Esta semana se ha inaugurado con gran boato el hospital de pandemias. Presupuestado en 50 millones dicen que ya va por 100 de sobrecoste, y no está terminado... No puedo evitar sentir que la situación es similar a cuando un coche accidentado como siniestro total sale del taller con un repaso de chapa y pintura pero sin tocar la mecánica, lo esperable es que no funcione.
Para un hospital de este tipo ya teníamos IFEMA, un espacio modulable capaz de dar un servicio de emergencia si se precisase a un coste bastante más ajustado. La chapa y la pintura.
Dentro de la Comunidad de Madrid muchos centros de salud lo están pasando muy mal. Carabanchel, la Elipa, Parla, el Álamo... Llevan muchos meses con grandes déficit de personal no cubiertos, especialmente gravosa la situación de médicos agotados por tener que asumir los cupos de pacientes que llevan meses sin suplente.
Para solucionar este problema no se ha invertido nada. Ni siquiera las medidas de equiparación salarial con los hospitales se han hecho efectivas, y mira que era fácil. Pero, ¡ay fatalidad! falta un papel de hacienda... Y de reforzar nada de nada y mira que se les ha llenado la boca a los responsables anunciando medidas que no se han concretado.
Un servidor pensaba que teníamos tormenta perfecta en un mes, para muchos me temo que ya está aquí.
Cualquier profesional sanitario sabe que una parada cardiorrespiratoria es un chungo. Y que nuestra sanidad está en estos momentos más de UCI que de consulta externa. Por mi parte coincido con muchos compañeros en que la Atención Primaria está en muerte técnica tras penosa enfermedad y larga agonía, y ahora ¿qué hacemos?
Lo primero pedir ayuda, pero como algunos llevamos dando gritos desde hace años y parece que esto sirve de poco habrá que hacer algo más. Propongo rescatar el clásico ABC de las urgencias: abramos la vía aérea, ventilemos y hagamos compresiones torácicas.
La vía aérea está cerrada. Por el cabreo, el cansancio, el agotamiento, la ansiedad, la tristeza, la incapacidad de aguantar tanta presión, por el maltrato institucional, por la desidia de los políticos, por la fatal de liderazgo de los gestores intermedios, por nuestra incapacidad para ponernos de acuerdo, por la impotencia para que se nos tenga en cuenta...
Para abrirla tendremos que reconocer que los tejidos están muy inflamados y meter con cuidado un tubo rígido que deje entrar el aire. Ese tubo son los valores, la ética, el ejemplo de nuestros maestros y compañeras que se han dejado la piel, incluso llegando a perder la vida. Apoyémonos en ellos.
Ventilar es meter aire y no hay mejor aire que la ilusión, la esperanza, la certeza de que tras este mare magnum vendrán tiempos mejores, la sensación de que no estamos solos, estamos rodeados de equipos que nos quieren y valoran, de una población que nos conoce, necesita y cuenta con nosotros. El aire de saber que somos custodios de una profesión cuya misión es cuidar, ayudar, proteger, acompañar a los más vulnerables, aquellos que padecen enfermedad, dolor o sufrimiento.
Y ponernos a aportar golpes de energía a un tórax bloqueado para volver a conseguir latido. Comprimir con todas nuestras fuerzas para poder levantarnos de la cama un día mas, para cubrir otro turno de servicio, para seguir sirviendo en nuestro puesto. Seguir dándolo todo para que lo fundamental no quede sin hacer, para que nuestras comunidades puedan seguir sintiéndose seguras.
Y mientras ventilamos y comprimimos alguien tendrá que seguir pidiendo ayuda. Tal vez con una llamada a la alcaldesa o a gente del barrio, tal vez compareciendo ante los medios de comunicación, a lo mejor escribiendo un comentario en redes sociales, publicando un blog, filmando un vídeo con amigos. Crear una narrativa que explique lo que hacemos y anime a la sociedad a defender su Atención Primaria como servicio esencial que es.
Sin ayuda nuestro paciente no saldrá de la parada. Si tú como lectora o lector de estas palabras no te pones a ello el paciente se nos va, se nos va para siempre.

Primary Care Cardiopulmonary Resuscitation
Any healthcare professional knows that a cardiorespiratory arrest is a doddle. And that our healthcare system is currently more in the ICU than in outpatient care. For my part, I agree with many colleagues that Primary Care is dying a technical death after a long and painful illness, and now what do we do?
