Congreso Somamfyc 2020. Plántate y crece.

 




El jueves 5 de noviembre en horario de mañana tendrá lugar la edición de este año del congreso de la Sociedad Madrileña de Medicina Familiar y Comunitaria, que será virtual como no podría ser de otra manera. 


Moderó una mesa de 10 a 11:10 en la que participarán Concha Herranz Calvo, Alberto Cotillas Romero y Alberto López García-Franco. Compartiremos lo que podamos usando la etiqueta #somamfyc en Twitter, para lo que no es necesario tener cuenta (si nos quieres hacer llegar una pregunta por esa vía si necesitarás una). 

En la propuesta de la mesa original previa a la pandemia se querían abordar algunos de los problemas de la medicina de familia en atención primaria, desde el punto de vista de 3 médicos de distintas generaciones (precariedad, emigración, sobrecarga asistencial, conciliación, desencanto, falta de reconocimiento) abordando también los puntos fuertes de la especialidad, las oportunidades y las estrategias de mejora: ¿Autogestión? ¿Reorganización? ¿Horarios? Todo ello se replantea ahora con los problemas y los cambios que ha supuesto la pandemia Covid-19.

 

Trataremos de reflexionar sobre la medicina de familia y la Antención Primaria y hacernos preguntas. No sé si encontraremos respuestas, lo que sí parece que tenemos claro es que si hay que resetear primaria lo tendremos que hacer con nuestras propias manos.







Un cómic sobre cáncer. Mom's cancer. 媽媽的癌症。

 




La aproximación a la enfermedad grave es una asignatura pendiente en una sociedad como la nuestra que considera tabú la discapacidad y la muerte. Por eso valoro mucho proyectos creativos como el de Brian Fiess en relación al cáncer. En su libro nos cuenta su experiencia como hijo de una madre con cáncer y nos invita a entrar en la dinámica de una familia que ve su vida transformada por una situación compleja frente a la cual cada protagonista reacciona de una manera. Al usar el lenguaje del cómic consigue que la realidad mostrada sea apta para todos los públicos y que incluso los más jóvenes puedan acercarse y abrir líneas de diálogo en sus propias familias. 

Pese a tocar un tema duro he de reconocer que he disfrutado con su lectura identificándome con los diferentes personajes y entendiendo que eso que llamamos salud es un regalo muy poco valorado. 








Mom's cancer.

The approach to serious illness is a pending subject in a society like ours that considers disability and death taboo. That's why I value creative projects like Brian Fiess' in relation to cancer. In his book he tells us about his experience as the son of a mother with cancer and invites us to enter into the dynamic of a family that sees its life transformed by a complex situation to which each protagonist reacts in a different way. By using the language of comics he makes the reality shown suitable for all audiences and even the youngest can approach and open lines of dialogue in their own families. 


In spite of touching a hard subject I have to admit that I have enjoyed reading it identifying myself with the different characters and understanding that what we call health is a gift very little valued.

Halloween sanitario. Sanitary Halloween. 萬聖節衛生




Veo mucho vampiro, bruja, monstruo y esqueleto en el mundo sanitario. Mucho zombi, espectro y alma en pena. Montones de compañeros tomando ansiolíticos y compañeras antidepresivos, entre otras cosas. Y en un día como hoy en el que las calles se llenan de disfraces yo me pregunto qué coño está pasando. Y la respuesta es quizá más amarga que la pregunta. Como sociedad hemos primado al dios dinero y a su primogénito el beneficio, ninguno de ellos nos defenderá de la peste ni de la estupidez humana, plagas que azotan los imperios desde tiempos pretéritos. 


Como médico cada día que llego al centro de salud me cambio de ropa poniéndome un pijama sanitario blanco, zuecos y bata. Luego quizá otra bata de protección, pantalla facial y guantes. Llevo ocho meses de mascarada, que sin duda serán más. Mis compañeras están hartas, no es para menos, la presión psicológica y laboral es enorme y como refuerzo solo hemos recibido algún aplauso hace tiempo y algún caramelo de los políticos y responsables de gestión. Aprovechando que estarán llenas, estoy por salir a la calle esta noche con mi hija con una calabaza de plástico a ver si alguien nos da alguna chuchería de más, que está la cosa muy mala y hay que seguir tragando amargura a raudales. Pero la sociedad tampoco reacciona, bloqueada con tanta mala noticia, bronca política y sucesos infaustos.


