Who will lead the health care? ¿Quién liderará la sanidad? 誰來領導醫療保健?





Who will lead the health care?

Many of us have been wondering for a long time if we will ever see someone who will adequately lead the health sistema. And the tricky thing is to get to know what the hell means in a proper way. For some, it's keeping costs down and maintaining the freedom to choose a private service for whomever they want. For others, it is that the service should be forever universal and free. The consensus is similar a Gordian knot. 

Therefore, there will be no leaders to suit everyone. Nor will we be able to reach an agreement if we define them as defenders of the common good.

I expect nothing from the management structures. I have been on the NHS boat for many years now and I have heard only good words and seen very few gestures. Nor do I expect anything from professional organisations, which are dense, dusty and unfriendly to change and innovation. 

I hope that if there is a value proposition, it will come from the health professionals, as we have seen in this pandemic in which health managers and organisations have lagged behind the care providers by weeks. 

The problem is that with the current level of overload, professionals are focused on a single priority: survival. And this, I am afraid, does not benefit anyone, not society, which is aware that its health care facilities are burnt out and on the verge of exhaustion, not the managers who see that it is impossible to make changes, not the politicians who, despite announcing new hospitals, see the system crumble. 

What strength is left for a family doctor when at the end of her day she leaves exhausted because she has seen her patients, those of her colleague who is on leave without a substitute, those without an appointment, the home visitsand the telephone calls? What energy is left for the ICU nurse with the units full? What encouragement do I give to those who support the emergencies and see how more and more people are coming?

Cannon fodder. It's sad to realize that we all are. We're gonna need to hit rock bottom to make the decision to step up. Swarming societies may not help, but that does not mean that all doors will be closed to us. Everyone can make a revolution at the edges of their own inner world. That is where the future lies. If we manage to make contact with ourselves and see that we need very little to survive and that the potential for human growth that is open to us is almost infinite, we may have a chance. 






¿Quién liderará la sanidad?

Muchos llevamos tiempo preguntándonos si llegaremos a ver a alguien que lidere de una manera adecuada la sanidad. Y lo complicado es llegar a saber qué diablos es eso de una manera adecuada. Para unos es que los costes se mantengan contenidos y mantener la libertad de elegir un servicio privado para el que quiera. Para otros que el servicio sea por siempre universal y gratuito. El consenso no es que sea difícil sino que ha devenido en nudo gordiano. 

Por tanto no habrá líderes a gusto de todos. Tampoco si los definimos como defensores del bien común, termino para el que de nuevo no nos conseguiremos poner de acuerdo.

No espero nada de las estructuras de gestión. Son ya muchos años en el barco y solo he escuchado buenas palabras y visto muy pocos gestos. Tampoco de las organizaciones profesionales, densas, polvorientas y poco amigas al cambio y la innovación. 

Abrigo la esperanza de que si hay alguna propuesta de valor esta saldrá de los profesionales como hemos visto en esta pandemia en la que las gerencias y organizaciones sanitarias han ido semanas por detrás que los asistenciales. 

El problema es que con el nivel actual de sobrecarga los profesionales se centran en una única prioridad: sobrevivir. Y esto me temo no beneficia a nadie, ni a la sociedad que consiente que sus sanitarios estén quemados y al borde de la extenuación, ni a los gestores que ven que es imposible hacer cambios, ni a los políticos que pese a anunciar nuevos hospitales ven como el sistema se desmorona. 

¿Qué fuerza le queda a una médica de familia cuando al terminar su jornada sale agotada por haber visto sus pacientes, los de la compañera que está de baja sin suplente, los sin cita, los domicilios y las llamadas telefónicas? ¿Qué energía le queda a la enfermera de UCI con las unidades llenas? ¿Qué animo a los que sostienen las urgencias y ven como cada vez acude más gente?

