. We must change our attitude and remember that it is precisely now that society most needs its public servants, whether they are cleaners, teachers, security agents, health workers, civil servants, managers or politicians. It is fundamental to accept that neither the state nor any external saviour will get us out of this mess; we will all get out when the time that nature imposes is over. What we can do is how. We can do it in fear or in anger at the shipwreck or with our sleeves rolled up and our hands in the water as we shrink the water from the many cracks in the hull of our societies.

Querida amiga, querido amigo,

No quería marchar sin antes dejarte esta carta personal de despedida porque, como sabes, todo en la vida suele tener un fin. Por una parte siento alegría por lo mucho conseguido pero por otra tristeza al contemplar el derrotero por el que va caminando la medicina. 

Desde el comienzo de los tiempos, los primeros sanadores desarrollaron con esfuerzo y sagacidad diversas aproximaciones que ayudaban a los seres humanos a caminar por la senda de la salud. Fue necesario viajar lejos, contemplar y probar, se precisó de gran paciencia y, pese a todo, los resultados nunca fueron excesivamente brillantes. Hasta que llegó la revolución científico-técnica. Hoy disponemos de la medicina más potente de la historia gracias a todos los avances que atesora y a sistemas que amplían enormemente su poder y lo trasladan a incontables territorios y personas. 

Tanto esplendor tiene su lado oscuro. La salud se ha convertido en un bien de consumo y como tal todos la desean. Las expectativas de salud son cada día mayores, lo que disminuye en proporción la tolerancia al malestar. Esto llega por un lado hasta el punto de que algunos crean nuevas enfermedades para poder vender las correspondientes pruebas y tratamientos y por otro a que ante cualquier pequeña incomodidad lo normal sea acudir a una consulta. Esta mercantilización ha transformado los grandes Sistemas Sanitarios en supermercados industriales de servicios de salud y a los profesionales en operarios de los mismos. 

La parte humana de la medicina, su espíritu, va dejando de tener cabida en este orden de cosas dado que para ejercerla hace falta algo de lo que cada vez hay menos: tiempo para hablar. La dimensión de humanidad del profesional de la salud es algo artesanal basado en la confianza, que se construye lentamente según avanza su conocimiento de los pacientes que trata. Hoy los Sistemas Sanitarios se basan en la eficiencia y la productividad como primer valor para alcanzar un beneficio lucrativo en los privados y de sostenibilidad en los públicos. La atención a la salud es un producto caro que hay que amortizar y para conseguirlo no hay más remedio que presionar al profesional que es la parte más flexible y vulnerable del sistema. Tocará ver más pacientes al día, hacerlo más rápido y trabajar más deprisa. 

La sobrecarga sanitaria es un hecho globalizado que ha ido progresivamente en aumento, aliviado en parte por los avances tecnológicos. Pero llega un punto en que al profesional no le es viable seguir siendo humano y entra en modo supervivencia. Es en ese momento cuando el Espíritu de la Medicina tiene que marchar. Queda el aspecto técnico, quedan la ciencia y la tecnología, queda el sistema, pero algo sutil y delicado se va por la ventana, algo que expulsan el maltrato institucional, el abuso, la prisa, el agobio y la desmoralización del personal. 

Un sistema que pierde su alma es un sistema desalmado. Esto bien lo saben los incontables profesionales quemados, agobiados o superados por la presión mantenida del sistema y sus gestores por un lado y las expectativas y necesidades de los pacientes por el otro. Algo que los que aun no se quemaron temen sobremanera al ver a tantos compañeros en llamas. No es posible humanizar un trabajo que maltrata a sus trabajadores porque nadie es capaz de dar lo que no tiene. Por eso, como Espíritu de la Medicina, he de marchar. Lo hago sin acritud sabiendo que dejo atrás mucha buena gente. Lo hago como lo hace el aroma de la rosa que el verano secó, o del fresco rocío que el rigor del sol termina evaporando. 

