Medicina espiritual Spiritual medicine

No reconocemos esos términos.
A nivel biológico, apenas hemos cambiado.

Spiritual Medicine

"The crisis that we are going through, the collapse of the natural world that we have created, as well as the culture in which we have lived in the last century is nothing but a spiritual crisis. We are the only civilization in history that does not revolve around something sacred. We have no religion, in the broad sense of the term, no notion of anything superior to us. There is nothing to kneel before, nothing to inspire humility. Neither gods nor goddesses nor divine nature. We do not recognize those terms. We think they are old-fashioned and empty. Evolving beyond religion is part of our myth of progress. We see the sacred as primitive. I don't think so, and it's been hard for me to realize that not believing in something higher has made us destroyers. We have placed ourselves at the center of the world. We have become a cancer for the planet. And my conclusion for all this, or perhaps I don't know whether to call it a conclusion, but rather a step along the way, is this: if we want our culture - or whatever we want to call it - to have a future, we are going to have to give nature the status of sacred. We will have to take a step back or forward towards the search for an animistic society with gods for all things: gods of the sea, of stones, of the wind, etc. We will have to take a step back or forward towards the search for an animistic society with gods for all things: gods of the sea, of stones, of the wind, etc. If we are not able to recognize that we are part of a vital network and only see things as resources, we are lost.

We are animals, we are identical to the people who painted the cave paintings 30,000 years ago. On a biological level, we have hardly changed. We are still wild creatures, which have not been domesticated. We don't put over the layer of civilization, but that can be torn very easily".

Paul Kingsnorth

Modern medicine is a highly technical scientific discipline. That has set aside its historical spiritual dimension and made its humanist version suffer. Health professionals have become technicians and scientists as well as practicing medicine and other disciplines in the field.

After many centuries of history we can now say that modern medicine is non-spiritual, with all the advantages that this may have. The question I ask myself is whether we will have lost something important along the way.

When accompanying processes of chronic illness, serious illness or the final stages of life, I find myself with the frustration of patients before a health system that is incapable of saving them. Was that all? Could we not do more?

It is not easy to explain that decrepitude and death are an intrinsic part of life and that medicine in the final stages can alleviate symptoms but not bring serenity or hope on its own.

When families have lost their inheritance of beliefs, hopes or spiritual aspirations and for many it is implantable to speak with a priest, little can be done by the health professional except the recognition of the importance of living each day with the maximum possible dignity. The most valuable thing in many cases is just to be  presence, to visit the patient and his family at home, to make them see that they are still being cared for and taken into account. Provide silence or conversation as the patient needs. And to rest in the desire of  the minimum possible suffering for them.

This approach is very difficult for professionals trained for action that badly tolerate impotence and nondoing. It is customary to offer retreat behaviors under the thought "it's none of my business" or "it exceeds my competence". The context of permanent overload offered by health institutions usually does the rest and the family is often alone and overwhelmed. The near future does not seem promising with the aging of the population, smaller or absent families and predominant paradigms of secular beliefs with little spiritual dimension.

Is it necessary to rescue the spiritual dimension of health care? Controversy is served. It seems that we are moving towards a hypertechnological fairy-tale medicine, as Juan Gérvas says, capable of all kinds of prowess. Maybe we are building a giant with feet of mud. What good is it for man to win the world if he loses his soul? What good are all the advances if in the end we do not find serenity, meaning or hope in the face of terminal illness, decrepitude and death?

Violencia de género Gender-based violence

Gender-based violence

After reading the guide on Gender Violence that has just been published in the Basque Country and that my friend Maxi Gutierrez has shared with me, I publish this brief reflection in the form of brief points.

1. Health professionals are usually blind to enormous problems that are not verbalized in the clinic.

2. Violence, in all its forms, is one of them.

3. Women suffer more violence because they are women.

4. The best way to detect violence and other hidden problems is through a relationship of trust built up over time.

5. You have to keep your attention high and try to see what you can't see, which is usually what is essential, as the Little Prince said.

6. Improving sensitivity to violence against women can help us do the same with violence against children, the elderly, people with addictions and men.

7. When faced with a suspicion or intuition, the right thing to do is to ask a facilitating question based on respect and non-judgment.

8. In all cases offer help and be able to respect the speed of the person when accepting it.

9. How are they going to tell us about these problems if we don't look into their eyes? Maximum care with the use of the computer in the visit and with our level of communication and listening.

10. I am ignorant about many of my patients' problems. For my part I can try to train myself but to really learn I need the invaluable help of the people who have them. Asking what I can do for you in this situation usually gives good clues.

