Diga treinta y tres. Say thirty-three. 说三十三。




En este tiempo no es fácil leer poesía. Vamos demasiado deprisa y la lírica tiene ritmos que rompen la lógica de un sistema hiperproductivista como el nuestro. Por eso es preciso reconocer que el asomarse a un libro de poemas es una actividad de riesgo. Y digo libro con pleno sentido. Los versos piden papel y abominan pantallas. Su levedad precisa de un poquito de materia a la que acariciar, susurrar, subrayar o pintar. Y de esa interacción entre el poeta, su mensaje escrito y el lector hay una posibilidad de que surja algo que con la prosa es casi imposible: el milagro. Leyendo 33, la última propuesta de Juan Manuel Casado Buendía, he de reconocer que me ha pasado. Quizá con mayor fuerza que en obras anteriores dado que en esta ocasión el título es fundamental. Un número natural, el epítome de lo concreto, nos abre la puerta a un espacio poético, el cenit de lo indeterminado. Y lo hace con 33 (35) poemas, un formato que se puede leer de una sentada y que de esta forma produce un efecto semejante a la degustación de una copa de vino a pequeños sorbos pero apurándola hasta el fin. El caldo es de calidad, no hablamos de un artista joven, hablamos de barricas de roble. El sabor es generoso y mezcla tonos de añoranza con regustos de futuro y un profundo matiz de clarividencia serena. Se abre en el paladar de forma progresiva dejando un regusto a melodía de Chopin, que llega suave desde el otro lado de la casa, con algún tono azul y otros naranjas. Espero lo disfruten.

Reading 33, the latest work by Juan Manuel Casado Buendía, I have to admit that this has happened to me. Perhaps with greater force than in previous works, given that on this occasion the title is fundamental. A natural number, the epitome of the concrete, opens the door to a poetic space, the zenith of the indeterminate. And it does so with 33 (35) poems, a format that can be read in one sitting and thus produces an effect similar to the tasting of a glass of wine in small sips, but drinking it to the end. The wine is of high quality, we are not talking about a young artist, we are talking about oak barrels. The taste is generous and mixes tones of longing with aftertaste of the future and a deep shade of serene clairvoyance. It opens on the palate in a progressive way leaving an aftertaste of Chopin's melody, which comes softly from the other side of the house, with some blue and some orange tones. I hope you enjoy it.

Los sistemas sanitarios públicos terminarán incorporándolo pero será bastante después de que todos tengamos algún sistema similar en el bolsillo.

De paso se lo recomendaremos a nuestros políticos y gestores sanitarios, que si hicieran caso seguro mejorarían la maltrecha sanidad.

Atención Primaria hundida. Primary care in the doldrums. 处于低迷状态的初级保健。

Entramos en la quinta semana de huelga de pediatras y médicos de familia de Madrid. La situación es de bloqueo dado que cada parte mantiene su posición y sus valores. La Consejería de Sanidad repite que es un pequeño conflicto laboral fruto de unos pocos exaltados, los 5000 facultativos siguen gritando que se ahogan. 

Lo que me queda claro es que los médicos de Madrid hemos tomado conciencia por fin de la situación real: la Atención Primaria está en fibrilación ventricular, no ha muerto todavía pero lo hará sin duda si nadie aplica una necesaria descarga de energía. Lo que pocos saben es que ese desfibrilador no vendrá. Vienen parches eso sí, vendrán médicos extracomunitarios, vendrán pequeños ajustes cosméticos, vendrá mucha propaganda institucional y extrainstitucional. Vendrán buenas palabras y nuevos discursos. Pero el corazón se parará. 

Ayer participé en una concentración en la plaza de Chamberí, entre varios cientos de médicos pude ver a dos enfermeras con una pancarta y a algún médico residente, pocos. ¿Qué significa esto? probablemente cada vez veremos menos residentes, lo que es dramático. Probablemente cada vez veremos menos profesionales de otras categorías que se animen a defender a los médicos más presionados del sistema ocupados como están con sus propias trifulcas. Son muchos años de defender cada uno lo suyo, hay que reconocer que ahí si lo ha hecho bien la organización. 