The first thing is to ask for help, but as some of us have been shouting for years and it seems that this is of little use, we will have to do more. I propose to rescue the classic ABC of emergencies: open the airway, ventilate and do chest compressions.
The airway is closed. Because of the anger, the tiredness, the exhaustion, the anxiety, the sadness, the inability to withstand so much pressure, because of the institutional mistreatment, because of the laziness of the politicians, because of the fatal leadership of the middle managers, because of our inability to agree, because of the impotence to be taken into account...
To open it we will have to recognise that the tissues are very inflamed and carefully insert a rigid tube that lets air in. That tube is the values, the ethics, the example of our masters and companions who have left their skin, even to the point of losing their lives. Let's lean on them.
To ventilate is to put in air and there is no better air than the illusion, the hope, the certainty that after this mare magnum better times will come, the feeling that we are not alone, we are surrounded by teams that love and value us, by a population that knows us, needs us and counts on us. The air of knowing that we are the custodians of a profession whose mission is to care for, help, protect and accompany the most vulnerable, those who suffer from illness, pain or suffering.
And to start giving energy to a blocked chest in order to get a heartbeat again. Compressing with all our strength to be able to get out of bed one more day, to cover another shift of service, to continue serving at our post. To continue to give everything so that the fundamental things are not left undone, so that our communities can continue to feel safe.
And while we are airing and compressing, someone will have to keep asking for help. Maybe with a call to the mayor or to people in the neighbourhood, maybe by appearing before the media, maybe by writing a comment on social networks, by publishing a blog, by filming a video with friends. Create a narrative that explains what we do and encourages society to defend its Primary Care as the essential service that it is.
Without help our patient will not leave the cardiac arrest. If you, as a reader of these words, don't get on with it, the patient will leave us, she will leave forever.

Mucho se ha escrito sobre gestión y gobernanza sanitaria, muchas buenas palabras, discursos, estudios y libros blancos. Unos y otros han lanzado propuestas, ideas, argumentos... han prometido cambios y mejoras, han lanzado protocolos y programas.
Pero nadie se ha tomado en serio la siguiente tesis: El sistema sanitario español es ingobernable.
Una aseveración tan simple nos ahorra muchísimo trabajo.
Mis argumentos:
1. Una megaestructura compleja basada en 17 subsistemas independientes no coordinados impide la implementación de planes, políticas y financiación coherentes.
2. Una sanidad pública basada en impuestos que ofrece servicios accesibles sin pago añadido por ellos termina saturada.
3. Los profesionales sanitarios tienen límites, no es posible estirarlos hasta el infinito. Además cada vez hay menos (por emigración a otros países y por jubilaciones).
4. Las estructuras de gestión no tienen apenas poder real para implementar cambios o mejoras.
5. La estanqueidad presupuestaria impide que se pueda reforzar la sanidad a expensas de otras partidas menos prioritarias. No ha sido posible ni siquiera con el desastre de la pandemia.
¿De qué nos sirve asumir esta hipótesis?
1. Nos permite centrarnos en lo que sí se puede cambiar, la microgestión de cada cual en su función y con sus pacientes y la gestión del malestar profesional.
2. Disminuye la pérdida de energía en forma de calor, tanto del sistema como (y sobre todo) personal.
3. A la hora de tomar medidas de protesta deja claro que las únicas opciones de los profesionales son dos: abandonar el sistema, acciones síncronas mayoritarias (por ejemplo que todos los médicos del país no hiciéramos labores burocráticas durante un mes). Todas las demás sirven de poco como bien lo demuestran las últimas décadas.
Conclusión
Si bien el papel del sistema sanitario es fundamental para una sociedad moderna, sus funciones dependen en una estructura compleja de recursos humanos. Maltratar a esos profesionales no es inteligente dado que la prioridad de los mismos deja de ser el servicio a los demás y pasa a ser la propia supervivencia. Es imprescindible trasladar a la sociedad su obligación de decidir qué reglas del juego quiere asumir en relación con los servicios sanitarios.
Posdata: si se asumiera la hipótesis como cierta, ¿qué hacemos con todos los cargos intermedios y de gestión?