Carne de cañón. Es triste darse cuenta de que todos lo somos. Vamos a necesitar tocar fondo para tomar la decisión de dar un paso al frente. Las sociedades convertidas en enjambre quizá no ayuden pero no por ello se nos cerrarán todas las puertas. Cada cual puede llevar a cabo una revolución en los límites de su mundo interno. Será ahí donde nos juguemos el futuro. Si conseguimos tomar contacto con nosotros mismos y comprobar que para subsistir hace falta bien poco y que las potencialidades de crecimiento humano que se nos abren son casi infinitas, tal vez tengamos alguna posibilidad.

La Atención Primaria ante la nueva normalidad, ¿alguna vez fue normal? Primary Care in the face of the new normality, was it ever normal? 面對新的常態,初級保健曾經是正常的嗎?





La sanidad española ha ido experimentado modificaciones con el tiempo que han tendido a reforzar con tecnología, financiación y recursos humanos los hospitales desatendiendo los servicios de Atención Primaria, Salud Pública y Salud Mental comunitaria.

Han pasado muchos años desde la reforma de la Atención Primaria de 1984. Y largos los ríos de tinta aportando ideas y pidiendo ajustes y mejoras. Mucho ruido y pocas nueces por cierto. 

Los hechos nos dicen que el cambio de modelo no llegó a cuajar del todo sobre todo por falta de apoyo político y financiación pero también por la inercia del sistema, la impotencia o dejadez de los profesionales y el sobreuso de la misma por parte de la ciudadanía. 

Pese a los esfuerzos de un gran número de profesionales abnegados nuestra Atención Primaria nunca fue normal del todo, más bien algo sub-normal, si me permiten la ironía. Aunque algunos centros de salud de gran tamaño han podido desarrollar su labor con ciertas garantías otros muchos, así como un gran porcentaje de consultorios locales, han sufrido un permanente estado de penuria y sobrecarga de larga duración.

¿Qué esperar para el futuro cercano de nueva normalidad?

Pues me temo que mucho deberían cambiar las cosas para dejar atrás la subnormalidad previa y avanzar hacia algo mejor. Si nada lo remedia iremos otra vez para atrás con una crisis económica que hará difícil conseguir una financiación sanitaria que nos iguale a los países de nuestro entorno. 

Y en cuanto a los cambios y ajustes que no requieren grandes presupuestos (minimizar burocracia, mejorar la coordinación entre centros y niveles, sistemas de información, historia clínica electrónica mejorada, autonomía de gestión de los centros, etc...) me gustaría ser optimista, dejémoslo ahí. 






Primary Care in the face of the new normality, was it ever normal?

Spanish health care has undergone changes over time that have tended to reinforce hospitals with technology, funding and human resources, neglecting Primary Care, Public Health and Community Mental Health services.


Many years have passed since the reform of Primary Health Care in 1984. And long rivers of ink have been pouring in with ideas and calls for adjustments and improvements. Much ado about nothing, by the way. 


The facts tell us that the change of model did not fully take hold, above all due to a lack of political support and funding, but also due to the inertia of the system, the impotence or neglect of the professionals and the overuse of the system by the citizens. 


Despite the efforts of a large number of dedicated professionals, our Primary Care was never completely normal, rather something sub-normal, if I may be ironic. Although some large local health centres have been able to carry out their work with certain guarantees, many others, as well as a large percentage of small rural clinics, have suffered a permanent state of hardship and long-term overload.


What to expect for the near future of new normality?


Well, I am afraid that a lot should change in order to leave behind the previous subnormality and move towards something better. If nothing remedies it, we will go back again with an economic crisis that will make it difficult to obtain health financing that will put us on a par with the countries around us. 


And as for the changes and adjustments that do not require large budgets (minimizing bureaucracy, improving coordination between centres and levels, information systems, improved electronic medical records, management autonomy of centres, etc...) I would like to be optimistic, let's leave it at that.