The guide is a material for health professionals that will serve to improve training. More steps will have to be taken, both inside and outside the health system. For the moment, I am delighted that a group of colleagues from Primary Care in the Basque Country have taken part in this initiative.

https://www.osakidetza.euskadi.eus/contenidos/informacion/guia_violencia_genero/es_def/adjuntos/guia_vg_es.pdf

Hacia una pedagogía del malestar Towards a pedagogy of malaise

Pablo d'Ors

  • ¿Cómo enseñar a alguien a sufrir?

Towards a pedagogy of malaise

  • How to teach someone to suffer?
  • Is it possible to learn to handle one's own discomfort? To become excellent at handling it?
  • How can we help the suffering person to look into the eyes of his own suffering?

  • How can we make the one who drowns in the difficulty that no matter how real his suffering may be, he cannot compromise by itself his life?

The usual thing when one experiences discomfort is:

1. Fleeing

If this doesn't work:

2. Attack

Either to the outside and above all to oneself. Self-punishment is one of our favorite activities, it is not easy to recognize it.

Our culture has sophisticated flight, among other things is considered a civilization of spectacle and distraction. More and more screens, gadgets and tech surround us, helping us in this mission to remain permanently absent-minded.

But there comes a point when nothing manages to distract us from discomfort when it is strong enough. At last we fall into its claws and grate. We begin a dynamic of cognitive and emotional centrifugation from which it is very difficult to get out. If you have ever seen a rodent running inside a wheel you know what I'm talking about.

Society ends up turning discomfort into a source of consumer goods. With the least discomfort we are offered countless products and services. Check out, consume, buy, have fun. And when the shopping centre is not enough, go to the health services that will be delighted to attend you. In this way the consultations of alternative, complementary, supplementary or regulatory therapists are overflowing, whether public or private, elitist or popular.

Healthcare professionals are burdened with an enormous amount of discomfort for which they have limited time to manage. The answer is often chemical or industrial. Take this pill or go to the next specialist in the chain. Discomfort is rarely called by his name. Rarely we help to those who feel malaise to recognize their responsibility in bearing it and cradling it.

We have in our hands the paradox of being a society in constant search of happiness, or what is the same: in permanent flight from unhappiness. And we have forgotten that both are the face and the cross of existence, there is no light without shadow and no shadow without light.

We health professionals have a pending subject to help the suffering person to recognize what is inherent discomfort in life and differentiate it from disease. We must also learn to accompany it, guide it and alleviate it and not just treat it. The easy answer for us is the pill, but it is not always the right one.

We citizens in general have another unresolved issue, that of better handling personal discomfort and unhappiness by building self-care based on acceptance, self-knowledge and the search for personal solutions instead of continuing to blame others for it and demanding the state to remove from us an unhappiness that is not its competence.

¿Merece la pena ser Influencer en salud? Is it worth being a health influencer?

Is it worth being a health influencer?

These days there has been some debate on Twitter about the eternal question of whether it is ethical for a doctor to become a businesswoman and start selling products in parallel with her clinical work.  The controversy was  opened when a known influence of the sector, a pediatrician known in the media was hunted. The crime: selling pacifiers.

I am not going to waste a second in the case because I believe that it does not deserve greater consideration on my part given other deep-seated problems that we have apart from the fact that other minds, clearer than mine, have extensively analyzed the subject.

What I do find worthy of reflection is whether society can be interested in its health professionals having a certain informative role and presence in social networks and the media. Health is an issue that concerns everyone and it is a fact that the health information that is more easily found on the Internet is not of much quality, mainly due to the private interests that produce it. Are health professionals more reliable than product brands, the pharmaceutical industry and others? Probably yes. The truth is that public exposure is not usually desired by them since it does not bring benefits and it is easy to produce problems (criticisms, misunderstandings, slips, Trolls...). On the other hand, if a nurse or doctor comes up with something to sell, don't doubt that from the first minute he get a good mob of angry colleagues over him.

In this matter there are degrees as in everything. The pharmaceutical industry continues to move millions to promote its products and that includes influencing the sanitary. I think it is much more serious to sell, promote or encourage drugs, vaccines or chemotherapy that offer little or no value (the list is long) than pacifiers. But that's just an opinion.

How can a healthcare provider recover the effort of maintaining a blog, a social network account or a video channel? Economically it is very difficult. The rewards tend to be social recognition, the visibility of content, participation in debates and some invitation to conferences or events, little more. On a monetary level, perhaps the most ethical thing is to write books, others advertise their contents or sell merchandaising. It is also possible to offer paid courses or conferences. The truth is that none of this usually produces remarkable benefits.

Is it possible to promote food or drink? Is it possible to support a parapharmacy product? Wheelchairs, glasses...? To what extent does the health professional who sells something stain his hands?