Entre los jóvenes médicos que huyen de la Atención Primaria y los más mayores que rezan por aguantar unos pocos años más, el núcleo de la protesta lo ocupan los que están en medio y tienen dudas razonables de que su salud física y mental resista lo necesario. Por mi parte creo que el enorme esfuerzo que ha constituido esta huelga ha cumplido su objetivo que era lanzar un mensaje claro al mundo sanitario y a la sociedad. Sin Atención Primaria de calidad solo quedará una medicina de beneficencia. Si eso es lo que quiere la sociedad, e imponen sus legítimos representantes, eso es lo que habrá. Dejaremos de sufrir cuando seamos capaces de aceptarlo.

El drama del príncipe Hamlet le sumió en el profundo sufrimiento de quien se angustia por una cuestión existencial. Cuando la voluntad nos empuja a una cosa y la realidad a su contraria la resultante no puede ser buena (en la obra mueren que yo recuerde al menos siete personajes: Polonio, Laertes, Claudio, Rosencrantz, Guildenstern, Ofelia y Gertrudis...).


Ayer estuve en una manifestación en Madrid, los pediatras y médicos de familia de Madrid llevamos un mes de huelga. Me llevé la calavera, y mi sufrimiento, que comparto con un buen montón de buena gente. Nos pasamos la mañana gritando, pitando y coreando, sabiendo que quien tiene que escuchar con probabilidad no lo hará. Porque nuestra voluntad nos empuja a ser excelentes facultativos y la realidad impuesta a convertirnos de máquinas administrativas de fast-medicine. No sé cuánto aguantaremos, pero me momento somos una piña. Nunca tantos médicos habíamos visto tan mal las cosas, y peor que se van a poner con la huida de residentes y jóvenes por un lado y las jubilaciones de los próximos años. Y aunque anuncien que llegan extracomunitarios no será suficiente ni de lejos, hay demasiados agujeros en un barco que ya no puede más. 


Revisiten a Shakespeare, tal vez descubran pistas o quizá se empapen de la justa ira del personaje.

La Atención Primaria necesita a Marie Kondo. Primary Care needs Marie Kondo. 初级保健需要Marie Kondo。

 




Cuando hablamos de desorden y armarios rebosantes la solución de moda es la que propone Marie Kondo. Por eso sería una buena propuesta pedir su ayuda para mediar en el conflicto que amenaza la Atención Primaria madrileña, que encabeza la lista de desorden del panorama internacional al ser la única sanidad del mundo con sus médicos de familia en huelga indefinida desde hace más de tres semanas. 


Los facultativos madrileños están hartos de tener su armario a presión. No les cabe ya más y siguen metiéndoles pacientes. Ellos se quejan de que así no hay manera de trabajar y de que todo queda hecho un ovillo y se llena de arrugas. La administración sin embargo los considera vagos y maleantes, ninguneándoles sus quejas y asumiendo que al armario hay que meter todo lo que llegue al centro de salud. 


La japonesa Marie Kondo defiende que el orden es la base de un hogar en armonía. Para ello es condición imprescindible desprenderse de todo lo que sea innecesario. Los pediatras y médicos de familia de Madrid llevan años intentándolo. Piden que se retiren certificados, volantes y demás burocracia inútil. Que se mejore el sistema de bajas labórales permitiendo la autocertificación de procesos leves de breve duración por el trabajador. Que se minimicen los procesos asistenciales redundantes o no necesarios. Que no se permita la prescripción diferida de fármacos, ambulancias o demás proveniente del hospital… Nadie ha escuchado sus denuncias, bueno sí, la inspección de trabajo que ha condenado a la administración, sin ningún efecto por cierto. 


Consentir que un médico tenga desordenado y sobrecargado el armario es peligroso. Hay problemas graves de seguridad, dado que un exceso de consultas innecesarias puede consumir el tiempo y la energía que se requiere para detectar problemas severos o atender correctamente los complejos. También tiene complicaciones en relación con la salud mental del facultativo, entrenado para asumir sobrecargas asistenciales puntuales pero no para mantenerlas de forma indefinida. 


La sociedad se está dando cuenta de que hay un problema serio en los centros de salud. Los médicos jóvenes se marchan y no los quieren, los mayores están deseando jubilarse y el resto aguanta como puede, pudiendo cada vez peor. 