On the ungovernability of the Spanish health system.
Much has been written about health management and governance, many good words, speeches, studies and white papers. Both have launched proposals, ideas, arguments... they have promised changes and improvements, they have launched protocols and programmes.
But nobody has taken the following thesis seriously: The Spanish health system is ungovernable.
Such a simple statement saves us a lot of work.
My arguments:
1. A complex megastructure based on 17 independent, uncoordinated subsystems prevents the implementation of coherent plans, policies and funding.
2. A tax-based public health system that offers accessible services without added payment for them ends up being saturated.
3. Health professionals have limits, they cannot be stretched to infinity. Moreover, there are fewer and fewer (due to migration to other countries and retirements).
4. The management structures have hardly any real power to implement changes or improvements.
5. Budgetary tightness prevents the strengthening of health at the expense of other less priority items. This has not been possible even with the disaster of the pandemic.
What is the use of assuming this hypothesis?
1. It allows us to focus on what can be changed, the micro-management of each person in their role and with their patients and the management of professional malaise.
2. It reduces energy loss in the form of heat, both from the system and (above all) from individuals.
3. When it comes to taking protest measures, it makes it clear that the only options available to professionals are two: to leave the system, or to take synchronous majority action (for example, that all the doctors in the country should not do bureaucratic work for a month). All the others are of little use, as the last few decades have shown.
Conclusion
While the role of the health system is fundamental to a modern society, its functions depend on a complex structure of human resources. Abusing these professionals is unwise, as their priority is no longer service to others but survival itself. It is essential to transfer to society its obligation to decide which rules of the game it wants to assume in relation to health services.
P.S.: if the hypothesis is assumed to be true, what do we do with all the intermediate and management positions?

El mar es lo que tiene, las olas vienen y van. En epidemiología suele pasar lo mismo, ya lo están viendo ustedes. Un servidor no es epidemiólogo, ni falta que le hace, pero sí puede aventurar que en enero, febrero y marzo tendremos otra ola. Con mucha probabilidad aumentarán los casos de covid y por lo tanto un porcentaje se pondrán enfermos y necesitarán hospitalización y algunos UCI. Veremos como la curva de ingresos y muertes aumenta.
¿Por qué habrá tercera ola?
Pues porque todos los años pasa lo mismo con las enfermedades respiratorias invernales (catarros, gripes y neumonías). El contagio es mayor con tiempo frío y seco.
Y este año en invierno tenemos la tormenta perfecta.
Centros de salud llenos por catarro y gripes.
Centros de salud llenos por covid.
Centros de salud llenos por vacunas de covid.
¿Se saturarán otra vez los hospitales?
Volverá a saturarse la Atención Primaria, luego las urgencias hospitalarias, posteriormente las plantas de hospital y por último las UCIs.
Seguramente veamos llenarse el nuevo hospital Isabel Zendal.
¿Qué podemos hacer?
Lavarnos las manos, ventilar, minimizar exposición pública con otras personas, uso de mascarilla y mantener distancias.
Mucho ojo con los encuentros con amigos y familiares no convivientes.
Caminar y salir al exterior todo lo que podamos en lugares no concurridos o por el campo.
Mantener el humor y el ánimo alto.
Abrazar y achuchar a los cercanos y telefonear y videoconferenciar a los lejanos (especialmente mayores y enfermos).

The sea is what it is, the waves come and go. In epidemiology the same thing usually happens, you are already seeing it. I am not an epidemiologist but I can assure you that in January, February and March we will have another epidemic wave. It is very likely that the number of cases of covid will increase and therefore a percentage will become ill and will need hospitalisation and some ICUs. We will see the curve of admissions and deaths increase.
Why will there be a third wave?
Because every year the same thing happens with winter respiratory diseases (colds, flu and pneumonia). The contagion is greater with cold and dry weather.
And this year in winter we have the perfect storm.
Health centres full of colds and flu.
Health centres full of covid.
Health centres full of covid vaccines.
Will the hospitals be overcrowded again?
The Primary Health Care will be saturated again, then the hospital emergencies, then the hospital floors and finally the ICUs.
We will probably see the new Isabel Zendal Hospital filled up.
What can we do?
Wash your hands, ventilate, minimise public exposure to other people, use a mask and keep your distance.