No quiero ser ni héroe ni heroína. I don't want to be a hero or a heroine. 我不想當英雄或女英雄。




No nos llamemos a engaño. Los profesionales sanitarios somos ciudadanos de a pie que trabajan para las personas de su comunidad. Trabajadores con un grado de precariedad laboral y sobrecarga similar a otros con el único agravante quizá de la penosidad que implica relacionarnos permanentemente con la enfermedad, sufrimiento y dolor de los demás. Algo que les puedo decir salpica el alma y la tiñe de oscuro. 

Pero no somos héroes ni heroínas, por más que haya compañeras y compañeros fuera de serie cuyo valor, audacia y méritos sobresalen con mucho. 

Estos días ha aparecido un dibujo de Bansky en un hospital de Southampton donde se aprecia a un niño que ha cambiado sus muñecos de Batman y Superman por otro de superheroína enfermera. Me da que pensar. Si se fijan bien los muñecos usados están en una papelera, que es donde probablemente acabemos los sanitarios después de esta crisis cuando la sociedad se aburra de nosotros. 

Es verdad que hemos sufrido contagios y muertes en mayor proporción que el resto por haber carecido de suficientes equipos de protección en tiempo y forma y que nos hemos enfrentado a una crisis extraordinaria sin un liderazgo a la altura de las circunstancias. Lo hemos pasado mal, lo estamos pasando mal.

Pero no somos héroes ni heroínas. Somos personas que tan solo pedimos un poco de respeto, medios para poder desarrollar un trabajo digno y la ayuda de la sociedad para hacer un uso prudente del Sistema Sanitario.

No pedimos el cielo, tan solo el suficiente sentido común para que las instituciones sanitarias no terminen ardiendo ante una sobrecarga continua que agota a los que la sostienen.





I don't want to be a hero or a heroine.


Don't be fooled. We health care professionals are ordinary citizens working for the people of our community. Workers with a degree of job insecurity and task overload similar to others with the only aggravation perhaps of the hardship involved in relating permanently to the illness, suffering and pain of others. But we are neither heroes nor heroines, even though there are outstanding companions whose courage, audacity and merits stand out by far.

But we are neither heroes nor heroines. We are people who only ask for a little respect, the material means to develop a dignified job and the help of society to make prudent use of the Health System.

We are not asking for heaven, just enough common sense so that the health institutions do not end up burning down in the face of a continuous overload that exhausts those who support them.

Los mimbres de la Atención Primaria. The wicks of Primary Health Care. 基層醫療服務。










Les cuento una historia. Consultorio local del Álamo, al sur de Madrid, cuenta con dos administrativas, tres enfermeras y un pediatra por turno, tres médicas de familia por la mañana y dos de tarde. Población de 9.470 habitantes, cinco residencias de ancianos y un nivel de problemática económico social alto. 

Lo habitual es que tenga una gran presión asistencial, casos complejos, pluripatológicos con mucha frecuentación y casi nunca se suplen las ausencias lo que condiciona una sobrecarga continua.

Las tres médicas y la pediatra de la mañana llevan 6 semanas asumiendo todo el trabajo. Seis semanas a una media de 40-50 llamadas diarias cada una, más avisos y consultas presenciales. 


Pueden imaginar el nivel de estrés. 


Esta semana les llega un refuerzo. Médico joven extra comunitario que no habla bien el idioma y en consecuencia no puede atender muchos pacientes... Duró dos días.

Estos son los mimbres ahora que están los centros de salud teóricamente vacíos. Se imaginan qué pasará cuando se llenen al 30, al 50, al 100 o al 125% (cuando la población se incremente en muchos pueblos con desplazados y veraneantes dentro de dos meses). Yo si me lo imagino y no me gusta nada lo que veo.


Si no se hace algo con los centros que están en esta situación la Atención Primaria puede explotar. Más claro no se puede decir.






El problema es que cuando caiga el PIB unos lo sentirán más que otros. En los municipios más empobrecidos habrá más carga de enfermedad y los recursos sanitarios ya ven cómo están.




The wicks of Primary Health Care. 