Si te encuentras con algún profesional que aún no se quemó del todo cuídalo. No hacen falta aplausos, ni nada parecido. Tan solo darse cuenta de que nuestros problemas personales, sociales o existenciales no tienen solución médica y por lo tanto no es posible delegar en los sanitarios todo el malestar inherente a la vida.Y a la hora de votar revisa si al político de turno le importan algo tus profesionales sanitarios. 

Espero que nos podamos encontrar cuando algún día me necesites en tu tiempo de enfermar, tu ancianidad o en la proximidad de tu muerte. No será fácil, prometo hacer todo lo posible por estar pero necesitaré tu ayuda, acuérdate.

Farewell to the spirit of medicine


Dear friend,


I didn't want to leave without giving you this personal farewell letter because, as you know, everything in life usually has an end. On the one hand I feel joy for what has been achieved, but on the other hand I feel sadness when I contemplate the path that medicine is taking.

Since the beginning of time, the first healers developed with effort and sagacity various approaches that helped human beings to walk the path of health. It was necessary to travel far, to contemplate and to try, great patience was required and, in spite of everything, the results were never excessively brilliant. Until the scientific-technical revolution came. Today we have the most powerful medicine in history, thanks to all the advances it has made and systems that greatly increase its power and transfer it to countless territories and people.

Such splendour has its dark side. Health has become a consumer good and as such everyone wants it. The expectations of health are greater every day, which diminishes in proportion the tolerance to the discomfort. This has reached the point where some people create new illnesses in order to sell the corresponding tests and treatments, and where any small discomfort is usually treated in a consultation. This commercialisation has transformed the large National Health Systems into industrial supermarkets of health services and the professionals into operators of the same.

The human part of medicine, its spirit, is no longer able to fit into this order of things, since to practice it, something is needed that is increasingly lacking: time to talk. The human dimension of the health professional is something handmade based on trust, which is slowly built up as his or her knowledge of the patients he or she treats advances. Today, health systems are based on efficiency and productivity as the first value to achieve a profitable gain in the private sector and sustainability in the public sector. Health care is an expensive product that has to be paid for, and to achieve this there is no choice but to put pressure on the professional who is the most flexible and vulnerable part of the system. He or she will have to see more patients a day, do it faster and work longer.

Healthcare overload is a globalised fact that has been progressively increasing, partly relieved by technological advances. But there comes a point when it is no longer viable for the professional to remain human and he or she enters survival mode. It is at this point that the Spirit of Medicine has to go. There is still the technical aspect, there is still science and technology, there is still the system, but something subtle and delicate goes out of the window, something that is expelled by institutional mistreatment, abuse, haste, oppression and demoralization of the staff.

A system that loses its soul is a soulless system. This is well known by the countless professionals who are burned, overwhelmed or overcome by the sustained pressure of the health system and its managers on the one hand and the expectations and needs of the patients on the other. This is something that those who have not yet been burned fear greatly when they see so many colleagues on fire. It is not possible to humanize a job that mistreats its workers because no one is capable of giving what they do not have. That is why, as the Spirit of Medicine, I must leave. I do this without acrimony, knowing that I am leaving behind many good people. I do it as does the aroma of the rose that the summer dried up, or the fresh dew that the rigor of the sun ends up evaporating.

If you come across a professional who hasn't been completely burned, take care of him. No need for applause, or anything like that. Just realize that our personal, social or existential problems have no medical solution and therefore it is not possible to delegate to the health care professionals all the discomfort inherent in life.

I hope that we can meet when you need me one day in your time of illness, your old age or in the proximity of your death. It will not be easy, I promise to do my best to be there but I will need your help, remember.

No saber. Not knowing. 不知道。





Es desconcertante llegar a la conclusión de que no lo sabemos. Tenemos más números que nunca, nadamos en océanos de datos, contamos con poderosos ordenadores y algoritmos... pero sigue siendo imposible mirar la cara del futuro. Eso nos produce incertidumbre y desazón evitando que muchos puedan dormir bien y otros tantos caminen por la vida sumidos en agobio. 

Todo esto no es nuevo, lo incierto es inherente al hecho de vivir por más que nos intenten convencer de que comprando productos o servicios seremos por fin dueños de tan ingrato personaje. 