Lo malo es que los sanitarios estamos pasándolas canutas y el purgatorio de la Atención Primaria se está contagiando gradualmente a los hospitales que ven con horror como se van llenando del mismo barro que lleva medio año asolando los centros de salud. Ojalá no se pongan malos. 





Sanitary Halloween


I see a lot of vampires, witches, monsters and skeletons in the health world. A lot of zombies, ghosts and souls in distress. Lots of companions taking anti-anxiety and anti-depressant medication, among other things. And on a day like today when the streets are full of freaks I wonder what the hell is going on. And the answer is perhaps more bitter than the question. As a society, we have given priority to the god of money and his first-born son, profit. None of them will defend us from the virus or human stupidity, plagues that have plagued empires since ancient times. 


As a doctor, every day when I arrive at the health centre I change my clothes into white sanitary pyjamas, clogs and gown. Then perhaps another protective gown, face shield and gloves. I've been on a masquerade for eight months, which will no doubt be longer. My colleagues are fed up, no wonder, the psychological and work pressure is enormous and as a reinforcement we have only received some applause for a long time and some candy from politicians and managers. Taking advantage of the fact that they will be full, I am about to go out tonight with my daughter with a plastic pumpkin to see if anyone will give us any extra sweets, which is very bad and we have to keep swallowing bitterness. But society does not react either, blocked by so much bad news, political anger and unhappy events.


The bad thing is that we health workers are having a hard time and the purgatory of Primary Care is gradually spreading to the hospitals, which watch with horror as they fill up with the same mud that has plagued the health centres for half a year. Let's hope you don't get sick.

Achicar. Bail out. 紓困。

 



Cuando alguna circunstancia adversa hace que una embarcación acumule agua es imprescindible achicar para evitar males mayores. Este año está siendo especialmente dificultoso para la navegación de la Atención Primaria, y todavía quedan fuertes marejadas y tormentas por llegar. La marinería está exhausta achicando agua que no cesa de entrar en tromba tanto por cubierta como por los múltiples y añejos agujeros del casco.


Los gritos de auxilio de la tripulación han servido de poco bajo el estruendo de una galerna cuyos rayos y truenos han puesto en jaque a la entera sociedad. No ha habido refuerzos ni los va a haber, más bien lo contrario cuando la lista de renuncias, dimisiones y bajas crece cada día incluyendo directoras generales, Viceconsejeras y otros cargos... Lo único que parece avanzar a buen ritmo es el nuevo hospital de pandemias donde hacen turnos día y noche los 1300 trabajadores que quizá deberían haberse contratado en su día como rastreadores... ¡Qué pena que los ladrillos no puedan pasar consulta!


En estos meses los profesionales sanitarios hemos tratado de elevar propuestas de mejora y sostenibilidad que si bien en algún caso han sido escuchadas no han producido ninguna mejora ni implementación destacable. 


Seguimos sin asumir lo evidente:


1. Los profesionales sanitarios de Atención Primaria están forzados y extenuados.

2. La sociedad lleva mal que la sanidad a la que están acostumbrados (accesible, gratuita, cercana y resolutiva) no esté disponible.

3. Los cuadros directivos no tienen capacidad ni de modificar el sistema ni de reforzarlo.



Y sin hacer lo imprescindible:


1. Proteger del burnout  a los profesionales para evitar su renuncia o obligarles a trabajar en modo zombi.

2. Explicar a la sociedad que en una emergencia sanitaria es obligatorio hacer un uso mínimo e imprescindible de la sanidad.

3. Priorizar que el nivel de gestión se encargue de los dos puntos anteriores.


¿Y esto cómo se puede conseguir?