Pese a la huelga aquí nadie quiere asumir arreglar un armario, cuyo problema principal es que no tiene fondo. Todo lo que llega al centro de salud se mete dentro. Y cada vez llegan más cosas dado que la sociedad está perdiendo la capacidad de asumir catarros, granos y diarreas. Nuestras abuelas eran capaces de cuidar la mayoría de las cuestiones que hoy llegan a la sanidad pública, y esto no hay Marie Kondo que lo arregle, por mucho que consigamos aumentar el presupuesto. 


No sé qué decirles. He estudiado a fondo los libros de esta señora y su método me resulta infalible. Pero, bien pensado, tengo mis dudas de que consiga éxitos si viene a Madrid.

That the deferred prescription of drugs, ambulances and others from the hospital should not be allowed... Nobody has listened to their complaints, well yes, the labour inspectorate has condemned the administration, without any effect by the way. 

Allowing a doctor to have a cluttered and overloaded wardrobe is dangerous. There are serious safety issues, as too many unnecessary consultations can consume the time and energy required to detect severe problems or properly attend to complex ones. It also has complications in relation to the mental health of the practitioner, who is trained to take on occasional overloads of care but not to maintain them indefinitely. 

Society is realising that there is a serious problem in health centres. The young doctors are leaving and are not wanted, the older ones are looking forward to retirement and the rest are hanging on as best they can, and getting worse and worse. 

Despite the strike, nobody here wants to take on the job of fixing a cupboard, whose main problem is that it has no bottom. Everything that arrives at the health centre goes inside. And more and more things are arriving because society is losing the ability to cope with colds, pimples and diarrhoea. Our grandmothers were able to take care of most of the issues that today reach the public health system, and there is no Marie Kondo to fix this, no matter how much we manage to increase the budget.

Chapapote sanitario. Health care asphalt stain. 卫生级粘稠沥青






Tras diez días de huelga de pediatras y médicos de familia estamos agotados. En la época de la movida solían decir aquello de “Madrid me mata” por la intensidad del ritmo de vida y los excesos de la noche. Hoy lo podemos retomar dado que la actual coyuntura y el pasotismo de la Consejería de Sanidad van a acabar con gran cantidad de facultativos y no es retórica. La sobrecarga laboral mantenida es tóxica pero es que la narrativa de decadencia que tenemos en la cabeza lo es mucho más. La distopía sanitaria que protagonizamos nos aboca al final de los tiempos de la Atención Primaria que conocíamos y muchos no dejan de pensar en un juicio final de llanto y crujir de dientes. Pensamientos de ruina y sentimientos de fracaso y desesperanza se nos quedan pegados como aquel chapapote espeso y negro que tiñó las costas gallegas hace unos años. Los sueños quedan contaminados de ese olor y nuestros amigos y seres queridos nos ven cabizbajos y agobiados. 


La situación es una catástrofe vital para muchos de nosotros semejante a la que cada día nos presentan nuestros pacientes a los que damos de baja y medicamos en muchas ocasiones por niveles de sufrimiento semejantes al nuestro. Pero así no podemos seguir. Ya estamos muchos medicados y otros tantos en terapia, en breve seremos más. No es mantenible. Nos falta una narrativa nueva, una buena noticia, algo que nos de un poco de esperanza. Quizá a alguno le ayuden sus creencias o su fe. Pero aquí nos hace falta algo que sea posible compartir entre todos, una senda que nos saque de este círculo infernal de queja y podredumbre que no descansa nunca. 


Hace falta silencio. Valor para caminar hacia el desierto vacío y silencioso que cada cual alberga dentro de sí mismo. Y allí escuchar. Habitualmente si conseguimos insistir se termina oyendo una leve voz interior que nos conforta, la misma que escuchan místicos, artistas y locos. Entre tanto grito es bien difícil pero así como nuestros pacientes lo consiguen y terminan saliendo de sus cuitas, también para uno hay salvación.


La situación es tan dura para muchos que estoy intentando sistematizar lo que resumo en estas líneas en un texto más amplio y en un taller que, si los elementos lo permiten, impartiré con la dra. Isabel Núñez en el Congreso de Comunicación de la Semfyc dentro de unos meses. Hablar de escatología sanitaria y catástrofes vitales tal vez escandalice a alguno pero ha llegado el tiempo de atrevernos con planteamientos nuevos dadas las dificultades y limitaciones de las distintas mesas sectoriales, donde la Administración suele ser quien meta los goles haciendo más o menos trampa.