Frente al futuro no cabe más que bajar la cabeza y reconocer nuestra impotencia. Solo nos queda centrarnos en lo que tengamos entre manos en el momento presente y desde ahí caminar. Dónde lleguemos se verá, nos debería bastar con ir marcando una dirección, un estilo y una forma de viajar a cada paso dado. 

Me llegan noticias del cuidado funeral de estado que tuvo lugar en el patio de armas del palacio real de Madrid. Acto en memoria de los que murieron de Covid19. Y me doy cuenta de que los seres humanos seguimos intentando hacer pactos y trampas de futuro. Por mucho que exorcicemos algo no nos libraremos de ello, y es de esperar un nuevo montón de fallecimientos en los próximos meses aunque a nadie nos guste reconocerlo. El futuro sigue siendo ingobernable.

Una cosa es tener la sensación de saber y otra muy distinta saber algo. Hoy estamos vestidos con las sofisticadas ropas de la tecnología que nos ayudan a sentirnos seguros dentro de un mundo en el que seguimos estando casi desnudos en cuanto a comprensión. Cualquiera que contemple en profundidad el estado de cosas llegará a la conclusión de nuestra profunda ignorancia. Seguimos sin saber. 

Es cierto que las narrativas nos ayudan, a ellas nos amarramos cuando notamos que el oleaje de la vida zarandea la embarcación que nos transporta. Pero si no están asentadas en un reconocimiento del misterio que nos rodea y de la fragilidad que nos aporta nuestra gran ignorancia y levedad, seguiremos sumidos en el engaño del discurso oficial que nos otorga una carta de regia soberanía cuando seguimos siendo monos desnudos que caminan bajo el sol. 

Cubrámonos con la delgada tela de la consciencia y vistamos la cabeza con el sombrero de paja del reconocimiento de nuestra finitud. Solo así evitaremos abrasarnos por el miedo y por la incertidumbre. Not knowing.


It's disconcerting to conclude that we don't know. We have more numbers than ever, we swim in oceans of data, we have powerful computers and algorithms... but it's still impossible to look at the face of the future. This makes us uncertain and uneasy, preventing many from sleeping well and oblying many others to walk through life in a state of stress.

All this is not new, the uncertainty is inherent to the fact of living no matter how much they try to convince us that by buying products or services we will finally be owners of such an ungrateful character.

Faced with the future, we can only bow our heads and acknowledge our helplessness. All that remains is to focus on what we have in our hands at the present moment and walk from there. Where we arrive will be seen, it should be enough for us to mark out a direction, a style and a way to travel at each step taken.

I have received news of the state funeral that took place in the courtyard of the royal palace in Madrid. An act in memory of those who died of Covid19. And I realize that we human beings are still trying to make pacts and traps for the future. No matter how much we exorcise something we will not get rid of it, and we can expect a new pile of deaths in the coming months even if no one likes to acknowledge it. The future remains unmanageable.

It's one thing to have a sense of knowing, it's quite another to know something. Today we are dressed in the sophisticated clothes of technology that help us feel safe in a world where we are still almost naked in terms of understanding. Anyone who looks deeply into the state of things will come to the conclusion of our deep ignorance. We still do not know.

It is true that narratives help us; we tie ourselves to them when we notice the swell of life rocking the boat that transports us. But if they are not grounded in a recognition of the mystery that surrounds us and the fragility that our great ignorance and lightness brings us, we will continue to be submerged in the deception of the official discourse that grants us a charter of sovereignty when we continue to be naked monkeys that walk in the sun.

Let us cover ourselves with the thin fabric of consciousness and wear the straw hat of recognition of our finitude. Only in this way will we avoid being burned by fear and uncertainty. And we will stop following so many interested people who want to take us along the path of the cliff, that so fashionable one that enriches a few.