Be careful with meetings with friends and family who are not living together.
Walk and go outside as much as possible in uncrowded places or in the countryside.
Keep your spirits up and your mood high.
Hug and cuddle those who are close to you and telephone and video conference those who are far away (especially the elderly and the sick).
El uso de mascarillas es una medida más de prevención de la transmisión del virus que no
sustituye a las demás, por lo que su recomendación debe ir asociada a garantizar unas buenas
prácticas que avalen el cumplimiento de la distancia física. Cuando esta última no es posible,
cabe recomendar la utilización de mascarilla.
Una de las cosas que está dejando claro la pandemia es que somos vulnerables. En este momento tenemos un montón de gente enferma por Covid en los hospitales, todas las semanas mueren varias centenas y todavía tenemos para largo.
Sin embargo hay poca reflexión sobre los cuidados, o por lo menos la que hay no se visibiliza lo suficiente. Si el sistema sanitario no da más de sí ¿quién va a cuidar? ¿cómo vamos a cuidar? ¿es posible adaptarse a un nuevo paradigma de cuidados?
José Laguna es teólogo, músico y pedagogo, además de un amigo. Me llena de orgullo leer su última colaboración en los cuadernos de Cristianismo y justicia, un foro intelectual de primer orden. En el texto analiza la vulnerabilidad humana y la incapacidad para sostenerla de una sociedad basada en el éxito, el crecimiento y la autosuficiencia. Vemos en televisión lo que pasa con los ancianos en las residencias, con los inmigrantes hacinados o hundidos, con las colas de tantos excluidos... nos llegan noticias de amigos y familiares enfermos, en paro, con problemas económicos o personales... ¿quién cuida?

Vulnerability: care as a political horizon
One of the things the pandemic is making clear is that we are vulnerable. Right now we have a lot of people sick from Covid in hospitals, several hundred dying every week and we still have a long way to go.
Yet there is little reflection on care, or at least not enough visibility. If the health system does not give more of itself, who is going to care? how are we going to care? is it possible to adapt to a new paradigm of care?
José Laguna is a theologian, musician and teacher, as well as a friend. I am proud to read his latest contribution to the notebooks of Christianity and Justice, a leading intellectual forum. In the text he analyses human vulnerability and the inability to sustain it of a society based on success, growth and self-sufficiency. We see on television what happens to the elderly in nursing homes, to immigrants who are overcrowded or who have collapsed, to the queues of so many who are excluded... news comes to us of friends and relatives who are sick, unemployed, with economic or personal problems... who is taking care of them?
I invite you to watch the video and read the booklet. I believe that it can contribute a lot both in the socio-health world and in the rest of society.

La inflación nos quita dinero sin que nos demos cuenta, los políticos derechos, los empleadores dignidad, los comercios calidad. Son cambios habitualmente sutiles que no nos obligan a cambiar de postura. Ocurren con una progresión tan lenta que los convierte en invisibles hasta que pasa el suficiente tiempo y ya suele ser tarde para revertirlos.
De este modo muchas sociedades se han visto empobrecidas en los últimos años. Quizá sea verdad que hemos recibido servicios públicos diversos pero también que estos cada vez se pagan más con deuda y se desinvierte en ellos para que sean menos costosos.
Por otro lado los empleos van incrementando la carga laboral sin que aumente proporcionalmente la remuneración. El mercado laboral va cambiando eliminando puestos de trabajo y modificando otros. En general hay menos oportunidades y estas son en muchos casos peores que antes.
Si ponemos la televisión o nos conectamos a algún medio de información seremos bombardeados instantáneamente por crispación y noticias falsas, el mundo de la política es un epítome de zafiedad que compite con los realities y los tertulianos por un trozo del pastel de la atención. En el universo de las redes sociales pasa lo mismo, el que más grita es el que triunfa.
El hecho de dar más importancia a lo desagradable y lo corrupto ha eliminado la estética, la armonía y el arte de las primeras planas de periódicos y telediarios. El coste es brutal para cualquiera, nos han robado la belleza. Y en cuanto al reservorio natural de la misma qué diremos en un mundo donde cada vez paseamos menos, nos desplazamos en vehículos y pasamos menos tiempo al aire libre...