I present you a Primary Health Centre in the south of Madrid, six kilometres from Navalcarnero, with two administrative staff, three nurses and a paediatrician on the morning and afternoon shifts, three family doctors in the morning and two in the afternoon. Population of 9,470 inhabitants, five nursing homes and a high level of socioeconomic problems. 

The usual situation is one of high care pressure, complex, multipathological cases with high frequency and almost never make up for the absences, which conditions a continuous overload.

If something is not done, the spanish Primary Health Care can explode. I can't say it any more clearly.

Apuntes de gestión de centros de salud


No es lo mismo navegar sin viento que con marejada, no es lo mismo gestionar una unidad sanitaria en tiempo de calma que en pandemia. Pero es útil aprender de las tormentas porque como bien sabe el marino, nunca vienen solas. 

Los sistemas sanitarios públicos europeos son enormes estructuras que podríamos asemejar a una flota con los hospitales como enormes naves y los centros de salud como barcos más pequeños y ágiles. En estos días de desbarajuste con una lluvia de protocolos desde la comandancia general pero sin una visión clara de a qué nos estábamos enfrentado, cada unidad hizo lo que pudo. Algunas maniobraron rápido otras no tanto, unas lo hicieron con destreza, otras fueron sobrepasadas por las circunstancias. 

En mi caso fui testigo de un buen ejemplo de autogestión y trabajo en equipo que logró adelantarse y elegir cursos de acción que resultaron adecuados. Si hubiésemos esperados a las consignas oficiales sin duda hubiéramos tenido más bajas entre la población y los propios sanitarios. 

Sin embargo el sistema sanitario no permite la autogestión de los centros de salud, sometidos a un rígido funcionamiento subrogado a una gerencia que trabaja en la distancia ateniéndose a indicadores cuantitativos y mediciones de resultados pero que sigue sin valorar cualitativamente y en consecuencia desarrolla una función incompleta. Si hay calma chicha el sistema funciona dado que tan solo hay que dejar que navegue a su albur, siguiendo la deriva que su inercia impone. Pero con viento o circunstancias de peligro la cosa cambia y la cadena de mando deja de ser operativa por la consiguiente lentitud y falta de agilidad.

En estas semanas poner encima de la mesa una propuesta audaz como era diagnosticar y tratar las neumonías leves en pacientes jóvenes sin comorbilidades en los centros de salud para aliviar la urgencia hospitalaria chocó inevitablemente con la imposibilidad de un sistema que no es capaz de flexibilizar su forma de hacer las cosas. Por un lado porque la coordinación con los hospitales es muy limitada y por otro porque la novedad no suele ser bienvenida por unos cuadros gestores que valoran más la seguridad del siempre se ha hecho así. Cosa comprensible por ser norma de la casa en las estructuras funcionariales del estado. 

Proponer que profesionales de baja o en cuarentena pudieran acceder remotamente a los sistemas de historia clínica electrónicamente tampoco fue considerado factible. Pese a que la proporción de bajas está siendo altísima y falta personal por todas partes. 

La única manera de innovar es no pedir permiso. Pero claro, esto limita muchísimo en profesiones tan normalizadas como las sanitarias donde es bastante complicado salirse del camino trazado. 

Un ejemplo es el uso de fotos o vídeoconferencia con pacientes dentro de las labores de telemedicina que hemos desarrollado. Muchos pacientes nos mandaban fotos de lesiones de su piel, de su garganta enrojecida, de un ojo irritado... Con otros proponíamos la videoconferencia para valorar estado general, respiración y otros factores. En todos estos casos lo hacíamos poniendo el interés y la seguridad del paciente en primer lugar pero careciendo del permiso o autorización dado que estas actividades no están contempladas en la organización sanitaria. 