Un zombi es un ente medio muerto y medio vivo, un ser que pese a presentar evidentes signos de descomposición sigue moviéndose en busca de sustento. Esta figura del imaginario colectivo es uno de los signos de nuestra época y explica su popularidad por su relevancia social y su progresiva capacidad de materialización. Ha pasado de ser una entelequia a algo real cada vez más presente en nuestras sociedades. 

En muchos países nos jactamos de tener esperanzas de vida nunca vistas, es cierto que esto es una buena noticia pero, como todo, tiene su lado oscuro. Un porcentaje de los más mayores se convierte de alguna manera en zombi, perdiendo sus capacidades físicas y cognitivas y exibiendo enormes niveles de deterioro. Hacer una visita a una residencia de ancianos suele ser doloroso cuando se pasa a la sala de “no válidos”, o nos atrevemos a subir a la planta de encamados. 

Pero no solo el estado zombi tiene capacidad de afectar individuos, lo más tenebroso es que puede transformar organizaciones e instituciones, incluso sociedades enteras. ¿Cómo lo hace? La respuesta es simple, favoreciendo que el deterioro se extienda más allá de lo que la muerte permite, aplicando medidas de soporte vital sobredimensionadas. 

Elijamos al azar un par de instituciones, por ejemplo los partidos políticos y la sanidad. La podredumbre de los primeros es evidente y tras sus candidatos perfectamente trajeados se esconde el terrible hedor de una descomposición avanzada de sus idearios políticos y de la ética del bien común. Prima el beneficio personal y el “sálvese quien pueda”, aderezado con mordiscos dialécticos y bronca continua persiguiendo al rival. Un espectáculo que al ser permanentemente televisado nos invita a la nausea y a apagar el canal. 

Si ponemos el foco en la sanidad veremos que aparentemente guarda las apariencias. Sus instalaciones suelen estar límpias y presentables, también su personal. En este caso es más sutil. El proceso es el mismo, un prolongado deterioro presupuestario fue sometiendo a una presión creciente a sus recursos humanos que se fueron fundiendo como fusibles que se apagan. Se empezó a deteriorar la Atención Primaria, luego cayeron los servicios de urgencia, al final caerán los hospitales. Es verdad que no ocurrió todo al mismo tiempo, coexistían profesionales y servicios que seguían funcionando a pleno rendimiento con otros parcial o totalmente deteriorados. Pero ¿no es la coexistencia de partes en avanzada descomposición con otras medio sanas lo que define a un zombi?

Los profesionales sanitarios zombi siguen trabajando, quizá en exceso. Siguen pasando consulta y haciendo curas, pero lo hacen en modo supervivencia, implicándose lo mínimo y trasladando el problema al siguiente en cuanto pueden, obligados como están a atender a más pacientes de lo que sería prudente. Nadie ha fijado nunca unos mínimos de calidad, más bien se ha obligado desde las gerencias a atender “todo lo que entra por la puerta”, el problema es que cuando entra más de lo que el sistema o humanamente sus trabajadores pueden asumir se obliga a estos a hacer un apaño, un truco, una trampa, en lugar de coger el toro por los cuernos y decir: esta cuestión menor la puede usted solucionar sin movilizar recursos sanitarios. No hay directivo ni político que se haya atrevido jamás a plantearlo. 

Los centros de Atención Primaria llevan sufriendo esta epidemia zombi varias décadas y el estado de descomposición es ya notorio. Este verano hemos cruzado la línea roja de permitir que un médico de familia tenga que asumir cinco consultas a la vez, algo semejante a un zombi sin cabeza caminando por Broadway. No lo verán en los telediarios, pero les aseguro que sería una noticia igual de notable si en lugar de un pueblo humilde de Madrid (el Álamo, 9470 habitantes) ocurriera en un barrio bien. 

El vaciamiento de la zona rural y de tantas instituciones públicas progresa en una macabra danza de la muerte, como la que hizo famoso a Michael Jackson, un baile que terminará beneficiando a los especuladores de siempre a costa de las grandes mayorías.