1. Filtros de entrada.

2. Bombas de achique de burocracia y servicios covid.

3. Asumir demoras.


Dado lo extraordinario de la situación no podemos permitir que un médico de familia mantenga en el tiempo agendas forzadas con más de 50 pacientes. Cada centro de salud tendrá que consensuar sus propias líneas rojas, cada profesional deberá asumir su propio límite y verbalizar cuando ya no pueda más. 


Para esto es fundamental mejorar los filtros de entrada de demanda del centro de salud. Permitir que los administrativos puedan registrar en la cita lo que el paciente quiera verbalizar como motivo de consulta y puedan implementar circuitos que conviertan visitas presenciales en telefónicas o en no presenciales.

Implementar aplicaciones de comunicación entre pacientes y el centro de salud donde puedan compartir imágenes, informes o realizar solicitudes de consulta, estas existen ya en varias comunidades y es inaudito que estén tardando tanto en otras. 

Gestión compartida de la demanda entre las enfermeras y las médicas con triaje en puerta cuando las condiciones del centro de salud lo permitan.


Seguimos con un exceso de burocracia y de servicios de diagnóstico, rastreo y tratamiento por la pandemia. No se han implementado circuitos que alivien la burocracia como declaraciones juradas de incapacidad laboral breve ni digitalización de los documentos de baja para evitar el papel. Tampoco se han externalizado carpas de diagnóstico ni el seguimiento telefónico. 


Por último tendríamos que asumir que si las consultas de los profesionales ausentes no se cubren y las de los presentes están sobrecargadas es inevitable que exista demora en la atención. Debería prohibirse que un médico pueda atender más de consulta y media (sus 30 pacientes y 15 del ausente) de forma mantenida, pero insisto que estas cifras tendrían que consensuarse en cada equipo. Los pacientes que acudan sin cita serán valorados y citados cuando corresponda. 


El problema de no achicar suficiente es que cada día que pasa hay más profesionales con el agua al cuello que o se terminarán ahogando o escaparán nadando hacia tierras más firmes. Espero que la nueva directora general de Atención Primaria de Madrid, que me consta conoce bien el barco, pueda ayudar a aliviar la situación.








Bail out.


When some adverse circumstance causes a boat to accumulate water it is essential to bail out to avoid further damage. This year is being especially difficult for Primary Care sailing, and there are still heavy swells and storms to come. The sailors are exhausted, bailing out water that does not cease to flow in from the deck and from the many old holes in the hull.


The crew's cries for help were lost under the din of a galley whose lightning and thunder have put the whole of society in check. There have been no reinforcements nor will there be any, quite the contrary as the list of resignations and casualties grows every day including general managers, vice-counsellors and other positions... The only thing that seems to be moving forward at a good pace is the new pandemic hospital where the 1300 workers who should perhaps have been hired at the time as trackers work day and night shifts... What a pity that the bricks can't pass consultation!


During these months, we health professionals have tried to put forward proposals for improvement and sustainability which, although in some cases have been heard, have not produced any notable improvement or implementation. 


We still do not assume the obvious:


1. Primary health care professionals are forced and exhausted.

2. Society has a hard time with the fact that the health care they are used to (accessible, free, close and responsive) is not available.

3. The management is not capable of modifying or strengthening the system.


And without doing what is necessary:


1. Protecting professionals from burnout to avoid their resignation or forcing them to work in zombie mode.

2. Explain to society that in a health emergency it is obligatory to make a minimum and essential use of the health system.

3. Prioritise that the management level takes care of the two previous points.


How can this be achieved?


1. Input filters.

2. Bureaucratic drainage pumps and covid services.

3. Assume delays.


Given the extraordinary nature of the situation, we cannot allow a family doctor to keep forced schedules with more than 50 patients. Each health centre will have to agree on its own red lines, each professional will have to assume his or her own limit and verbalize when he or she can no longer cope. 


To do this it is essential to improve the health centre's demand input filters. Allow the administrative staff to record in the appointment what the patient wants to verbalize as a reason for consultation and to implement circuits that convert face-to-face visits into telephone or non-presential visits.