The debate is on the table. And it will be good to reflect on the subject. Society needs its health professionals to help them with information, training and dissemination and a priori it would be licit to encourage it. The problem is that if the rewards are not clear, things will continue as they are, with a powerful pharmaceutical industry acting as a sponsor behind the scenes and professionals who do not find the right ways to monetize a dissemination effort that takes hours of work to maintain.

El fin de la medicina que conocemos. The end of the medicine we know.

The end of the medicine we know

Society needs doctors with a humanist vision who are not mere technicians or bureaucrats. But what would happen if the health systems in which these doctors work were not allowed to do so?

Today's European health systems are based on the philosophy of public services emanating from the welfare states  of industrial societies. Consequently, their structure is also industrially based. Large hospitals that function as chains of diagnostic and therapeutic services and smaller health centres designed to absorb large volumes of minor health problems. Health and disease have become monetizable units suitable for global consumer markets. The aim of health sistemas is to provide service by giving priority to the quantitative over the qualitative, given that the economic indicators used to organize the objectives are of the first type. One consequence of this design is that the greater the pressure on care, the worse treated both professionals and patients feel, as the system does not contemplate corrective measures for the overloads to which all systems are subjected. It is not uncommon for a significant percentage of the working hours of a health professional to have to attend to an excessive number of patients. In these conditions it is very difficult not to fall into a defensive medicine which, lacking sufficient time to rely on quality communication with the patient and a detailed physical examination, relies on excessive diagnostic tests (overdiagnosis) and excessive treatments (overtreatment).

It is common to find health professionals who feel incapable of developing the practice for which they are trained and are forced to offer a minimal medicine that does not usually satisfy the patient because they do not have a minimum of time or sufficient reflection.

Are there alternatives? Some may argue that private medicine may be one, but if we look closely at it, it is also subject to pressures and time limitations that do not make the development of human medicine easy.

On the other hand, the pressure of technology makes the continuous training of professionals and professional performance more complex with computer systems of clinical history and diagnostic and therapeutic means that progressively increase their difficulty of use.

The end of medicine with a human face has been announced for some time and we have not yet been able to implement the necessary changes to reverse it, which, despite being known, no one has risked to promote.

To have reference health professionals immersed in complaint and discomfort is not recommended for anyone. It is a fact that each time this disease is more widespread without moving card to solve it. The laws of the market push to evolve towards dichotomous systems with a quality assistance for the elites and a basic charity for the rest. If civil society does not become aware and forces the political status to make the necessary adjustments this will be inevitable.

The rise of biometrics, personal medical assistants, virtual health reality and health robots and algorithms based on artificial intelligence will broaden the spectrum of health service providers to a level we cannot imagine. The downside will be that these new channels and services will lack a human face. Dehumanized medicine cannot be fully satisfactory. Can we venture that this technology-driven medicine will become the new charity while elites will retain access to professionals? It is a possibility that the market pleases. The future will depend on the dialogue established between actors (citizens, professionals, managers and politicians) to organize priorities and the roadmap that leads us towards objectives of social or economic base as chosen. Each actor has difficult challenges ahead, citizens take responsibility for self-care and the management of their life habits, professionals for their social participation outside consultation and in the development of sensitivity and humanist ability, managers for the development of a new form of leadership and motivation, and politicians for a determined defence of the common good.

I know that health professionals are doing their job correctly and that patients want to be treated with technical and human excellence. In order for both points of view to meet, a structure is needed that allows this. Tax-financed public health systems are still an interesting alternative. But if they do not evolve they will end up as skeletal dinosaurs on the verge of disappearing in a world ravaged by permanent economic cataclysms. Perhaps now is the time to give way to a myriad of dinosaurs.

¿Conoces el club de la batas blancas? Do you know the white coats club?

Do you know the white coats club? Doctors are a guild to feed part. On the one hand, its members are selected from among the best of a country's students: excellent academic records, countless hours of study and a marked tenacity. On the other hand we deal with human pain and suffering, adverse working conditions (guards, primary care...) and various borderline situations. The result is that we head statistics of addictions, suicide, professional burn out and other miseries. That's why when a group of doctors converse in a non-working environment, they usually do a catharsis exercise that can be insufferable for profane people because the sanitary anecdotes with which they are accustomed to compete as eschatological, bloody or disgusting as they are. My friends Juan and Guido (Yo, doctor) have published a graphic novel that gives account of this facet of the doctors not always known by the public. The ancestral need of any group of doctors who meet outside of work, especially if they are residents, to turn into narrative the ghosts, errors, fears and other incredible situations that dealing with people provides.
There are thousands of anecdotes, those shared by the authors have been lived by them showing us that on many occasions reality surpasses fiction and that the human being is much more complex than we think. I congratulate them for the quality of the work and for the excellent graphic language they master. I hope it will allow us to understand a little more the complexity of patients' life and of those of us who, as doctors, try to help them.