Sirva esta pequeña reflexión para poner de relieve el hecho de que en un mundo cada vez más complejo que se enfrenta a un horizonte de dificultad e incertidumbre la única forma de sobrevivir es adaptándose y eso implica flexibilidad, creatividad e innovación. Los sistemas sanitarios van a ser sometidos a sobrecargas, peligros y circunstancias que van a exigir de ellos mucho más de lo que se ha exigido hasta ahora. Si mantenemos las manos de los profesionales atadas no será posible. Cuestiones como la autogestión de los centros, la optimización de la tecnología existente, la disminución de la burocracia para dotar de más tiempo y capacidad al profesional y la incentivación de la creatividad en lugar de su castigo son cursos de acción que habrá que atreverse a implementar. A la sociedad no le va a valer si en la próxima no estamos mejor preparados. 





Primary Health management notes

However, the health system does not allow the self-management of the health centres, which are subject to a rigid operation subordinated to a management that works from a distance, following quantitative indicators and measurements of results, but that still does not evaluate qualitatively and consequently develops an incomplete function. If there is a dead calm, the system works because it must be left to its own devices, following the drift imposed by its inertia. But with wind or dangerous circumstances things change quickly and the chain of command stops working because of the consequent slowness and lack of agility.

In these weeks, putting on the table a bold proposal such as diagnosing and treating mild pneumonia in young patients without comorbidities in Primary health centres to alleviate the hospital emergency inevitably came up against the impossibility of a system that is not capable of making its way of doing things more flexible. On the one hand, because coordination with hospitals is very limited and, on the other hand, because the novelty is not usually welcomed by managers who value the safety of the system more than it has always done. In all these cases we did it putting the interest and the security of the patient in the first place but lacking the permission or authorization since these activities are not contemplated in the sanitary organization. 

This small reflection serves to highlight the fact that in an increasingly complex world that faces a horizon of difficulty and uncertainty the only way to survive is to adapt and that implies flexibility, creativity and innovation. Health systems are going to be subjected to overloads, dangers and circumstances that are going to demand much more of them than has been demanded up to now. If we keep the hands of professionals tied, it will not be possible. Issues such as the self-management of centres, the optimisation of existing technology, the reduction of bureaucracy to give professionals more time and capacity and the encouragement of creativity instead of punishment are courses of action that we will have to dare to implement. Society won’t let us being not better prepared next time.

Gestionar crisis nunca ha sido fácil, sobre todo las que por su magnitud o complejidad nos sitúan en escenarios que no habíamos pisado. Lo cierto es que tenemos entre manos una pandemia que producirá devastadores efectos económicos, políticos y sociales, aunque paradójicamente no se puede considerar un riesgo mayor para cada ciudadano que utilizar su coche. Esta dificultad para separar riesgos y efectos poblacionales e individuales hacen que en paralelo a la pandemia vírica se extienda otra de igual calado basada en el miedo. 

Es el miedo social el que está condicionando el traspaso de liderazgo desde los expertos a los políticos con consecuencias que serán evidentes a toro pasado. De momento estamos en manos de dirigentes políticos que se ven en la obligación de tomar medidas. 

En el actual escenario de confinamiento nacional será imprescindible evitar que el propio sistema sanitario se convierta en foco de contagio. Esto implica vaciar en lo posible hospitales y centros de salud. Las medidas iniciales son evitar visitas a hospitales o acompañantes a centros de salud, salvo de menores o personas dependientes. La siguiente posponer operaciones y consultas. 

Lo deseable es que las próximas dos semanas los centros de salud estén lo más vacíos posible. Esto implicará un gran esfuerzo de organización para potenciar al máximo la telemedicina: asistencia telefónica, correo electrónico, mensajería instantánea y videoconferencia. Pero ¿estamos preparados para esto?

La pasada semana el centro de salud de Soto Del Real al que pertenezco consiguió tener las salas de espera vacías gracias al uso intensivo del teléfono. Se llamó a todos los pacientes citados para escuchar y valorar sus necesidades y problemas. De esta forma toda la actividad burocrática y administrativa se hizo de forma no presencial y se pudo dar información y consejos de autocuidados a una gran mayoría que de esta forma no tuvo necesidad de moverse de su domicilio. Aquellos casos que requerían una valoración presencial fueron o bien invitados a acudir al centro de salud o visitados a domicilio.