Implement communication applications between patients and the health centre where they can share images, reports or make consultation requests. These already exist in several communities and it is unheard of that they are taking so long in others. 

Shared demand management between nurses and doctors with triage at the door when conditions at the health centre allow it.


We continue to have an excess of bureaucracy and diagnostic, screening and treatment services due to the pandemic. Circuits to alleviate bureaucracy such as short declarations of incapacity for work and digitalisation of discharge documents to avoid paper have not been implemented. Neither have diagnostic tents or telephone follow-up been outsourced. 


Finally, we would have to assume that if the queries of absent professionals are not covered and those of those present are overloaded, it is inevitable that there will be a delay in attention. It should be forbidden for a doctor to attend more than one and a half consultations (his 30 patients and 15 of the absent one) on a sustained basis, but I insist that these figures should be agreed upon by each team. Walk-in patients should be assessed and cited where appropriate. 


The problem of not getting enough is that every day there are more professionals with water around their necks who will either end up drowning or will escape by swimming to safer ground. I hope that the new Director General of Primary Health Care in Madrid, who knows well the terrain, will be able to help alleviate the situation.

Semillas de revolución. Seeds of revolution. 革命的種子。

 




La confusión reinante no permite la toma de conciencia. Se parece a esa típica sensación en la que un señor camina distraído entre una muchedumbre ensordecedora mientras un carterista hábil le desvalija sin que se dé ninguna cuenta. 


No nos estamos enterando que nos quedamos sin planeta. Vamos cuesta abajo hacia un acantilado sin que nadie frene el despropósito. Y no será por señales, los incendios, sequías, inundaciones y demás no pueden ser más explícitos.


Nosotros a lo nuestro, tratando de buscarnos las lentejas y mirar por lo propio. Los políticos a lo suyo que viene a ser exactamente lo mismo a mayor nivel. Y nadie plantea la posibilidad de ralentizar la economía, la vida ordinaria, el nivel de consumo. Nadie nos dice que es posible vivir de una forma más lenta y más sencilla. Nadie nos enseña a disponer nuestros ahorros de una forma que no añada más madera a la caldera. Nadie nos proporciona las imprescindibles narrativas a las que nos podamos agarrar en estos tiempos de gran cambio. O quizá todo eso esté ya delante de los ojos y simplemente no queramos verlo. El Mercado ha puesto un velo sobre todo lo que pudiera servir de ayuda a despertar y nos sigue cociendo a fuego lento, un fuego agradable por cierto, sin que nadie note gran cosa dentro de la olla. ¿Cómo vamos a darnos cuenta de algo cuando nuestro sueldo, propiedades y modo de vida dependen de que no nos enteremos?


¿Quién podría hoy intervenir la economía global, nacionalizar todos los medios de producción, cambiar de forma extraordinaria las reglas del mercado?


Ningún sabio ha sabido responder. Alguno visualizan el desastre pero no dan alternativas.


Y en esas estamos, presintiendo por un lado el hundimiento y haciendo grandes esfuerzos por otro para mantenernos todo lo distraídos que podemos. Algo que no nos está saliendo bien y que cada vez nos hace sentir más nerviosos, tensos, preocupados, angustiados y miserables. Si pudiéramos hacer explícitos los sueños lo veríamos meridiano. Estamos compartiendo a nivel global un oscuro horizonte onírico plagado de distopia y pesadilla que está empezando a contaminar nuestras creaciones. ¿No se han preguntado alguna vez porqué hay tanta película de zombis, cataclismos, fin del mundo y demás desgracias, y tan pocas de mundos utópicos?


Tenemos la oportunidad de ser significativos para la historia. Nunca antes fue tan revolucionario pasear, sonreír o soñar. Porque cada pequeño gesto cuenta y eso convierte este momento en algo único que puede cambiar el curso de la balanza. Alguien normal en un lugar normal tal vez comience a tirar de la palanca de emergencia que consiga detener este gran tren descontrolado. Quizá alguien normal en un lugar normal empiece hoy a soñar un horizonte nuevo con una nueva narrativa. Tal vez nosotros podamos ser protagonistas. Esa podría ser la propuesta para hoy.