Pongamos también una pandemia en la ecuación y obliguemos al personal a taparse la cara con mascarillas, aumentar la distancia unos de otros para que corra el aire y evitemos los bares, teatros y demás. Apaguemos la vida cultural y social, obliguemos a permanecer en el domicilio con toques de queda.
La resultante de todos estos cambios es que nos hemos quedado sin alegría desangrados por tantas cuestiones. Cuando el objetivo de la vida no es otro que sobrevivir nos olvidamos de esas pequeñas cosas que hacen que el día valga la pena. Y estamos hablando de la parte rica del planeta, la que si abre el grifo tiene agua y la que cena caliente.
No verán ninguna manifestación reclamando alegría, ni a tertuliano o político reivindicándola. Solo los poetas, artistas, locos e infantes la necesitan como el aire, si alguien protesta algo seguramente sean ellos. Pero hay pocas probabilidades de que los saquen en un telediario, ya saben. Por eso comparto esta reflexión para que nos unamos a esa búsqueda de la alegría tan necesaria como imprescindible.
Quizá nos demos cuenta de que es necesario apagar un poco las pantallas, dejar de consumir como posesos, caminar un poquito más lentos, salir a tomar aire con más frecuencia y volver la mirada hacia lo bello, bueno y verdadero. No es difícil, tan solo hay que querer.

Inflation takes money away from us without us noticing, politicians rights, employers dignity, businesses quality. These are usually subtle changes that do not force us to change our position. They occur so slowly that they become invisible until enough time passes and it is often too late to reverse them.
In this way many societies have become impoverished in recent years. It may be true that we have received a variety of public services but also that these are increasingly being paid for with public debt and disinvested in them to make them less costly.
On the other hand, jobs are increasing the burden of labour without a proportional increase in remuneration. The labour market is changing by eliminating jobs and modifying others. In general there are fewer opportunities and these are in many cases worse than before.
If we turn on the television or connect to some information medium we will be instantly bombarded by tension and false news, the world of politics is an epitome of sapphire that competes with reality shows and talk shows for a piece of the attention pie. In the universe of social networks the same thing happens, the one who shouts the most is the one who succeeds.
Giving more importance to the unpleasant and the corrupt has removed aesthetics, harmony and art from the front pages of newspapers and news programmes. The cost is brutal for anyone, beauty has been stolen from us. And as for the natural reservoir of it, what can we say in a world where we walk less and less, travel in vehicles and spend less time outdoors?
Let's also put a pandemic in the equation and force the public to cover their faces with masks, increase the distance between them so that the air flows and we avoid bars, theatres and so on. Let's turn off cultural and social life, force people to stay home with curfews.
The result of all these changes is that we have been left without joy, bled dry by so many issues. When the aim of life is none other than to survive we forget those little things that make the day worthwhile. And we are talking about the rich part of the planet, the one that if you turn on the tap has water and the one that has a hot dinner.
You won't see any demonstrations demanding joy, nor any talk show or politicians claiming it. Only poets, artists, madmen and children need it like air, if anyone protests anything it will surely be them. But there is little chance that they will be featured on a news programme, you know. That is why I share this reflection so that we can join in this search for joy that is as necessary as it is indispensable.
Perhaps we will realise that it is necessary to turn off the screens a little, stop consuming like possessed, walk a little slower, go out to take the air more often and turn our gaze towards the beautiful, good and true. It is not difficult, you just have to want to.
Ayer mi amigo Ángel González celebró sus 12 años en Twitter, al rato me di cuenta de que compartía con él la efemérides. Y ya son años. En ellos han cabido muchas cosas, mucha gente, muchos proyectos y algunos resbalones. La maquinaria de la red social ha ido cambiando y el algoritmo ha manipulado progresivamente la conversación para beneficio de sus responsables y de la miriada de empresas que vive de la misma. Viene bien recordar que no es un servicio gratuito, pagamos con nuestros propios datos y, sobre todo, con la atención que depositamos en la herramienta.
Muchos se han dado cuenta de que ahora lo que más atención demanda es lo falso, zafio y feo, la bronca, los desplantes, los gritos. Y la red social pone rápidamente los focos allá donde hay sangre o suciedad, sabiendo que es lo más rentable para sus intereses.