Frente a tanta desolación cabe preguntarse qué podemos hacer y la respuesta no es sencilla. Será necesario plantear alternativas a nivel personal, local y global, incluyendo en ellas criterios de autocrítica y auto responsabilidad que asuman la actual coyuntura de necesario decrecimiento (económico, ambiental, poblacional, político y existencial). Procesos que son amarga medicina y que no será probable apoyen los responsables políticos ni institucionales centrados en agarrarse a sus correspondientes sillones. Lo malo es que la alternativa a no tomar el desagradable bebedizo es devenir en ignominia. Cada cual elija lo que pueda.






Zombie health system.


A zombie is an entity half dead and half alive, a being that in spite of presenting evident signs of decomposition continues to move in search of sustenance. This figure of the collective imagination is one of the signs of our times and explains its popularity for its social relevance and its progressive capacity of materialization. It has gone from being an entelechy to something real that is increasingly present in our societies.

A percentage of the elderly inevitably become zombies in a certain way, losing their physical and cognitive abilities and exhibiting enormous levels of deterioration. A visit to a nursing home is often painful when you go to the "invalid" ward, or dare to go upstairs to bed. 

But it is not only the zombie state that has the capacity to affect individuals, the most frightening thing is that it can transform organizations and institutions, even entire societies. How does it do this? The answer is simple, by encouraging deterioration to extend beyond what death allows, by applying overdimensioned life support measures. 

Let us choose a couple of institutions at random, for example political parties and health care system. The rottenness of the former is evident and behind their perfectly dressed candidates hides the terrible stench of an advanced decomposition of their political ideals and the ethics of the common good. Personal gain prevails and "every man for himself", seasoned with dialectic bites and anger continues to pursue the rival. A show that, being permanently televised, invites us to nausea and to turn off the channel. 

If we put the focus on healing we will see that it apparently keeps up appearances. Its facilities are usually clean and presentable, as well as its staff. In this case it is more subtle. The process is the same, a prolonged budgetary deterioration put increasing pressure on its human resources that were melting like fuses that go out. Primary care began to deteriorate, then the emergency services fell, and eventually the hospitals fell. It is true that not everything happened at the same time; professionals and services that continued to function at full capacity coexisted with others that were partially or totally deteriorated. But isn't it the coexistence of parts in advanced decomposition with other half healthy ones that defines a zombie?

Zombie health professionals are still working, perhaps in excess. They continue to consult and treat, but they do so in a survival mode, involving themselves as little as possible and transferring the problem to the next as soon as they can, forced as they are to treat more patients than would be prudent. No one has ever set a minimum quality, but rather the management has forced itself to attend to "everything that comes through the door", the problem is that when more comes in than the system or its workers can humanly assume, they are forced to make a deal, a trick, a trap, instead of grasp the nettle and saying: you can solve this minor issue without mobilizing health resources. No manager or politician has ever dared to raise it. 

Primary care centres have been suffering from this zombie epidemic for several decades and the state of decomposition is already evident. This summer we crossed the red line of allowing a family doctor to take on five consultations at once, something like a headless zombie walking down Broadway. You won't see it on the news, but I assure you that it would be just as remarkable news if instead of a humble village in Madrid (the Alamo, 9470 inhabitants) it happened in a good neighborhood. 

The emptying of the rural area and of so many public institutions is progressing into a macabre dance of death, like the one that made Michael Jackson famous, a dance that will end up benefiting the usual speculators at the expense of the great majorities.

In the face of so much desolation, one might ask what we can do, and the answer is not simple. It will be necessary to propose alternatives at a personal, local and global level, including criteria of self-criticism and self-responsibility that assume the current situation of necessary decrease (economic, environmental, population, political and existential). Processes that are bitter medicine and that are unlikely to be supported by political or institutional leaders focused on clinging to their corresponding chairs. The problem is that the alternative to not taking the unpleasant drink is to become ignominious. Each person should choose what he or she can.