After ten days of strike in Madrid by paediatricians and family doctors, we are exhausted. In the days of the "movida" they used to say "Madrid kills me" because of the intensity of the pace of life and the excesses of the night. Today we can take it up again, given that the current situation and the passivity of the Regional Ministry of Health are going to do away with a large number of doctors, and this is not rhetoric. The sustained work overload is toxic, but the narrative of decadence we have in our heads is much more so. The healthcare dystopia we are witnessing is leading us to the end of the days of primary care as we knew it, and many cannot help but think of a final judgement of weeping and gnashing of teeth. Thoughts of ruin and feelings of failure and despair stick to us like that thick, black tar that stained the Galician coast a few years ago. Dreams are contaminated by that smell and our friends and loved ones see us crestfallen and overwhelmed. 


The situation is a vital catastrophe for many of us, similar to what our patients present to us every day, whom we discharge and medicate on many occasions due to levels of suffering similar to our own. But we cannot go on like this, not for long. Many of us are already on medication and many others are in therapy, and soon there will be more. It is not sustainable. We need a new narrative, some good news, something to give us a little hope. Maybe some people are helped by their beliefs or their faith. But here we need something that we can all share, a path that will take us out of this infernal circle of complaint and rottenness that never rests. 


We need silence. Courage to walk towards the empty and silent desert that each one of us harbours within ourselves. And there to listen. Usually, if we manage to insist, we end up hearing a faint inner voice that comforts us, the same one heard by mystics, artists and madmen. In the midst of so much shouting it is very difficult, but just as our patients manage to get out of their troubles, there is also salvation for us.


The situation is so hard for many that I am trying to systematise what I am summarising in these lines in a broader text and in a workshop which, if the elements permit, I will give with Dr Isabel Núñez at the Semfyc Communication Congress in a few months' time. Talking about health scatology and vital catastrophes may scandalise some, but the time has come to dare to take new approaches, given the difficulties and limitations of the different sectoral roundtables, where the Administration is usually the one who scores the goals by cheating to a greater or lesser extent.

El bloqueo. The blockade. 封锁




 


Primera semana de huelga de médicos de familia y pediatras de Atención Primaria de Madrid. En mi caso servicios mínimos cubriendo las necesidades de mi pueblo en turno de mañana en solitario. Más de lo mismo. En el caso de mis compañeros en centros grandes dos mínimos por turno con bastante seguimiento. En el caso de la mesa negociadora: bloqueo.


En general estamos todos bloqueados de pensamiento, palabra, obra y omisión. De pensamiento porque las posturas son alejadísimas. Por un lado la administración defiende que su gestión es impecable pero basta ver un vídeo de la presidenta de la Comunidad de hace unos años prometiendo el oro y el moro para ver que no se ha cumplido mucho de lo dicho. Por otro lado el sindicato que defiende la huelga pidiendo presupuesto y menos sobrecarga que pese al enorme apoyo de los facultativos no dispone de un plan de implementación de mejoras mientras no haya más recursos



Necesitamos mediadores. No se imaginan cómo, dado que el problema de las sanidades públicas europeas es complejo, la española la más débil y la Atención Primaria es el canario de la mina. 


Hacen falta recursos, sí. Eso requiere que los grupos propongan avances en sus programas electorales y se lo tomen en serio. También que de una vez se implementen políticas a nivel nacional que defiendan los recursos humanos sanitarios de este país para que no se sigan dañando más y migrando cuando puedan. 


También hacen falta otras cosas. Si asumimos que el modelo está enfermo quizá sea tiempo de ensayar alternativas que vayan en la dirección de la autogestión de equipos pequeños. Quizá podamos abrir la puerta a médicos extracomunitarios para cubrir plazas de difícil cobertura, de refuerzo o de los puntos de atención continuada que están vacíos. Quizá podamos abrir la mano para reformar y potenciar la enfermería de Atención Primaria permitiendo que avance la prescripción enfermera de un catálogo de fármacos básicos que facilite la gestión de problemas de salud leves que acuden sin cita al centro de salud. 