Mientras más experiencia tengo menos me apetece decir nada. Algún pequeño comentario, algún poema. Mirar de vez en cuando lo que dicen perfiles escogidos y aprender de otros siempre que se pueda. Ejercer la potestad del silencio, la libertad de desconectar, la alegría de recordar que hay otros cerca.
Una higiene de la narrativa es necesaria. Igual que nos lavamos las manos a diario deberíamos lavar y cuidar nuestra forma de expresión. Aunque no tengamos mucha visibilidad los primeros beneficiarios de expresarnos con más consciencia, compasión y delicadeza seremos nosotros mismos.
Yesterday my friend Ángel González celebrated his 12th birthday on Twitter, and I soon realised that I was sharing the event with him. A long time. There has been room for many things, many people, many projects and some slips. The machinery of the social network has been changing and the algorithm has progressively manipulated the conversation for the benefit of its managers and the myriad of companies that live from it. It is worth remembering that it is not a free service, we pay with our own data and, above all, with the attention we put into the tool.
Many people have realised that what now demands more attention is the false, the ugly, the rudeness, the shouting... And the social network quickly puts the spotlight where there is blood or dirt, knowing that it is the most profitable for their interests.
The more experience I have, the less I feel like saying anything. Some small comment, some poem. To look from time to time at what the chosen profiles say and to learn from others whenever possible. Exercise the power of silence, the freedom to disconnect, the joy of remembering that there are others around.
A hygiene of the narrative is necessary. Just as we wash our hands daily, we should wash and care for our form of expression. Even if we don't have much visibility, the first beneficiaries of expressing ourselves with more awareness, compassion and delicacy will be ourselves.

Mientras mayor es el ruido de fondo menor es la calidad de una señal. Tal vez por eso cada vez comprendemos peor lo que sucede a nuestro alrededor. No es casual sin embargo que las cosas estén como están y que la bronca pública, la mentira y la falsedad reinen en todos los canales. Donald Trump ha sido un ejemplo y sus muchos seguidores han valorado su actitud y su maestría en el arte de hacer ruido. Pero miren donde miren encontraran voceros y fuentes de distorsión. La manipulación precisa hoy de estas herramientas, que nos rodean cada vez más y se nos cuelan hasta lo más profundo de nuestras vidas.
Por ponerles otro caso más cercano podríamos hablar del manejo político de la pandemia en España, asunto que ocupa buena parte del tiempo de televisiones, medios y redes sociales. Pues bien, más allá de lo que digan y hagan unos y otros aireando cifras de incidencia y de mortalidad, durante estos meses nadie parece haberse dado cuenta de un muerto que no ha entrado a formar parte de las estadísticas. Ese deceso discreto se llama Atención Primaria y muchos llevamos anunciando su fallecimiento desde hace tiempo. Elijo sin embargo el certificado de defunción que hace Juan Simó, un colega que admiro. Si una sociedad deja morir una institución que se supone creada para defenderla y cuidarla, parecería lógico que habría que montar un buen sepelio. Pero no ha sido así, se ha optado por unos y por otros en aceptar su transformación en zombi, algo ni vivo del todo ni muerto completamente. Un estado que sigue estando con nosotros, que ocupa espacio y da servicio pero que es tan solo un remedo sin vida de lo que fue.

The greater the background noise, the lower the quality of a signal. Perhaps this is why we understand what is happening around us less and less. It is not by chance, however, that things are as they are and that public anger, lies and falsehoods reign on all channels. Donald Trump has been an example and his many followers have appreciated his attitude and his mastery of the art of making noise. But wherever you look you will find spokespersons and sources of distortion. The precise manipulation today of these tools, which surround us more and more and are slipping into the depths of our lives.
To put another case in point, we could talk about the political handling of the pandemic in Spain, an issue that takes up a good deal of television, media and social network time. Well, regardless of what some people say and do, airing figures on incidence and mortality, during these months no one seems to have noticed a death that has not become part of the statistics. This discreet death is called Primary Care and many of us have been announcing his death for a long time. I choose however the death certificate made by Juan Simó, a colleague I admire. If a society lets die an institution that is supposed to defend and care for it, it would seem logical that a good burial should be arranged. But this has not been the case, some have chosen to accept their transformation into a zombie, something neither completely alive nor completely dead. A state that is still with us, that occupies space and gives service but is only a lifeless imitation of what it was.