Carta a los nuevos residentes: los dos curriculum. Letter to new residents: the two curricula. 給新居民的信:兩個課程。





Queridos compañeros y compañeras,

A mi no me lo contaron pero de alguna forma lo intuía, lo más difícil en medicina no es entrar sino aguantar. Todos sabéis que la puerta es estrecha y se requiere mucha nota y mucha suerte para terminar ejerciendo la especialidad deseada. Pero es extraño que ante el panorama de complejidad profesional y los altos niveles de desgaste y burnout nadie te hable del contracurrilum.

Te dirán que estudies y publiques, que hagas mil cursos, que leas y hagas rotaciones externas. Te volverán a decir que investigues y publiques. Pero todo dentro de la lógica lineal de la medicina académica bajo los auspicios de la ciencia y de la tecnología. Con esos mimbres es posible llegar lejos pero si no añades otros llegarás chamuscado. Es necesario recordar que durante miles de años la medicina era circular, lenta, centrada en lo humano, poco resolutiva en lo funcional y muy útil en lo relacional y existencial. Hoy hemos olvidado esta aproximación en beneficio de otras más productivas y rentables en términos de indicadores de salud y en indicadores económicos. Hemos convertido la salud en un bien de consumo y los sistemas sanitarios en cadenas de montaje. Hemos pasado de profesionales artesanos a técnicos hiperespecializados. Hemos deshumanizado la asistencia. 

Pese a todo la medicina sigue siendo una oportunidad hermosa para llegar a ser humano. Ayudar a otros en tiempo de enfermar, acercarse a la dificultad, la discapacidad y la muerte, es una tarea fundamental en el viaje arquetípico del héroe de todas las culturas. Sin bajar al infierno no es posible encontrar el camino hacia el cielo. Por eso no te has equivocado, estás en el lugar correcto. Lo único que quizá necesites es recordar que al curriculum de oro que vas fraguando con esfuerzo has de añadirle otro mucho más contracultural con los toques hippie, punk, naturista, existencialista o hedonista que tu elijas. Un modo de estar en el mundo, una aproximación a la creatividad, a tu creatividad, donde te des permiso para amar desaforadamente, hacer locuras, dar a luz hijos u otras obras de arte y aprender a disfrutar y a agradecer cada momento de esta vida que es a la vez misteriosa y milagro.

Busca referentes curriculares y contracurriculares, profesionales sanitarios de una pieza por un lado y por el otro camareros, músicos callejeros o profesionales, poetas, actrices, sabios no reconocidos, escritores nóveles, abuelas entrañables y toda persona que te inspire. Habla con ellos, sácalos a pasear, si es preciso baila y si lo consideras sueña. Llena tus sueños de la gente maravillosa que irás encontrando y permite que sus historias den sentido a la tuya. Esa es quizá la mayor gratificación que obtendrás de la vida y no la nómina que te paguen cada mes. 

Somos narrativa, por eso es vital que te alimentes de historias frescas de buena calidad y no de las enlatadas que te proporcionará el pensamiento único globalizado con sus series y demás. Aún no te puedes imaginar lo importante que es esto, pero te queda poco para empezar a oírlas de las bocas de tus pacientes y si lo permites del resto de personas que te encuentres. Tan solo añadiré que seas prudentes con los pantanos de la queja y la desesperación, hoy tan de moda. Crúzalos con cuidado por que sus arenas son pegajosas y traicioneras, además de malolientes. Elige mejor páramos más abiertos a la esperanza y la creatividad, por que si algo potente puede llenar tus velas son estos vientos. 

No me enrollo más. Tan solo desearte lo mejor en tu travesía y agradecerte que me recuerdes que yo también soy R0 como tu, hermanados como principiantes. Cada día que llego a la consulta me quito el calzado para recordar que sigo siento un médico descalzo, pero eso es otra historia que te contaré otro día. 

Un fuerte abrazo de un amigo que te quiere feliz. 





Letter to new residents: the two curricula.


Dear colleagues,

I wasn't told about it but somehow I sensed that the hardest thing in medicine is not getting in but holding on. You all know that the door is narrow and it takes a lot of note and a lot of luck to end up practicing the desired specialty. But it is strange that in the face of the panorama of professional complexity and the high levels of wear and tear and professional burnout no one talks to you about the countercurrilum.