Seeds of revolution.

The prevailing confusion does not allow us for awareness. It's like that typical feeling where a man walks absent-mindedly through a deafening crowd while a skilful pickpocket takes him away without him noticing. 

We're not realising that we're out of a planet. We are going downhill towards a cliff without anyone stopping the nonsense. And it won't be for signs, the fires, droughts, floods and so on cannot be more explicit.

We go about our business, trying to find the dayly bread and look for our own. Politicians do their own thing, which is exactly the same at a higher level. And no one raises the possibility of slowing down the economy, ordinary life, the level of consumption. Nobody tells us that it is possible to live more slowly and more simply. No one teaches us to arrange our savings in a way that does not add more wood to the boiler. No one provides us with the indispensable narratives that we can hold on to in these times of great change. Or perhaps all this is already before our eyes and we simply do not want to see it. The market has put a veil over everything that could help us to wake up and it continues to simmer us, a pleasant fire by the way, without anyone noticing much inside the pot. How can we realize anything when our salary, properties and way of life depend on us not knowing?

Who today could intervene in the global economy, nationalize all the means of production, change the rules of the market in an extraordinary way?

No wise man has been able to answer. Some visualise disaster but give no alternatives.

And there we are, sensing on the one hand the collapse and making great efforts on the other to keep ourselves as distracted as possible. Something that is not going well and that makes us feel more and more nervous, tense, worried, distressed and miserable. If we could make our dreams explicit we would see them talking loudly to us. We are sharing on a global level a dark dream horizon plagued by dystopia and nightmare that is beginning to contaminate our creations. Haven't you ever wondered why there is so much film of zombies, cataclysms, the end of the world and other misfortunes, and so little of utopian worlds?

We have the opportunity to be meaningful to history. Never before has walking, smiling or dreaming been so revolutionary. Because every little gesture counts and that makes this moment unique and can change the course of the balance. Someone normal in a normal place might start to pull the emergency lever that can stop this great runaway train. Maybe someone normal in a normal place will start today to dream of a new horizon with a new narrative. Maybe we can be the protagonists. That could be the proposal for today.

¿Qué pasaría si de un mes para otro la mayor parte de las médicas y médicos de familia renunciaran a sus puestos?


Este horizonte ha pasado de la inexistencia teórica a la alta posibilidad práctica en meses. La crisis del coronavirus ha ejercido tal presión en el sistema sanitario que ha puesto al límite a la parte más vulnerable del mismo que somos los profesionales. Y cuando digo límite lo digo con todo lo que significa: llanto, insomnio, ansiedad, agobio, tristeza, depresión, bloqueo...


La unión de una sobrecarga extrema mantenida unida a la necesidad de relacionarse en remoto entre pacientes y profesionales, el gran nivel de ansiedad social y la enorme desgobernanza política y sanitaria se están llevando por delante a lo más granado de las profesionales sanitarias de primera línea. 


La alternativa al descalabro no es una huelga con un 100% de servicios mínimos, ni publicar cartas de protestas ni participar en grupos de mejora. La única alternativa posible es renunciar. Son ya muchos los años proponiendo ideas, libros blancos, artículos y evidencia científica. Son ya muchas las horas participando en grupos e intentos de mejora. Son ya muchos los años de desfinanciamiento, empeoramiento de condiciones de trabajo y de maltrato institucional. 


Los últimos meses han dado la puntilla a una Atención Primaria herida de muerte. Las médicas de familia llevan años con sus agendas de trabajo hechando humo, multiplicarlas por dos ha demostrado ser totalmente inviable. 


Ante esta situación caben tres posibilidades:


1. La queja.

2. El silencio.

3. La renuncia. 


La queja es frecuente en las charlas de café de los sanitarios o en sus casas. Los familiares lo saben bien. El malestar sale a borbotones a la más mínima oportunidad pero sin embargo cuesta mucho visibilizarlo en medios de comunicación o en redes sociales. 