El problema principal, como digo, es el bloqueo. Cada colectivo sanitario tiene sus intereses y sus representantes sindicales los priorizan sin atender al bien común. Tampoco se ha conseguido de momento crear foros o plataformas que aúnen la voz de los distintos profesionales y velen por el bien de todos. Por eso la administración no puede tener una hoja de ruta creíble, eligiendo el camino a trompicones según las dificultades que se encuentren. Imaginen si esto ocurriese en un barco… en este momento son los remeros los que gritan, por tener consultas que doblan o triplican en muchos casos los pacientes que atienden médicos de familia en otros países, especialistas de hospital o directamente la enfermera de la consulta de al lado. Pero puestos a gritar todo el mundo sabe y en algún momento lo termina haciendo. ¿Tendrá la Consejería sabiduría para discernir el peso de los mismos o actuará en función de criterios electoralistas?, ustedes lo intuyen como yo. 


Lo que quizá no sepa el ciudadano medio es que el daño que pediatras y médicos de familia llevamos en el cuerpo desde la pandemia es inmenso, por que ya veníamos tocados de antes. Un daño que hace que la mayoría esté tomando psicofármacos para aguantar o esté yendo a terapia o buscando ayuda. Esto no había pasado antes a este nivel. Son muchos años de sobrecarga infumable en los que hemos ido viendo como los pocos suplentes que había o los escasos refuerzos en zonas rurales en verano se volatilizaban. 


Y lo peor de todo es que los responsables lo saben. Los gestores saben lo que está pasando con los residentes que abandonan la especialidad de Medicina de Familia o se van a otros lugares. Conocen el nivel de malestar y dolor de los centros más sobrecargados. Conocen la cantidad de médicos enfermos por este motivo. Conocen que en cinco años se jubilará un 25% de la plantilla facultativa. Pero miran al suelo, a las siguientes elecciones, no más allá. Y a los directivos que se atreven a hacerlo se los cargan, basta mirar la lista de dimisiones de los últimos años que es muy ilustrativa. Hay una responsabilidad en el aparato de gestión de la Atención Primaria, cientos de puestos de trabajo que no están defendiendo a sus hermanos asistenciales. Hay una responsabilidad en el nivel de alta gestión que asume sin chistar lo que les impone el político de turno sin establecer líneas rojas. Hay una responsabilidad en los políticos cuyo discurso es “bajar impuestos” en lugar de priorizar la gestión de los mismos o “libertad” en lugar de defender el acceso a la misma. Y hay una responsabilidad en el ciudadano a la hora de exigir una sanidad digna a sus representantes. Habrá que hacerlo saliendo a la calle, mandando cartas, yendo a hablar con alcaldes o partidos, organizándose en los barrios, informándose de lo que pasa en su localidad, y por supuesto a la hora de votar. 


Para sacar a un paciente de una parada cardiaca hay que hacer cosas. No nos podemos quedar bloqueados mientras se apaga. Aquí pasa lo mismo, y ya van muchos años avisando, el paciente tiene ya mal color.

In general we are all blocked in thought, word, deed and omission. In thought because the positions are very far apart. On the one hand, the administration defends that its management is impeccable but it is enough to see a video of the president of the Community from a few years ago promising the gold and the silver lining to see that not much of what was said has been fulfilled. On the other hand, the union that defends the strike asking for a budget and less overload, despite the enormous support of the doctors, does not have a plan to implement improvements while there are no more resources. 


We need mediators. You can't imagine how, given that the problem of European public health is complex, the Spanish one is the weakest and primary care is the canary in the mine. 


Resources are needed, yes. This requires the groups to propose advances in their electoral programmes and to take it seriously. It also requires policies to be implemented at the national level that defend the human resources for health in this country so that they do not continue to be damaged and migrate when they can. 


Other things are also needed. If we assume that the model is sick, perhaps it is time to try alternatives that go in the direction of self-management of small teams. Perhaps we can open the door to non-EU doctors to fill hard-to-fill positions, reinforcement positions or vacant points of continuous care. Perhaps we can open our hands to reform and strengthen primary care nursing, allowing the nurse prescription of a catalogue of basic drugs to make it easier to manage minor health problems that come to the health centre without an appointment. 