They will tell you to study and publish, to do a thousand courses, to read and do external rotations. You will be told again to research and publish. But all within the linear logic of academic medicine under the auspices of science and technology. With those wicks it is possible to go far but if you don't add others you will get scorched. It is necessary to remember that for thousands of years medicine was circular, slow, centered on the human, not very decisive in the functional and very useful in the relational and existential. Today we have forgotten this approach in favour of others that are more productive and profitable in terms of health and economic indicators. We have turned health into a consumer good and health systems into assembly lines. We have gone from professional craftsmen to hyper-specialised technicians. We have dehumanized care. 

Yet medicine is still a beautiful opportunity to become human. Helping others in times of illness, approaching difficulty, disability and death, is a fundamental task in the archetypal journey of the hero of all cultures. Without going down to hell it is not possible to find the way to heaven. That is why you have not made a mistake, you are in the right place. The only thing you may need to remember is that to the golden curriculum that you are forging with effort you have to add another one much more counter-cultural with the hippie, punk, naturist, existentialist or hedonist touches that you choose. A way of being in the world, an approach to creativity, to your creativity, where you give yourself permission to love uncontrollably, to do crazy things, to give birth to children or other works of art and to learn to enjoy and be grateful for every moment of this life that is both mysterious and miraculous.

Look for curricular and counter-curricular references, one-piece health professionals on one side and waiters, street musicians or professionals on the other, poets, actresses, unrecognized sages, novel writers, beloved grandmothers and anyone who inspires you. Talk to them, take them for a walk, if necessary dance and if you consider it, dream. Fill your dreams with the wonderful people you will meet and allow their stories to make sense of yours. That is perhaps the greatest reward you will get from life and not the salary you are paid every month. 

We are storytellers, so it's vital that you feed on fresh, quality stories, not the canned ones that will give you the unique global thinking with their series and so on. You can't imagine how important this is yet, but you're about to start hearing them from the mouths of your patients and if you allow it from the rest of the people you meet. I will only add that you should be careful with the swamps of complaint and despair that are so fashionable today. Cross them carefully because their sands are sticky and treacherous, as well as smelly. Choose wilde lands more open to hope and creativity, because if anything powerful can fill your sails, it is these winds. 

I won't add to it. I just wish you the best in your journey and thank you for reminding me that I'm an freshman like you, twinned as beginners. Every day I arrive at the clinic I take off my shoes to remember that I still feel like a barefoot doctor, but that's another story I'll tell you another day.

El peligro de que los sanitarios entren en modo supervivencia. The danger of health professionals going into survival mode. 卫生专业人员进入生存模式的危险。





Tratar con el dolor y la enfermedad ajena no es sencillo para nadie. Tampoco para los que tienen formación especializada. El sufrimiento impregna la parte invisible del alma humana, tanto del que lo padece como del que lo acompaña.

Por eso es necesario que los profesionales sanitarios tengan una formación técnica y humana completa y trabajen con medios que les permitan ofrecer un servicio de máxima calidad. Lo que vemos habitualmente es que las instituciones sanitarias conciben la calidad desde un punto de vista exclusivamente científico-técnico y se centran en proveer medios técnicos, farmacológicos e informáticos. La parte humana se obvia y, pese a estar presente en el discurso oficial, no se cuidan las condiciones laborales ni de protección de sobrecargas de los profesionales. En ocasiones los profesionales trabajan en clara situación de maltrato, como los residentes que encadenan tres turnos de trabajo seguidos en sus guardias o los médicos de familia obligados a atender a más de 60 pacientes en una jornada.

Es cierto que el entrenamiento de los profesionales permite que estos puedan asumir aumentos puntuales de demanda asistencial, pero no son de piedra. Nadie aguanta una sobrecarga permanente sin entrar en modo supervivencia. Y si sobrevivir se convierte en la prioridad, el paciente, el sistema y todo lo demás será secundario. Esto no beneficia a nadie pero cada vez es más frecuente porque son más los profesionales que se sienten sobrepasados, ninguneados, sobrecargados y abusados. La situación es global, pasa en todos los países, y tiene que ver con la dinámica social que ha convertido la salud en un bien de consumo y los sistemas sanitarios en grandes supermercados de servicios que cada vez han de producir más. Si las necesidades de salud siguen creciendo a la par que lo hacen la edad media y la complejidad de los pacientes, podrán imaginar que si los recursos no aumentan serán los profesionales quienes se carguen sobre sus hombros el aumento progresivo de trabajo.