Lo más frecuente es el silencio (de los corderos) al intuir cercano el peligro del lobo que no es otro que el temido burnout.


Por último tenemos la opción del “ahí te quedas” en forma de búsqueda de otro trabajo, emigración, excedencia o baja laboral de larga duración. Es el curso de acción más extremo y su posibilidad solo surge cuando las condiciones externas son también extremas.


Un grupo de médicas de familia ha creado una plataforma @yorenuncioap con un  manifiesto y un incipiente grupo de trabajo. No les puedo dar números pero sí informar de que es un completo desastre para el sistema. Emigración médica y renuncias siempre ha habido pero nunca se han hecho públicas como en este momento y nunca han puesto tan en evidencia a directivos y políticos sanitarios, responsables últimos de la enorme crisis sanitaria. 


Tengo que reconocer que un servidor también se ha planteado esta alternativa. No descarto ese curso de acción dependiendo de cómo avance la partida. 


De momento sigo trabajando y asumiendo con dolor la marcha de profesionales de gran valor. Si esto no mueve a políticos y sociedad a darse cuenta de que nos estamos quedando sin sanidad nada lo hará. Si esto no mueve al resto de profesionales sanitarios de cualquier perfil a tomar medidas, nada lo hará.

What would happen if from one month to the next most of the family doctors resigned from their posts?

This horizon has gone from a theoretical absence to a high practical possibility in weeks. The coronavirus crisis has put such pressure on the health system that it has pushed the most vulnerable part of it, the professionals, to the limit. And when I say limit I mean with all that it means: crying, insomnia, anxiety, oppression, sadness, depression, blockage...

The combination of extreme overload, together with the need to relate remotely between patients and professionals, the high level of social anxiety and the enormous political and health imbalance are taking away the majority of frontline health professionals. 

The alternative to the disaster is not a strike with 100% minimum services, or publishing letters of protest or participating in improvement groups. The only possible alternative is to quit. For many years now, ideas, white papers, articles and scientific evidence have been put forward. There are already many hours participating in groups and attempts at improvement. There have been many years of lack of funding, worsening working conditions and institutional abuse. 

The last few months have seen the end of a primary care system that has been mortally wounded. Family doctors have been fuming for years with their work agendas; doubling them has proved to be totally unfeasible. 

In this situation there are three possibilities:

1. The complaint.
2. Silence.
3. Running away. 

The complaint is frequent in the coffee talks in the caffe time or in their homes. The relatives know this well. The discomfort gushes out at the slightest opportunity but it is nevertheless very difficult to make it visible in the media or on social networks. 

Most often, it is the silence (of the lambs) as they sense the danger of the wolf nearby, which is none other than the dreaded burnout.

Finally we have the option of  saying bye bye in the form of looking for another job, emigration, leave of absence or long-term leave. 

A group of family doctors has created a platform @yoRenuncioAP with a small manifesto and a nascent working group. I can't give you numbers but I can report that it's a complete disaster for the system. Medical migration and resignations have always occurred but they have never been made public as they are at the moment and they have never brought health managers and politicians, who are ultimately responsible for the enormous health crisis, so much to the fore. 

I have to admit that I have also considered this alternative. I do not rule out this course of action depending on how the game progresses. 

For the moment I continue to work and take on with pain the departure of highly valued professionals. If this does not move politicians and society to realise that we are running out of healthcare nothing will. If this does not move the rest of the health professionals of any profile to take action, nothing will.

"En la salud, ustedes mandan pero no saben", nosotros los profesionales sanitarios tampoco sabemos mucho. We know little. 知道的很少。






Hace unos días 55 sociedades científicas españolas escribieron un manifiesto dirigido al presidente del gobierno y a los de las Comunidades Autónomas criticando la forma en que se ha gestionado la actual pandemia. 

Puedo coincidir con algunos puntos del decálogo pero creo justo reconocer que los profesionales sanitarios tampoco sabemos mucho de pandemias. Me explico.