The main problem, as I say, is the blockage. Each health collective has its own interests and their union representatives prioritise them without considering the common good. Nor has it so far been possible to create forums or platforms that bring together the voices of the different professionals and look after the good of all. This is why the administration cannot have a credible roadmap, choosing its path in fits and starts according to the difficulties encountered. Imagine if this were to happen on a boat... at the moment it is the rowers who are shouting, because they have consultations that in many cases double or triple the number of patients seen by family doctors in other countries, hospital specialists or even the nurse in the consulting room next door. But everyone knows how to shout and at some point ends up doing so. Will the Regional Ministry be wise enough to discern the weight of these or will it act according to electioneering criteria, you can sense it as well as I can. 


What the average citizen may not be aware of is that the damage that paediatricians and family doctors have been carrying in their bodies since the pandemic is immense, because we were already affected beforehand. This damage means that most of us are taking psychotropic drugs to cope or are going to therapy or seeking help. This has never happened before at this level. There have been many years of unbearable overload in which we have seen how the few substitutes we had or the scarce reinforcements in rural areas in the summer have gone up in smoke. 


And the worst thing is that they know it. The managers know what is happening with the residents who abandon the speciality of Family Medicine or go elsewhere. They know the level of discomfort and pain in the most overburdened centres. They know the number of doctors who are sick for this reason. They know that in five years 25% of the medical staff will retire. But they look to the ground, to the next elections, not beyond. And the managers who dare to do so are being killed, just look at the list of resignations in recent years, which is very illustrative. There is a responsibility in the management apparatus of Primary Care, hundreds of jobs that are not defending their care brothers. There is a responsibility at the senior management level who unquestioningly accept whatever the politician of the day imposes on them without drawing any red lines. There is a responsibility on the politicians whose discourse is "lower taxes" instead of prioritising tax management. And there is a responsibility on citizens to demand decent healthcare from their representatives. They will have to do so by taking to the streets, sending letters, going to talk to mayors or parties, organising in the neighbourhoods, informing themselves about what is happening in their locality, and of course when it comes to voting. 


To bring a patient out of cardiac arrest you have to do things. We can't just stand by while he or she shuts down. The same thing happens here, and we have been warning for many years, the patient already has a bad colour.

La solución de la Atención Primaria de Madrid. The solution for Primary Care in Madrid. 马德里初级保健解决方案。

El problema de la Atención Primaria madrileña no se pueda arreglar en una mesa sectorial en la que estén sentados la administración y un sindicato. Es demasiado complejo. Trataré de simplificarlo en dos minutos:


1. Tras años de infrafinanciación las platilla de pediatras y médicos de familia está exhausta.

2. Los médicos residentes jóvenes abandonan Madrid, los médicos mayores que pueden también.

3. En cinco años se jubilan un 25% de los médicos de familia actuales. 

4. La sobrecarga es inmensa: vacantes no cubiertas de larga duración (20% de toda la plantilla) + ausencias por permisos e IT (9%). Esto implica que además de una agenda de pacientes amplia hay que asumir por sistema una media de un 30% más de trabajo permanentemente, con picos de variable duración.

5. Los datos de burnout y malestar psicológico de los profesionales son más altos que nunca. 


Consecuencia: la situación es insostenible.


¿Qué se puede hacer?

1º. Asumir lo que hay. La situación es crítica y es fundamental que tanto los profesionales como la ciudadanía lo asuman. 

2º. Pedir a los partidos políticos de Madrid que hagan propuestas presupuestarias y de organización (dejo ejemplo de ayer en este hilo de Javier Padilla).

3ª. Plantear un debate en la Asamblea de Madrid que busque un consenso de mínimos.

4º. Pedir a los partidos políticos nacionales y al Gobierno una estrategia de fortalecimiento de la Atención Primaria que incluya partida presupuestaria. 


Los médicos de familia llevamos años perdiendo (salud mental) y los ciudadanos cada vez van a perder más (pueblos y barrios desfavorecidos sin médicos, más listas de espera, peor servicio). 

Ha llegado la hora de asumir que la Atención Primaria es un servicio básico esencial que requiere financiación prioritaria o asumir que como sociedad nos quedamos sin pediatras y médicos de familia públicos.