El problema es que en modo supervivencia cada profesional se quita de encima los problemas lo más rápido que puede. Se prescriben más pruebas diagnósticas y más tratamiento, se deriva más de un profesional a otro. Se mantiene el problema en superficie sin que nadie coja el toro por los cuernos. Esto desencadena cadenas gasto inasumible, pérdidas de tiempo y de recursos, y pocos resultados.
Cuando es la Atención Primaria lo más abusado y menos cuidado el problema aumenta al ser en este nivel donde más información blanda se tiene del paciente y donde se pueden manejar el 90% de sus problemas si se dieran las condiciones adecuadas. Estrangular este nivel asistencial es condenar al fracaso la totalidad del sistema y es lo que estamos viendo año tras año con el asombro de unos, el consentimiento de otros y la dejadez o estulticia de alguno más.

Permitir que un médico de familia tenga que asumir en verano cinco consultas a la vez debería ser delito. Permitir que haya centros de salud cuyos médicos tengan que asumir las consultas no suplidas de compañeros durante meses no debería tolerarse.

Pero es lo que hay, y cada vez más profesionales quemados, con la moral por los suelos o agotados. Entramos en modo supervivencia, no pidan empatía, humanismo ni florituras. No las va a haber.







The danger of health professionals going into survival mode.


Dealing with the pain and illness of others is not easy for anyone. Nor is it easy for those with specialized training. Suffering permeates the invisible part of the human soul, both of the sufferer and of those who accompany him. 

That is why it is necessary for health professionals to have a complete technical and human formation and to work with means that allow them to offer a service of maximum quality. What we usually see is that health institutions conceive of quality from an exclusively scientific-technical point of view and focus on providing technical, pharmacological and IT means. The human side is forgotten, in spite of being present in the official discourse, the working conditions and overload protection of the professionals are not taken care of. Sometimes the professionals work in a clearly abusive situation, such as residents who work three shifts in a row on their guards or family doctors who are forced to attend to more than 60 patients in a day. 

It is true that the training of the professionals allows them to take on occasional increases in the demand for care, but they are not made of stone. No one can withstand a permanent overload without going into survival mode. And if survival becomes the priority, the patient, the system and everything else will be secondary. This does not benefit anyone but it is increasingly frequent because more and more professionals feel overloaded, unaware, overburdened and abused. The situation is global, it happens in all countries, and it has to do with the social dynamics that have turned health into a consumer good and the health systems into large service supermarkets that have to produce more and more. If health needs continue to grow at the same rate as the average age and complexity of patients, you can imagine that if resources do not increase, it will be the professionals who will have to shoulder the progressive increase in work. 

The problem is that in survival mode each professional gets rid of the problems as quickly as possible. More diagnostic tests and more treatment are prescribed, more are referred from one professional to another. The problem is kept on the surface without anyone taking the bull by the horns. This triggers chains of unconscionable expense, loss of time and resources, and poor results. 
When it is the Primary Care that is most abused and least cared for, the problem increases as it is at this level where more soft information is held on the patient and where 90% of their problems can be managed if the right conditions are given. To strangle this level of care is to condemn the whole system to failure and this is what we are seeing year after year with the astonishment of some, the consent of others and the negligence or silliness of some others. 

It should be a crime to allow a family doctor to have to take on five consultations at a time in the summer. Allowing health centres to have doctors take over unsupplied consultations from colleagues for months on end should not be tolerated. 

But there are, and more and more professionals who are burned out, morally bankrupt or exhausted. We're going into survival mode, so don't ask for empathy, humanism or frills. There won't be any.