En el propio centro se creo un circuito para pacientes con problemas de aparato respiratorio y otro para el resto de cuestiones separados físicamente y gestionados por una enfermera de triaje encargada de que no se produjeran aglomeraciones ni situaciones potencialmente peligrosas. Se establecieron turnos con participación de administrativos o médicas pero liderados en todo momento por las enfermeras que de forma voluntaria asumieron esta responsabilidad.

El problema es que para hacer telemedicina salvo el teléfono no tenemos nada más. Y el teléfono de centralita del centro produce número largos que mucha gente no contesta. De momento los centros de salud no disponen de teléfono móvil que permita comunicación por mensajería instantánea y los profesionales solemos ser celosos a la hora de compartir el número personal para menesteres laborales. Eso no quiere decir que no lo usemos puntualmente o que podamos enmascarar la llamada (escribiendo #31# delante del teléfono del paciente que estemos llamando).

Lo que sí tenemos disponible en el centro es un ordenador adaptado para videoconferencia con cámara y micrófono. Faltaría implementar la cultura de organización para usarlo en aquellas situaciones que puedan requerir una teleconsulta con imagen del paciente.

Sobre mensajería instantánea tal vez se pudieran probar números de teléfono virtuales para ser usados desde ordenadores de sobremesa para este fin, pero dadas las dificultades que se han tenido para usar correo electrónico con pacientes creo que seguimos sin cultura de organización que facilite este canal. 

La telemedicina también implicaría la posibilidad de teletrabajo facilitando que el profesional sanitario pudiera acceder en remoto a su propio ordenador bien por que que esté haciendo una cuarentena, por flexibilizar horario por conciliación familiar o por otras razones. Bastaría disponer de un permiso y un software adecuado. De momento pese a ser técnicamente posible (y barato) no es contemplado por los gestores. 

Si asumimos que estamos en una situación epidémica que afectará a la mayor parte de la población con cuadros leves hemos de asumir que la mayoría de los sanitarios también se afectarán. Si el sistema sanitario no responde con flexibilidad habrá problemas. 

Por eso me parece importante hacer un esfuerzo para proveer de herramientas de telemedicina a los profesionales. Hacen falta medios de protección y recursos humanos y materiales pero también ingenio y coraje para superar inercias y problemas administrativos. 





Telemedicine, pandemics and primary health care

This difficulty in separating population and individual risks and effects means that, in parallel to the viral pandemic, another one of equal importance is spreading, based on fear. 

It is social fear that is conditioning the transfer of leadership from experts to politicians with consequences that will be evident in the future. At the moment we are in the hands of political leaders who are obliged to take action. 

In the current scenario of national confinement, it will be essential to prevent the health system itself from becoming a focus of contagion. This implies emptying hospitals and local clinics as much as possible. The initial measures are to avoid visits to hospitals or accompanying persons to health centres, except for minors or dependent persons. The following postpone operations and consultations. 

It is desirable that the next two weeks the health centres be as empty as possible. This will imply a great organizational effort to maximize telemedicine: telephone assistance, e-mail, instant messaging and videoconferencing. But are we ready for this?

Telemedicine would also imply the possibility of teleworking, making it easier for health professionals to access their own computers remotely, either because they are in quarantine, because they are making their hours more flexible for family reasons or for other reasons. It would be sufficient to have a permit and appropriate software. At the moment, although it is technically possible (and cheap), it is not contemplated by the managers. 

If we assume that we are in an epidemic situation that will affect the majority of the population with mild cases, we must assume that the majority of health care providers will also be affected. If the health system does not respond flexibly there will be problems. 

That is why I think it is important to make an effort to provide professionals with telemedicine tools. We need means of protection and human and material resources, but we also need inteligence and courage to overcome administrative inertia and other problems.