1. Son acontecimientos poco frecuentes y cuando su extensión es tan grande como la actual desbordan la capacidad de los sistemas sanitarios y de la sociedad. Hay epidemias más fáciles de controlar que otras, las pandemias por virus respiratorios son de las más complejas de manejar cuando su nivel de contagiosidad es alto. 

2. La evidencia científica es la principal fuente de luz de los sistemas sanitarios. En la actual pandemia nos hemos mantenido a oscuras por varias razones. La principal la poca calidad de los datos, la secundaria la dificultad para realizar investigación en tiempo y forma. 

3. Los tiempos para desarrollar evidencia científica son largos. Pongo por ejemplo el desarrollo de vacunas que requiere de meses a años en el mejor de los casos. 

4. Donde si creo que podemos aprender es en mejorar la transparencia y gestión de los datos por un lado y en la comunicación y coordinación entre el nivel político, el técnico y la sociedad. 


Les pongo por ejemplo la famosa curva de infectados con las dos olas, que es una imagen inconsistente por ser las condiciones de medida distintas en la primera fase y en la segunda.

Como vemos poco tiene que ver una curva con otra, lo que de da fe de lo sencillo que es manipular los datos, o sencillamente no darlos corregidos. 






Si a esta realidad sumamos que no hay suficientes estudios que nos informen sobre cuestiones de manejo básico de mascarillas, distancia, transmisión de aerosoles y demás, el nivel de confusión se mantendrá alto. 

En los centros de salud a esto se suma que estamos desarrollando mucha labor telefónica para proteger a los pacientes evitando que acudan a un lugar con alta posibilidad de contagio. Este tipo de asistencia no presencial nos ciega a muchas cuestiones que literalmente no vemos. 

Por todo esto creo necesario aportar humildad. No es un tiempo fácil para nadie, para los sanitarios menos todavía. Creo que además de para nuestro pacientes debemos estar disponibles para aportar a quien nos lo solicite, sean directivos sanitarios, políticos o periodistas. Tratar de aportar evidencia científica y sensatez siempre que podamos. Y si me permiten también un toque de esperanza. Hemos pasado lo peor de esta pandemia. Hay que seguir avanzado, de esta sin duda vamos a salir.







"In health, you are in charge but you don't know", we health professionals don't know much either.



A few days ago, 55 Spanish scientific societies wrote a manifesto addressed to the President of the Government and those of the Autonomous Communities criticising the way in which the current pandemic has been managed. 



I can agree with some points of the Decalogue but I think it is fair to acknowledge that we health professionals do not know much about pandemics either. Let me explain.



1. They are rare events and when they are as widespread as the current one they overwhelm the capacity of the health systems and society. Some epidemics are easier to control than others; respiratory virus pandemics are among the most complex to manage when their level of contagion is high. 



2. Scientific evidence is the main source of light for health systems. In the current pandemic we have been kept in the dark for several reasons. The main reason is the poor quality of the data, and the secondary reason is the difficulty of carrying out enough research. 


3. The times to develop scientific evidence are long. For example, the development of vaccines requires months to years at best. 


4. Where I believe we can learn is in improving transparency and data management on the one hand, and in communication and coordination between the political, technical and social levels on the other. 



I will give you, for example, the famous curve of those infected with the two waves, which is an inconsistent image because the measurement conditions are different in the first phase and in the second.

As we can see, one curve has little to do with another, which is proof of how easy it is to manipulate the data, or simply not to correct it. 


If we add to this reality that there are not enough studies to inform us about issues of basic mask handling, distance, aerosol transmission and so on, the level of confusion will remain high. 

In health centres we are also doing a lot of work on the telephone to protect patients from going to a place with a high possibility of contagion. This type of non-presential assistance blinds us to many issues that we literally cannot see. 

For all these reasons, I believe it is necessary to bring humility. It is not an easy time for anyone, least of all for the health workers. I believe that in addition to our patients, we must be available to contribute to anyone who asks us, whether they are health managers, politicians or journalists. Trying to provide scientific evidence and good sense whenever we can. And if I may also add a touch of hope. We are past the worst of this pandemic. We must continue to move forward, from this we will undoubtedly emerge.
